Herbal Drugs Used In Acne
1. INTRODUCTION
Acne is a disease that affects the skin's oil glands. The small holes in the skin
(pores) connect to oil glands under the skin. These glands make an oily
in
substance called sebum. The pores connect to the glands by a canal called a
follicle. Inside the follicles, oil carries dead skin cells to the surface of the skin.

A thin hair also grows through the follicle and out to the skin. When the follicle
of a skin gland clogs up, a pimple grows.
Acne is a skin condition that affects the hair follicles and the sebaceous
glands in the skin, which secrete an oily substance called sebum. Acne is
most common on the face, arms, back and chest.
The onset of acne is usually around puberty, but in a minority of cases it may
also start in adulthood. About 80% of people will have some degree of acne
between the ages of 11 and 30.
In puberty, acne occurs because of changes to hormone levels, which cause
the sebaceous glands to produce increased amounts of sebum. Together with
dead skin cells, the sebum blocks the hair follicles, which enables the

formation of 'spots' ranging from blackheads to painful red nodules.
As with the other physical changes that happen during puberty, acne usually

However, in some people this may take many years and can potentially cause

permanent scarring, so a variety of treatments are available.
Almost 100% of the people between the ages of twelve and seventeen have
at least occasional breakouts of whiteheads, blackheads or a pimple, now
and then. Most of these young people can manage their acne with, over the
counter treatments. But for some, acne is more serious. By their mid-teens,
nearly 35% of teenagers have acne severe enough to require some treatment
from a medical professional.

2. EPIDEMIOLOGY OF ACNE
Acne affects both young men and women, although the percentage of young
men with acne is a little higher. Young men are more likely to have more severe,
in
longer lasting forms of acne. Young men are less likely to visit a dermatologist for
their acne. Some young women are more likely to have occasional acne flair-ups

Prevalence followed a significant linear trend over age with peak prevalence

3. TYPES OF ACNE
A. Mild to Moderate acne vulgaris consists of the following types of acne

on the surface. These types of acne lesions sometimes seem to be begging to
be popped. Make sure you read about how to pop a pimple before you attempt
this. Whiteheads are normally quicker in life cycle than blackheads.

 Papules: Papules are inflamed, red, tender bumps with no head. Do not
squeeze a papule. It will do no good, and may exacerbate scarring. The

red circle with a white or yellow center. Pustules are your garden-variety zit.

Nodules and cysts characterize severe acne vulgarism:
 Nodules: As opposed to the lesions mentioned above, nodular acne consists
of acne spots which are much larger, can be quite painful and can sometimes
last for months. Nodules are large, hard bumps under the skin's surface.
Scarring is common. Absolutely do not attempt to squeeze such a lesion. You
may cause severe trauma to the skin and the lesion may last for months longer
than it normally would. Dermatologists often have ways of lessening swelling

 Cysts: An acne cyst can appear similar to a nodule, but is pus-filled, and has

painful. Again, scarring is common with cystic acne. Squeezing an acne cyst

B. Severe Acne:
Severe acne can affect many facets of a person’s life, causing a great deal of
embarrassment and stress. Severe acne may significantly limit one’s social life

4. SYMPTOMES OF ACNE
Acne spots appear in areas of skin that have a large number of sebaceous
glands. They appear most often on the face, but can also form in other places

Acne consists of several different types of 'spot', which can be roughly divided as
follows:

Blackheads - tiny dark coloured blocked pores. The black of the
blackheads is due to skin pigment, not dirt.

Whiteheads - small, firm bumps with a white centre.

Pustules - pus-filled spots with an obvious balloon of white pus, which may
turn yellow as the spot begins to heal.

Nodules - hard lumps under the skin that can be very painful, go deep into
the skin and often cause scars. These are the most common type of spot
found in very severe acne, and they often run together in groups. When

5. CAUSES OF ACNE
Acne that begins at puberty is the result of increased sensitivity to the hormone
testosterone, which is present in both boys and girls. Testosterone causes

1) Role of Androgens
Androgens (male sex hormones) stimulate growth of sebaceous follicles and
enhance production of sebum. After puberty, boys produce 10 times as much
androgen as girls; therefore it is not surprising that more boys than girls
develop severe cases of acne. During the second decade of life, sebum
production closely parallels both androgen production and the presence of
acne. Although testosterone is the most potent androgen, its metabolites and
weaker androgens are increased in acne patients and may stimulate
sebaceous gland activity.
2) Sebum Production

Sebum is produced in the sebaceous glands and consists of glycerides, wax
esters, squalene, and cholesterol. At times, a relationship between increased
sebum secretion and severity of acne is present. The glyceride component of

P. acnes is part of the normal flora in the sebaceous follicle and plays an
important role in the initial development and maintenance of the inflammatory

6. PATHOPHYSIOLOGY OF ACNE
The causes of acne are multifactorial and not completely understood. Theories
on
the
causes
of
acne
include
the
role
of
androgens
(hormones),
Propionibacterium acnes, sebum production, and follicle growth.
Acne is generally limited to the parts of the body that have the largest and most
abundant sebaceous glands - the face, neck, chest, upper back, and upper arms.
These sebaceous glands normally produce sebum, a substance which

moisturizes the skin. Acne lesions are typically centered about the pilosebaceous
follicles, which open onto the skin surface as the pores of the skin.

Sebaceous glands develop at puberty in response to androgen stimulation and
are attached to the follicle canal by sebaceous ducts. Acne is believed to result

from a change in the inner lining of the follicle that prevents sebum from passing
through. For reasons not completely understood, cells from the lining of the
follicle are shed too fast and clump together. The clumped cells plug up the

follicle's opening so sebum cannot reach the surface of the skin. One of the jobs
of the sebum is to carry cells shed by the glands to the surface of the skin. But
because the excess sebum is blocking the openings of the glands, called ducts,
both cells and sebum accumulate. This mixture of oil and cells causes bacteria
that normally live on the skin, called Propionibacterium acnes (P. acnes), to grow

cause inflammation, which is a characteristic reaction of tissues to injury and is
