2015 SOUTHMOORE FOOTBALL YOUTH SKILLS CAMP
APPLICATION FORM
The Southmoore Football Youth Skills Camp is a non-padded camp designed to help develop
fundamental football skills in a supervised, organized, and enthusiastic environment. Each
camper will gain knowledge and skills through demonstrations and drill sessions taught by
Southmoore High School Football coaches and players. They will be grouped according to
age or ability. Campers will also have the opportunity to hear from former SaberCat
players who are playing at the next level.

to compete in all camp activities without restriction. In addition, I acknowledge that I have medical
insurance to cover the cost of any injury or illness that may occur during my child’s participation in this
camp. Furthermore, I authorize the Southmoore Football Staff to act for me according to their best
judgment in an emergency requiring medical attention. I hereby release the Southmoore Football Staff and
Moore Public Schools from any civil or monetary damages due to injury sustained while attending this camp.

All campers will receive a camp T-shirt. Enrollment may take place the first day (5:30pm) of the camp at
the rate of $60 per camper. Campers should report to the Southmoore High School (2901 S. Santa Fe)
Football Fieldhouse (Southwest Corner of Campus) ready to participate. Confirmation of enrollment will be
sent via email.
