Hastings Area Rotary Merit Scholarship
General Instructions
FOR ADAMS COUNTY HIGH SCHOOL SENIORS WHO ARE INVITED TO
PARTICIPATE IN THE ROTARY LUNCHEON ONLY. Adams county high
school Graduating Seniors invited to the Rotary Luncheon and a citizen
of Adams County, may file an application. On or before April 20, 2015,
three (3) copies of the application must be mailed or delivered to:

Awarding of this scholarship will be at the Hastings Area Rotary
Luncheon on May 8, 2015. Luncheon is held on the CCC-Hastings
Campus.
Scholarship and leadership are the criteria by which applicants will be
judged. Only students of outstanding merit, who show an appreciation
of the value of an education, and who are willing to struggle to achieve
success, have a chance to win our awards.
Hastings Area Rotary Merit Scholarships are for one year and the award
is $1,000.00.
After completing this application, send or deliver three (3) copies to Dr.
Ed Lockwood at address above and retain a copy for your records.
The following Merit Standards will prevail:
 Scholarship _____________________ 600 points
 Leadership ______________________ 300 points
 Community Involvement ___________ 100 points

General Instructions
1. Applicant must use the official Hastings Area Rotary Merit Scholarship application form. It is to be signed
and dated by the student, and parent(s) or guardian. Submit three (3) copies. Sign all three (3) copies. The
application must be completed on a computer and printed or typed.
2. Applications and all supporting documents must be in English or English translations.
3. Recommendation letters must be originals on one side of a single sheet of 8.5” by 11” paper signed by the
author.
4. Official high school transcript of student records from the beginning of the ninth grade to due date of
application may be photocopies that bear an original signature of the proper school authority.
5. SAT and/or ACT test scores must be included but may be photocopies. It is preferable that exhibits such as
awards and media items be photocopies.
6. ALL applications must be properly bound on the left side in a standard binder or cover. Elaborate bindings
are discouraged. Clear plastic or bulky covers are undesirable.
7. Counting the Rotary application as one page (4 actual pages), the completed brochure must not exceed 20
pages (one side only) or 10 ounces in weight.
8. The application must be arranged in the follow order:
a. Application form.
b. The applicant shall prepare a personal statement of 300 words or less setting forth his or her
vocational or professional goals and relate how past, present, and future activities make the
accomplishment of these goals probable. The applicant by deed and circumstance must
demonstrate his or her worthiness. The personal statement must be signed.
c. Official signed transcript of high school grades.
d. Required college entrance test scores.
e. Current dated, signed one-page letters of recommendation from at least one person, but not
totaling more than three, in authority from high schools attended by the applicant. Letter may
cover the applicant’s ability, work habits, leadership, personality, and integrity.
f. One letter of endorsement from responsible community (i.e. non-school) person. This person
should not be related to the applicant. They should be capable of reporting on the applicant’s
participation in the community in terms of work service, leadership, notable skills, and outstanding
recognition. The letter should be completed on a computer and printed or typed, dated, signed,
and no more than one side of an 8.5” by 11” paper.
g. Copies of exhibits of achievement in scholarship, leadership, athletics, dramatics, community
service, or other activities may be attached but the applicant should avoid being repetitious.
