2015/2016 Registration Information
On behalf of the 2015/2016 Board of Directors, we hope your summer is going well. It’s time to
start thinking about another hockey season.
Your 2015/2016 Board of Directors consists of:
Steve Crandall, Gary Cutsy, Peter Howard, Ken Jakubowski, Nick Niemiec,
Greg Panaretos, Wally Pivetta, Matt Sharkey, and Bob Toaso
Officer Positions:
President – Pete Howard
Treasurer – Mike Hermann
Vice President – Steve Crandall
Secretary – Dan Hunt
To have a successful season, we need you to fill out and return the attached Registration and
Acknowledgment of Risk Form along with your check.

REGISTRATION DEADLINE: FRIDAY, JUNE 26, 2015
All material must be postmarked by this date or your registration will not be accepted. If
you are planning a leave or to retire, please send back the form by the deadline so we
can update our records. Please remember that playing in our league is a privilege; there
are over 95 people on our waiting list.
