STUDENT INSTRUCTIONS about LICENSURE
WEST VIRGINIA APPLICATION
1. Fill out the white West Virginia Application using the instructions provided with the form and this information.
READ ALL INSTRUCTIONS CAREFULLY! TYPE OR PRINT CLEARLY IN INK.
2. Use this information to complete Question #18.

3. You will need one 2x2 passport style photo.
4. Sign the photo across the bottom front and sign the application (middle of Page 4) in the presence of a notary.
(Photo will be attached to application after Student Services completes page 5.) Jane Dailey, Brenda Deane,
Betty Jurick, Lorie Stowers, and Brandy Toothman are notaries, or use a notary in your local community.
5. Certification I must be completed by a person who has known you for at least 5 years (NON-family
member).
6. Pages 1-4 must be completed before turning the WV Application into Student Services Office.
7. Certification II will be completed by Student Services; we will obtain the Dean’s signature and return to you.
You will mail the application once it is completed.
8. Attach a money order or certified check for $70.00 made payable to WV Board of Examiners for Registered
Professional Nurses. Personal checks are NOT acceptable.
NCLEX EXAMINATION REGISTRATION
1. You must register for the NCLEX online through the following website: http://www.vue.com/nclex/.

WVU Transcript Request
You will request transcripts through your MIX account. It may take at least two weeks after graduation for your
final grades and degree to be posted on your transcript. Please check your MIX account regularly and order your
transcript ONLY AFTER your grades and degree are posted. If applying for boards in West Virginia, you will
need to have one copy of your transcript sent to the WV Board of Nursing at “WV Board for RN’s, 101 Dee
Drive, Charleston, WV 25311-1620.” At the “Select Recipient 1 Type” screen, select the “Other” option and enter
in the address above. The “Special-Purpose Recipient” screen does not have the address for the WV Board of
Nursing.
**The Student is responsible for any outstanding WVU fees (all fees must be paid or a transcript will not be sent.)
OUT OF STATE BOARDS
1. Find the address for the Board of Nursing in the state where you wish to be licensed from the National Council
of Boards of Nursing, Inc. at their web site- http://www.ncsbn.org. Student Services will assist you as needed.
2. Call or write that Board to obtain an application.
3. READ INSTRUCTIONS CAREFULLY! Follow the instructions received with the application. Remember
forms that need to be notarized must be signed in the presence of a Notary. Don’t forget to request a transcript,
if a transcript is required. You are responsible for mailing out applications.
IMPORTANT
Before mailing your forms, make sure you know in which state you wish to be licensed. Most state fees are
not refundable. NCLEX will assess you an additional fee to change the state you marked in item #22. If you
change the state where you are going to take your boards, Student Services must also be notified.
