THE 2015 ROBERT W. TEETER AWARD NOMINATION FORM
Nominations are due Friday, March 20th at 5pm.
To nominate an individual for the 2015 Robert W. Teeter Award, a written statement must be submitted to the
Pav YMCA. The written statement should be detailed and clearly outline how the candidate has influenced and
dedicated themselves to our community. Additional information may be submitted if desired. Selection of the
award recipient is based on this written statement.
The recipient of the 2015 Teeter Award will be notified by the end of March. The Teeter Award will be presented
on Friday, May 8th at the Pav YMCA’s Annual Teeter Award Celebration and Dinner Dance.
