How to Use this Directory
In this directory, Providers are listed by county in alphabetical order by specialty to
make finding a provider in your area easy. You will find an array of services available
based on your personal needs. These may include:

Adult Day Care: Caregiver support and respite care in an active, socialized
setting.

Adult Family Care Home: Provides 24 hour supervision, assistance, socialized
peer interaction, dining and housekeeping services.

Assisted Living Facility: Provides 24 hour supervision, assistance, socialized
peer interaction, dining and housekeeping services.

Care Management: Evaluation and coordination of services

Emergency Response System: Provides 24 hour access to emergency
services

Home Health Care: Bath visits, homemaking, companion and respite care.

Home Delivered Meals: Nutritious meals delivered to the home.

Medical Equipment and Supplies: Incontinent supplies and nutritional
supplements.

Nursing Home: Provider 24 hour supervision and assistance
There is a share of cost associated with Assisted Living Facility, Adult Family Care
Home and Skilled Nursing Facilities.

for anyone who meets the state eligibility requirement. There is a share of cost
associated with Assisted Living Facility, Adult Family Care Home and Skilled Nursing
Facilities.
Humana American Eldercare is required to inform members and potential members
of any restrictions on a counseling or referral services based on moral or religious
ground beliefs. Some providers may choose not to perform certain services
based on religious or moral beliefs.
