2.Print your name, address, city, state, zip code, phone number, email address, and the number of participating products purchased
on the official offer form below.
• To receive a $60 mail-in rebate check, send original receipt(s) showing proof of purchase(s) of one (1) five-gallon pail
of Mobil Delvac 1™ ESP synthetic heavy-duty diesel engine oil. Limit two (2) $60 pail rebates per household.
• To receive a $12 mail-in rebate check, send original receipt(s) showing proof of purchase(s) of one (1) one-gallon jug
of Mobil Delvac 1 ESP synthetic heavy-duty diesel engine oil. Limit five (5) $12 jug rebates per household.
• To receive a $25 mail-in rebate check, send original receipt(s) showing proof of purchase(s) of one (1) five-gallon pail
of Mobil Delvac™ 1300 Super heavy-duty diesel engine oil. Limit two (2) $25 pail rebates per household.
• To receive a $5 mail-in rebate check, send original receipt(s) showing proof of purchase(s) of one (1) one-gallon jug
of Mobil Delvac 1300 Super heavy-duty diesel engine oil. Limit five (5) $5 jug rebates per household.
3.Mail original receipt(s) from a participating retailer dated between March 1, 2015, and May 31, 2015, with eligible

per household for the $60 rebate offer; limit five (5) $12 jug rebates per household for the $12 rebate offer; limit two (2) $25 pail rebates per household for the $25 rebate offer; and limit five (5) $5 jug rebates per household for the $5 rebate offer. Limited to purchases made between
March 1, 2015, and May 31, 2015, at participating retailers posted online at mobildelvac.com/rebate. All rebate requests must be postmarked by June 30, 2015, and received by July 15, 2015. All online submissions must be completed by 11:59:59 CST June 30, 2015.
Allow approximately six to eight weeks for delivery. Rebate valid only to individual consumers; requests from groups, clubs or organizations will not be honored. Rebate cannot be combined with any other offer, coupon or discount card. Incomplete, damaged, photocopied, mechanically
