Tournament/League. I acknowledge that I have received a copy of the official rules and regulations for Rebounderz Indoor Trampoline Arena Dodgeball
Tournament/League. I hereby certify that I am in good physical condition and that a licensed physician has verified that my physical condition is at a sufficient
level to enable me to participate in the nature of the activities safely. I agree to follow all instructions, rules and regulations of Rebounderz regarding the use of
its facilities while engaged in its Dodgeball activities.
