By signing below, I acknowledge my understanding that there is a certain amount of risk involved
in Irish dancing and release Rising Tide Irish Dance Academy and its instructors from liability in the
case of an accident or injury to myself or my child(ren). I confirm that I, or my child(ren), am/is/are
in good physical condition and am/is/are able to participate fully in the classes. I have outlined all
medical alerts that Rising Tide Irish Dance Academy should be aware of on my completed registration form. I understand and acknowledge that the instructors at Rising Tide Irish Dance Academy
will be conducting classes in as safe a manner as possible.

Rising Tide Student Code of Conduct - Spring/Summer 2015
As a student at the Rising Tide Irish Dance Academy, I commit to:

• Taking responsibility for my behaviour by:
• Arriving on time to every class with proper footwear and attire, prepared to learn

Spring/Summer 2015 payment policy
Registration
A registration, waiver form and code of conduct contract must be completed and submitted prior to or on the first
night of class. In the case of a family with multiple dancers, each dancer must have their own forms. Forms can be
found on our website. Completed forms can be scanned and emailed, mailed, or brought to the first night of class.
Payment methods
Payments are due the first class of the term as cash or cheque. Cheques are payable to Rising Tide Irish Dance Academy. Classes are based on $15/hour. You may choose to pay term fees in a lump sum, or in four (4) cheques post-dated
to the first of every month in the term (i.e. January 1). If payment is not received by the last day of the first month of
the term (i.e. January 31) dancers will not be permitted to enter class until payment is received. Drop-in fees are due
at the beginning of every class.
If paying on the first night of class, please email or call ahead so we have a better estimate of numbers.
Cancellation and refunds
Rising Tide Irish Dance Academy reserves the right to cancel classes if a minimum enrollment of four dancers is not
met. If your class is cancelled, a full refund will be given. Refunds will be granted should a dancer decide to withdraw
from classes. A full refund will be given if the dancer withdraws prior to the start of a term. Once the term begins,
a refund will be granted for the remaining unattended classes. Please note, non-attendance does not constitute a
withdrawal; please notify us directly if you wish to withdraw.

Coaching fee: All competitive dancers with two dances in Novice or above will be charged a coaching fee. The fee is 10% of a dancer’s
class tuition (before taxes), and is due at the beginning of each term. This fee is included in the intermediate and championship term
fees listed above only. Note that intermediate and championship students paying on a per-class basis will also pay the coaching fee
