charges will apply.
• A fee of $1.99 per kilogram applies if patient is greater that 20kg at time of surgery.

CAN I CHANGE MY PLAN?
Yes! You may change your plan at any time.
CAN I CANCEL MY PLAN?
You may cancel the plan at any time. Simply
provide your request in writing at least 30 days
prior to the desired cancellation date. You
are responsible for the services used less the
monthly payments made if canceling during the
term.

require payment for coverage that you may never use, our
