Read for you... Publications EAO congress enthusiastic about SKY elegance abutments 40 Years of Dental Innovations Interview with Peter Brehm - CEO of the bredent group Marianne Steinbeck, Project Manager, BDIZ EDI konkret / EDI Journal The 40th anniversary is the culmination of the company‘s history as one of the last family-run and yet internationally operating key companies in the dental sector. Initially a specialized dentaltechnical provider, bredent has now become a supplier of comprehensive implant-prosthetic materials and technology - from the high-performance polymer BioHPP to the closed digital workflow. Marianne Steinbeck, Project Manager BDIZ EDI konkret / EDI Journal uses the forthcoming date for a conversation with the CEO, Peter Brehm. Marianne Steinbeck: bre-dent, that is Brehm-Dental, the name says it all. How can a family-run company that is located in the Hochlohnland in Germany maintain its strong position in the dental market which has been highly competitive especially during the past few years? Peter Brehm: It is the claim of the bredent group to be among the best and to be a role model in the dental sector. For this purpose it is necessary to have one’s ears and eyes wide open to the market but also to the own company so that we can identify the needs of our customers and employees and fulfill them. The resulting information creates the basis for continu- ous innovations based on quality „Made in Germany“. This promise is still synonymous for highly qualified employees and profound know-how, production with local roots and precision craftsmanship. In combination with the abundance of ideas and the suggestions of our customer, the resulting solutions offer real added value to assert oneself against competitors and to avoid the fact that the price becomes the sole decision criterion. Marianne Steinbeck: How often do you have discussions based on the motto „cheap is cool“? Peter Brehm: Such discussions are always held if one can be reduced to a single product. The bredent group which consists of bredent to preserve the oral health, in particular the hygiene, using our therapeutic concepts and system solutions. It is essential to restore the health with esthetic affordable restorations that are easy to clean and to make sure that the profit which is necessary for practices or laboratories can be achieved. An innovative and helpful example in this context is the SKY fast & fixed therapeutic concept which enabled us to offer a standardized protocol ranging from implant placement to the temporary restoration to patients of the generation „50 plus“ who are facing edentulousness. They can leave the practice on the same day and have a fixed implant-supported restoration. The resulting economic benefits do not only convince the patients concerned but also the attending dentists and dental technicians. As a result, the discussions mentioned before can be avoided. Marianne Steinbeck: After purchasing the SKY implant system from Prof. Manfred Lang in Decem- and bredent medical considers itself a full-line dental provider which offers harmonized therapeutic concepts and system solutions to surgeons, prosthodontists and dental technicians as well. The resulting symbiosis of dentistry and dental techniques puts the focus exclusively on the benefits, wishes and needs of the patients. It is important for us to enable the dental team consisting of the dentist and the dental technician Peter Brehm CEO bredent group ber 2004, bredent soon involved the dental technician. A brilliant idea, as we know today. What was your motivation to be a pioneer? Peter Brehm: Especially in the field of dental techniques, the bredent group can look back on 40 years of own experience and manufacturing. The fact that this know-how is reflected in the implant business from the beginning with the focus on implant prosthetics, is the logical consequence and only defines what should be combined in reality. We are very grateful to Dr. Bayer and his colleagues from Landsberg - they introduced the SKY fast & fixed concept in our company and entrusted us with the project of the implant-prosthetic symbiosis. Marianne Steinbeck: With the SKY fast & fixed concept you have opened a highly innovative segment once again and you have gained renowned international experts and users for this system - where do you see the special indication and advantages in the practical use? Peter Brehm: As already mentioned, SKY fast & fixed is a comprehensive therapeutic concept to offer patients who are facing edentulousness an affordable fixed implant-supported bridge restoration - in numerous cases within one day - and to help them regain lost quality of life, self-esteem and self-confidence. If these patients (normally between 45 and 65 years old) are informed, that their last teeth need to be extracted, this is frequently accompanied by the stigma of being old 40 Years of Dental Innovations and useless and the loss of quality of life, self-esteem and self-confidence. After all, this group of individuals, is frequently socially and professionally exposed and simply feels that they are too young to wear a removable denture. The use of SKY fast & fixed allows to offer immediate restorations to these patients with a reduced number of implants and prosthetic components as well as matched dental materials. This concept has proved its reliability in more than 20,000 patients and its effectiveness for immediate implant restorations has been confirmed for more than seven years. Although almost all implant manufacturers offer similar components now, we are still the only company that offers all materials - from the implant to the temporary and definitive restorations -, e.g. with the visio.lign veneering system. For the dental practice, this standardized concept means that the entire team will be familiar with the work processes and that each patient can be treated in the best possible way with a limited amount of surgery. Moreover, thanks to successful referral marketing, SKY fast & fixed has become a source of growth for numerous practices since satisfied patients who were convinced by the therapeutic concept sent new patients to the practice, who also wished to receive fixed teeth. Peter and Brigitte Brehm in 1974 beginning of a development in which we - as the bredent group want to play a key role. Marianne Steinbeck: As far as events are concerned, and this applies certainly also to the International SKY Meeting in May 2014, you have repeatedly emphasized the hygiene capability of the implant-prosthetic restorations; you are focusing on biofilm management and innovative, more resistant materials ... SKY fast & fixed: therapeutic success based on more than 7 years of experience and 20,000 patients treated worldwide Marianne Steinbeck: In your opinion, what will be the new trends in prosthetics, and especially in implant prosthetics in the next few years? Peter Brehm: New prosthetic materials based on high-performance polymers, such as the ceramicreinformce BioHPP, are going to change prosthetics drastically within the next few years. We have been working with these materials for years and - with the for 2 press vaccum press device and BioHPP - we offer a system solution to make sure that laboratories can process the new material in a dependable and safe manner. The enormous demand after the IDS 2013 showed that we have identified the latest trend and posed a major challenge for our logistics system. Numerous users have immediately recognized its inherent potential - whether as an individual SKY elegance abutment with white shade and off-peak properties, onto which, for example, a ceramic crown can be attached, or as a monolithic crown abutment for which BioHPP which is pressed to the titanium base without the formation of a gap is directly veneered with the visio.lign veneering system. Thanks to it elasticity which is similar to that of natural bone, BioHPP provides tremendous advantages, regardless whether screw-retained or telescopic restorations need to be fabricated. Here we are at the Photo: MDT Oliver Heinzmann, Heppenheim Peter Brehm: The biofilm and the hygiene capability are in fact our major concerns. There is still a lot of effort required by dentists and dental technicians. Above all patients must be better informed about the risks of the biofilm to the general health. To some extent, this was also one of the reasons why we have integrated the antimicrobial photodynamic HELBO therapy (aPDT) into the bredent group, We have wanted and still want to establish scientific proven procedures for almost painless control of infections in the biofilm all over the world for more than ten years. As a global leader of individual, transversal screw-retained restorations we consider it our task to 40 Years of Dental Innovations advance the development in the field of partially removable restorations because we believe that this is the only way to ensure permanent success of the implant restorations. BioHPP framework - Labor Schwindt, Landau technologies“, „Immediate restorations with SKY fast & fixed“ as well as „Digital Workflow“ will be outlined and discussed with renowned international lexturers and, of course, participants from May 22 to 24, 2014 during the bredent group days SKY Meeting 2014 in Berlin. Marianne Steinbeck: We look forward to many more years and ideas and thank you for this interview. Antimicrobial photodynamic therapy (aPDT) according to the HELBO procedure This subject and the related subjects „Esthetics through innovative materials and Innovations Based on Therapeutic Concepts SKY elegance abutments - the smart way to immediate or delayed restorations with BioHPP high-performance polymer bredent medical has opened new horizons in implant prosthetics by introducing the new prefabricated SKY elegance abutments. The SKY elegance abutment combines both the properties of a temporary and a definitive abutment and hence allows drastic reduction of the treatment steps. As a result, maximum protection of the gingiva is guaranteed and the treatment time is reduced for users and patients as well. The SKY elegance titanium base, which was presented during the IDS 2013 and which enables dental technicians to fabricate individual hybrid abutments made of BioHPP and titanium, has exceeded the high expectations by far. The properties of a material combination consisting of a titanium base and a pressed-on coat of the ceramicreinforced high performance polymer BioHPP have convinced the users. The „off-peak“ and „white shade“ material properties result in a reduction of the forces on the implant, minimized stress of the bone and avoidance of dark shadows in the gingiva to ensure perfect esthetic results. Moreover, the soft tissue responds very positively to BioHPP, in particular as far as gingival attachment is concerned. These properties are now also of- does not need to be removed. The gingiva is not traumatized. Conventional or digital procedures can be used for the impression and the fabrication of crowns, which means that the SKY elegance abutment is perfectly suitable for the applications like a zirconium abutment. fered by six different prefabricated types of the SKY elegance abutment. Since the bone-like properties of the SKY elegance abutment provide protection against overloading, it can be placed immediately during surgery and a temporary restoration can be fabricated in cases of adequate primary stability. The definitive restoration can be fabricated a few weeks later and the abutment digital workflow using an intraoral scanner and CAD/CAM fabrication thanks to the suitability of BioHPP for scanning. Since it is not required to screw in any gingiva formers and impression abutments, users will save time during chairside treatment and the clinical results can also be improved. The SKY elegance abutment is, of course, also suited for delayed immediate restorations or for classic and the range of angled abutments - from 0 to 15 degrees - available in three different diameters (S, M and L) each gap can be closed so that only minor modifications of the gingival line are required. The prefabricated SKY elegance abutment can be used for crown and bridge restorations made of ceramic, metal and BioHPP or as a crown abutment in combination with the visio.lign veneering system. The six designs of the prefabricated SKY elegance abutments can be easily customized at the chairside. Thanks to the round basic shape Asked for you... Ensuring the long-term success of implants the focus of the bredent group days 2014 expressly on biofilm management and regeneration? Prof. Dr. Dr. Anton Sculean University of Bern Healthy, white teeth and a bright smile are not only symbols for beauty and attractiveness but there is also broad acceptance in studies associating healthy teeth and gums with the health of the entire organism and a series of serious systemic diseases. Together with users, the bredent group considers it a task to provide treatment concepts and systems to support proven methods for the preservation of the oral health, in particular functional hygiene, but also to offer restorations that are easy to clean and enable permanent functional and esthetic rehabilitation. The provider, which is normally associated with dental-technical and prosthetic hardware, therefore puts the focus of one of the main subjects of the international bredent group days from May 22-24, 2014 in Berlin on biofilm management. Mrs. Marianne Steinbeck talked to one of the key speakers and acknowledged expert in this field, Prof. Dr. Dr. Anton Sculean, Bern/Switzerland. Marianne Steinbeck: During major events in the field of implant dentistry the focus is frequently on hardware and technology - why is Prof. Dr. Dr. Sculean: It is correct, the efforts in implant dentistry concentrated on the mechanicaltechnical aspects of insertion and prosthetics. Today thorough planning, perfect preparation of the osseous imlant site and careful aftercare and hence biofilm management play decisive roles to ensure the long-term success of implants since this is what our patients who need to invest a lot of money are mostly interested in - in addition to the esthetic improvement of the oral situation. Marianne Steinbeck: What are the most essential issues in implant dentistry and which ones will be dealt with during the congress? Prof. Dr. Dr. Sculean: Today, above all the avoidance and successful treatment of peri-implant diseases are key factors in implant dentistry. We are currently witnessing how serious the consequences can be if we do not attach the appropriate relevance to the aspect of implant aftercare. Consequences may be periimplantitis or even implant loss. A change is required if we intend to treat our patients in a predictable manner with implants in future as well. Marianne Steinbeck: Can the participants in Berlin look forward to special new materials and innovative technologies? Prof. Dr. Dr. Sculean: I support this congress because I have gained new results on minimally invasive and safe treatment of periodontal and periimplant diseases in connection with my studies that I would also like to present there: for me, aPDT (antimicrobial photodynamic therapy) that is offered by HELBO represents a complementary procedure and an alternative to conventional methods. Although photodynamics have been scientifically examined and tested in general medicine and in the dental sector for about 15 years, still represent a groundbreaking innovation for many female and male colleagues. Moreover I know that bredent - based on the symbiosis of dental-technical and dental knowhow and competence - has made every effort for 40 years to provide „Perfect implant-prosthetic solutions through innovative materials and technologies“. This is also the motto of the congress; I am sure that we may look forward to other exciting innovations. Marianne Steinbeck: What is the importance of a concept such as SKY fast & fixed in modern dentistry in your opinion? Prof. Dr. Dr. Sculean: Immediate placement of implants and rehabilitation of edentulous ridges allow to fulfill the wishes of numerous patients. Reduced therapy time and cost control are key factors in this context. Nevertheless suitable framework conditions need to exist for such a concept in order to be able to meet the high expectations. It needs to be clearly stated that not all basic situations are suitable for the use of the concept! Initially, the overall situation must be clearly analyzed to obtain a basis for the decision. In addition to thorough examination of the bone substance available, some results of studies seem to suggest that treatment with the HELBO system after tooth extraction and prior to immediate placement of implants allows inflammatory processes to subside more quickly and has a positive influence on wound healing in general. Besides the necessary perfect combination of dentistry and dental techniques is certainly very important; accordingly providers of concepts need to offer perfect solutions for both sectors; this is what bredent does. Marianne Steinbeck: You are considered to be an expert in the field of photoynamics for many years. In the meantime some providers have established themselves in the market with different concepts which makes the decision for dentists more difficult - what is your opinion about this? Prof. Dr. Dr. Sculean: Basically, I think it is good for the user to have a choice and to be able to decide in favor of the procedure that is most suitable for his requirements. However, in the case of PDT I think that there are some elementary preconditions including the fact that a marketed procedure must be scientifically tested in vitro and in vivo and its effectiveness has been confirmed. In addition to the effectiveness, it needs to be determined whether a therapy has any undesired side effects. Moreover, I con- Marianne Steinbeck, publisher, pip Asked for you... sider the use of sterile products, e.g. the dye solution, in these bacterially contaminated areas mandatory. From today‘s perspective these essential criteria that were addressed are fulfilled by the HELBO therapy, which is also confirmed by the fact that - due to the data available one of the largest private health insurance companies of Germany issued official accounting and billing recommendations and only the HELBO procedure fulfills the necessary preconditions. So if you don‘t want to rely on the statements of providers, the advantage of having a choice must be carefully weighed off against the disadvantage of a necessary and thorough examination of the procedures that are available. Marianne Steinbeck: Thank you very much for your explanations. Read for you... Antibiotic resistances have reached an alarming level C. In the editions of Implants 03/2013 of oemus Media AG and the European Journal of Esthetic Dentistry 04/2013 of Quintessenz, articles were published about SKY fast & fixed and about the whiteSKY zirconium implants. Both publications report positively and emphasize the successful use. Clinical evaluation of zirconium oxide implants in the esthetic zone: a serial case study SKY fast & fixed as an alternative treatment in cases of periodontitis profunda Dr. Regina Schindjalova (Varna, Bulgaria) The experienced SKY fast & fixed user Dr. Schindjalova (Varna, Bulgaria) shows the consequences of periodontitiis profunda in a detailed x-ray analysis. The remaining teeth of the female patient were not worth preserving. Since the patient was only 44 years old, it was quite obvious that she wanted to have a fixed denture; the author fulfilled her wish by utilizing the SKY fast & fixed therapeutic concept. Based on this typical case, Dr. Schindjalova demonstrates the possibilities of SKY fast & fixed without augmentation in the case of considerably reduced amount of bone. She used wax set-ups from the beginning to give her patient an idea of the future restoration. View of the PES parameters In her summary, the author notes that the SKY fast & fixed therapy is particularly beneficial for this patient group since placement of implants at specific angles makes optimum use of the available bone and avoids complex augmentation procedures. The high-performance polymer BioHPP could become a real alternative to ceramic materials for the fabrication of dental implant restorations so that dental technicians may be able to fabricate esthetic restorations more easily and at reduced costs compared to all alternative ceramic restorations. The technique of pressing BioHPP to the titanium base without the formation of a gap to fabricate a crown abutment results in an enhanced marginal seal of the restoration, which has a positive effect on the gingiva around the implant and produces an improved esthetic result. The results of a preliminary study of 12 single tooth implants are presented, to which a monolithic crown abutment was attached; a BioHP abutment that is pressed to the titanium base without the formation of a gap is veneered with visio.lign.
