The Summer Violin program is a great opportunity for any student WITHOUT EXPERIENCE or ONE YEAR of
EXPERIENCE entering grades 1-8 this August. PLEASE NOTE THE LESSON SCHEDULE BELOW. All
interested families should complete the form below and give it to their Band/VIOLIN Director. Don’t forget to include

PROGRAM DESCRIPTION: The program is open to all musicians who have NO EXPERIENCE
OR ONE YEAR OF

BEGINNER SUMMER BAND PROGRAM
Dear Parents & Musicians:
The Beginner Summer Band program is a great opportunity for any student WITHOUT EXPERIENCE entering
grades 3-8 this August. After completing this two week session, many students are ready to join the experienced
players in their home school band program. Most students have seen & heard the instruments offered in the band at
their school’s Spring Concert. Please visit PaulEffmanMusic.com to see and hear a demonstration of each band
instrument. STUDENTS MAY NOT WAIT AND TRY SEVERAL INSTRUMENTS; PLEASE CHOOSE ONE
WHEN COMPLETING THIS FORM. All interested families should complete the form below and give it to their

PROGRAM DESCRIPTION: The program is open to all musicians who have completed at least one
school year of band instruction. Music styles will range from Classical to Jazz to Pop.
SCHEDULE: – Choose
one of two identical sessions:
Morning Session = 10am – noon.
Afternoon Session = 3pm – 5pm.
SESSION DATES = Monday 6/8, Tuesday 6/9, Wednesday 6/10, Thursday 6/11, Friday
6/12, Monday 6/15, Tuesday 6/16, Wednesday 6/17, Thursday 6/18, Friday 6/19.
There will be one informal concert on Friday 6/19 TBA.

Protection (included with all rentals): The Paul Effman Music Service Damage Protection Plan provides assurance that your child’s rental instrument is
covered for any accidental damage. Repair costs of rental instruments can be significant. This plan covers any damage to the instrument excluding blatant
negligence, loss, or theft. This plan does not cover included accessories such as reeds, drum sticks, neck straps, etc.

Traditional Rental Conditions: (1) I agree to pay the replacement value for lost instruments (alto saxophone and specialized instruments are $550, all other instruments are $275). (2) I agree to return instrument on or before the
last lesson taken during the 2015-2016 school year. (3) I agree to be responsible for missing parts, broken mouthpieces, broken drum heads, willful damage, vandalism, loss or theft of instrument. (4) I agree to return instrument in
as good condition, as when received, less normal wear. (5) If you return your rental instrument before September 15th 2015, a credit less $75 will be applied to your account. (6) There is a $25 fee for all returned checks. (7) By
completing this form I am agreeing to all terms and conditions.
