It’s the Law!

Listen to an overview of the Illinois one-call system and a discussion of the Illinois Underground Damage Prevention Act. Normal
and emergency locate requests will be discussed as well as recommended excavation practices. Specific examples of poor excavation
practices will be presented as well as statistics for the enforcement

called upon to assess a community playground for hazards? This
session will present an executive overview of the two-day National Playground Safety Institute playground safety certification
program. Included will be a brief historical look at playgrounds,
current injury data, identification of hazards and the development of an effective program for managing safe, yet challenging

Training is a necessary element in developing and maintaining
effective hazard control activities. Adult learning concepts and
good design affects the impact of that training. This interactive,
participant-centered session offers participants the opportunity
to learn about ways to reinforce adult learning so that you get the
most from training. At the conclusion of this presentation, you
will be able to identify seven ways adults learn and eight creative
ways to reinforce learning. (Workshop 4A/5A)

Learn what information companies are required to include in a
drug and alcohol program. Topics covered include the objective,
policy, testing protocol, pre-employment screening, post accident
testing, reasonable cause testing, refusal to test, and the handling
of negative and positive test results. This session will cover basic
requirements for a company procedure and post accident/justifiable cause drug/alcohol testing protocol. (Session 4B)

5C What’s Killing Workers?
Nick Walters, OSHA
In this session, OSHA representatives will address information
about recent workplace fatalities and the results of the OSHA
inspections. (Session 5C)
Bill Alderson is Field Operations Manager for the Office of the Illinois
State Fire Marshal in Springfield, Illinois. He holds a B.S. from Sangamon
State University. He formerly worked with the Illinois Department of
Transportation as an engineering technician and as a sales representative
for Steel Tank Manufacturing. For the past 15 years he has been with the
Fire Marshal’s Office regulating underground and aboveground tanks
throughout the state of Illinois.

Learn to navigate the useful information available on the OSHA
website. This session will address how to stay updated on new
standards, how to access compliance tools, sample programs, and
a variety of information that may help you prevent injuries and

This session will offer a discussion on motor carrier safety
regulations and what documents are required to be kept on file at a
company. If your company owns and/or operates trucks, this will
be a very informative fifty minutes. (Session 6A)
6B Lock Out/Tag Out
Tom Grey, Human Resources Growth Partners, Inc.
You will be guided through meaningful, effective lock-out/tagout training techniques. The light will shine on the best safety
practices and change the way your employees approach lock-out/
tag-out procedures. (Session 6B)
6C What Employers Must Know About Multi-Employer Worksites
Julie O’Keefe, Armstrong Teasdale, LLP
This session explains the responsibilities faced by all employers at
work-sites where more than one employer is present. This will be
pertinent to both general industry employers who hire contractors
to come into the facilities and to all contractors working on
construction sites.

6D Industrial Forklifts Today: A Refresher Look
Randy Mahring, Altorfer Rents
Get a refresher of today’s regulations, required skills and tips on
safety for industrial forklifts. Discussion also includes equipment
and personnel issues facing today’s management, anecdotal
experiences and helpful tips concerning forklift safety training and
enforcement. (Session 6D)
Karl Armstrong is a Compliance Officer/Safety Specialist with OSHA
at the Peoria area office. In 2003, he joined OSHA after working in
the private sector in both construction and industry. He has conducted
inspections in a variety of industries, and is experienced in identifying
hazards and providing abatement solutions. He is a graduate of Illinois
State University with a B.S. in Occupational Safety.
Tom Bielema is a 1995 graduate of Purdue University with a B.S. in
Environmental Health Science/Industrial Hygiene. He is a Compliance
Officer/Industrial Hygienist with the Peoria OSHA office and has
conducted inspections in a wide variety of work sites.
Rob Bonack is a Compliance Safety and Health Officer with OSHA
in Peoria and has conducted inspections in a variety of work sites. He
holds a B.S. in Safety, Minor in Environmental Health from Illinois State
University, and a B.S. in Professional Aeronautics, Minor in Aviation/
Aerospace Safety from Embry-Riddle Aeronautical University. He is also
a safety manger in the Illinois Air National Guard for the 264th Combat
Communications Squadron. He served on active duty in the United
States Air Force for 10 years prior to joining OSHA.
Brian Bothast is a Team Leader with OSHA in Peoria. He has worked for
Caterpillar in the Environmental, Health and Safety Department and as
an Industrial Hygienist and a Compliance Assistant with OSHA. He has
conducted health and safety assessments, training, and hazard evaluations
throughout the United States and Europe. He is a 1988 graduate of
Illinois State University with a B.S. in Environmental Health.
Mark Briggs, CSP, ARM, is Risk Manager for the University of Illinois.
With over 20 years of experience in occupational safety and health, he
has worked with a variety of companies and organizations to improve
employee safety, ensure OSHA compliance, reduce workers compensation,
and conduct employee training programs and more. He holds a B.S. in
Safety and Accident Prevention.
Jeffrey Clark, owner of 2 B Safe, Inc., offers over 20 years of experience
in the safety and health fields as well as expertise in the mechanical and
construction safety area. He has worked with various industries developing
customized safety and health policies and training outlines. He is an
authorized OSHA Outreach trainer and conducts contract training
throughout many of Illinois’ community colleges.
Tim Crouse currently works as a Whistleblower Investigator for Federal
OSHA based out of the Peoria area office conducting investigations
throughout much of Illinois. Prior to joining OSHA, he spent 15 years
with the Indiana OSHA state plan, including four years as an Industrial
Hygiene Compliance Officer and Discrimination Investigator, ten years
as the Director of Industrial Hygiene Compliance and one year as Acting
Deputy Commissioner for IOSHA. He received a B.S. in Public Health
from Indiana University and an MBA from Indiana University’s Kelley
School of Business.
Sue Ellen DeManche is an Industrial Hygienist for OSHA in the Peoria
OSHA office. Prior to joining OSHA in 2001, she worked as a consultant
in private industry. She received her M.S. in Environmental Health/
Industrial Hygiene from Old Dominion University. 4
Allen Durr is Safety Director for Williams Brothers Construction, Inc. He
received a B.S. in Occupational Safety from Illinois State University. He
is a certified CPR instructor, certified AED instructor and an OSHA 500
and OSHA 10-Hour certified trainer. He develops and implements safety
programs, performs OSHA audits on job sites, and manages all aspects of
workers’ compensation.
Tammy Duvendack is Director of Outcomes Management for Methodist
Medical Center of Illinois. She also teaches part-time at Bradley
University in the Department of Nursing. She has a Ph.D in Educational
Administration and Foundation Programs, a M.S. in Medical-Surgical
Nursing and a B.S. in Nursing from Bradley University. In her current
position she is responsible for organizational quality improvement planning
and implementation for Methodist Hospital and 30 physician offices. She
oversees the patient safety program for the hospital and physician offices
and is in charge of the Infection Control and Sterilization Program for the
hospital and physician owned offices.
Anthony Gilson is an Industrial Hygienist for OSHA. He holds a B.S. in
Health Care Services and has completed graduate work in Environmental
Health from George Washington University in Washington D.C. He
served as an Environmental Health and Safety officer with the United
States Navy for 21 years prior to joining OSHA and is certified as a
Radiation Health officer.
Robert Gonzenbach is an Industrial Services Hygienist with Illinois
Department of Labor. He earned a B.S. in Health & Safety Education as
well as his Masters in Occupational Safety & Health at Southern Illinois
University. He has been in his current position with IDOL since 1990. He
is owner of Southern Safety, an occupational safety/health consulting firm. Thomas Grey is a highly skilled trainer and coach in employer and
employee development of safety performances. He has over 28 years of
hands-on experience as a former plant manager, human resources manger
and quality control manager in manufacturing, distribution and trucking
industries. His manufacturing experience ranges from automotive, food
processing and packaging, consumer products and personal care products.
His extensive background, knowledge and expertise solidify his role as
Performance Growth Partners, Inc., Safety Practice Leader.
Tom Kalousek is the current chair of the National Playground Safety
Institute (NPSI), an official certification program of the National
Recreation & Park Association. He has over 35 years of experience in the
field of Public Parks, Recreation and Natural Resource Management. He
is lead instructor with NPSI and serves on the ASTM-F1487 Standards
committee that has been developing safety standards for almost 20 years.
He is currently the Executive Director of the Winnebago County Forest
Preserve District in Rockford, Illinois.
Dale Kunz serves as Damage Prevention Manager for JULIE, Inc., the
one-call underground excavation services for the state of Illinois. A native
of Canton, Illinois, he graduated from Western Illinois University with
a B.S. in Education. He also holds a M.S. and M.B.A. Since 1979, he
has held a number of field and management positions in the electric and
natural gas utility industry. Prior to joining JULIE, Inc., he served as
Executive Director of IMUA where he expanded safety related programs
for its members. Since joining JULIE, he is actively involved in promoting
safe digging practices and educating the public regarding the Illinois
Underground Utility Damage Prevention Act.
Randy Mahring is an Account Executive/Safety Trainer for Altorfer Rents,
The CAT Rental Store. He has a B.A. from University of Iowa. He is a
certified safety trainer for all types of aerial platform lifts and is a certified
OSHA trainer. He has trained hundreds of operators on forklifts and aerial
lifts in Illinois and Iowa and has worked with many of the labor unions. Rita Mosley is a Certified Hazard Control Manager, a Certified Safety
Professional, and holds a M.S. in Communication and Training. She has
been employed for the past 25 years with the Illinois Onsite Safety &
Health Consultation Program and is currently Manager of Training and
Promotions. Prior to the consultation program, she spent 13 years in
private industry as a personnel manager and safety director in the metal
fabricating industry.
John Newquist has a B.S. in Fire Protection and Safety Engineering
from the Illinois Institute of Technology and a Masters of Science in
Industrial Management from Northern Illinois University. He is a
Certified Safety Professional and a Certified Fire Inspector. He has
taught hundreds of safety seminars across the country including for
the Associated General Contractors, Construction Safety Conference,
American Society of Safety Engineers, Building Trade Councils, and
the OSHA Training Institute. He has worked for OSHA since 1983 and
presently is the Area Director of the Peoria Area Office.
Julie O’Keefe is a partner in the law firm of Armstrong, Teasdale,
LLP out of St. Louis. She is a graduate of Illinois State University and
St. Louis University Law School. Her focus practice is in the areas of
OSHA, environmental, regulatory, labor and construction law. She has
represented employers and corporations throughout the country. She is
a member of the Missouri Bar, US District Court, Eastern District of
Missouri, Illinois State Bar Association and she is a frequent presenter
for various trade associations and industry groups concerning OSHA
and environmental issues.
Leon Platt is Safety Director for MACC Safety Inc. He has a B.S.
in Speech Therapy from the University of Washington and a B.S. in
Physical Therapy from the University of Alaska. He is a member of
the ASSE, a member of the AGC of Illinois, and a member of the
International Association of Aggregate Producers Safety Committee.
He is responsible for implementing the Injury Prevention Process for
MACC.
Dr. Laura Schelly is a graduate of Illinois Central College and
Bradley University. She did her graduate studies at National College
of Chiropractic. She has a B.S. in Human Biology and is a Doctor of
Chiropractic. Dr. Shelly is a member of the Illinois Chiropractic Society,
the Illinois Prairie State Chiropractic Association and was a “40 Leaders
Under 40” award recipient in 1995.
Dr. William S. Scott is the Medical Director for the Center of
Occupational Health at OSF St. Francis Medical Center. Prior to this
position, he worked in similar clinics at the University of Oklahoma and
Carle Clinic in Urbana, Illinois. He has worked the past 14 years as a
specialist in Occupational and Environmental Medicine.
Dale Tanke is a Storage Tank Safety Engineer with the Office of the
Illinois State Fire Marshal. He holds a B.S. in Civil Engineering from
Southern Illinois University in Carbondale. He has worked for the State
of Illinois in various capacities for over 26 years. He has been with the
State Fire Marshal’s office as a Storage Tank Safety Engineer for the past
16 years.
Wes Taylor is a Compliance Assistance Specialist in the Fairview
Heights District Office of OSHA. He has previous experience in both
the military as a nuclear weapons and explosives safety supervisor as well
as the private sector. He has worked for OSHA since 1975.
Nick Walters is a Compliance Officer/Safety Specialist for OSHA in the
Peoria office. He joined OSHA in 1992 and has conducted numerous
inspections in a variety of workplaces. He is a graduate of Northern
Illinois University with a B.S. in Industrial Technology/Safety.
George Wyness is a graduate of McKendree College in Lebanon,
Illinois. After working in both the private and public employment
sectors he began his career with the Illinois Department of
Transportation, Division of Traffic Safety. As a Compliance Specialist,
he is responsible for administering the provisions of the Illinois Motor
Carrier Safety and Hazardous Materials Regulations.
