FROM THE PRESIDENT
A FAREWELL MESSAGE FROM PETER
by Dr. Peter Rechmann
Most times the final message from a president is
self-congratulatory, recounting the progress of
organization during his time on duty. And it would be
fitting and proper if I did that since we have made
significant advances in our relationships with the dental
Peter Rechmann,
community—The

General
Dentistry in particular, as
Prof. Dr. med. dent.
well as broad inroads into Dental Schools. Furthermore
at the American Dental Education Association during the first Special Interest
Group - Lasers in Dentistry meeting Academicians from eight different
Universities discussed integration of Lasers in Dentistry in their teaching
curricula.
The credit for the success should be assigned to the Board, Committee
Chairs and ALD members, as well as the tireless work of Gail and her
staff. Functioning tirelessly as a team is what made all of these things
happen. Having said all of that, I could go on and on, but now I want to
devote my final thoughts on science, particularly the concept of thermal
relaxation time.
What’s this, you might be thinking, science belongs on the other pages,
specifically in the Journal of Laser Dentistry (JLD). But I think there is a
lot more to learn from laser science than simply how to use our technology
more skillfully. We all know that when we use lasers, we have to pay
special attention to thermal relaxation time if we want to achieve the desired
and best treatment results without causing collateral damage.
I have observed, and at this point I invite you to imagine that the
Internet and particularly the ability to communicate with email, is a laser—
powerful for sure, but dangerous and easily abused. The abuse that I would
like you to think about, happens when we do not pay sufficient attention,
frequently we do not pay any attention at all, to the consequences of our
messages and responses. If we think about communication talking face-to-
face to someone allows us to observe his body language, the tone of his
voice and the content of his sentences - a three-dimensional experience.
Talking on the phone is already reduced to a two-dimensional event and
when we only have writing we reduce it further. We don’t appreciate the
one dimensional nature of this form of communication, nor the way that
our email will be received. When I say received I mean 1- read, 2-understood
and 3-experienced emotionally. As a scientist and researcher, I would like
to bring you back to the necessity to respect the thermal relaxation time of
human beings, especially in communication. We need time to relax, to
think, to consider and to properly compose both our thoughts and language
and then finally to fully imagine how what we say will be received. This
takes time, relaxation time if you will …
I will close with some thoughts you perhaps might want to print and post
on your computer screen.
NEVER SEND A MESSAGE IN ANGER, NEVER, EVER
Instead:
READ...
READ TWICE...
WRITE whatever you think is the best message or answer,
BUT DO NOT HIT SEND UNTIL YOU AT LEAST
SLEEP ONE NIGHT.
Next day, READ YOUR MESSAGE AGAIN —
Is it still valid? Can it be misinterpreted? Can it hurt someone?
In doubt? PICK UP THE PHONE.
EDITOR’S MESSAGE
Put Your Whole Self into Your Treatment Plan Presentations
by Dr. Ana Triliouris, Communications Chair
In my practice we
have two-hour staff
meetings every week. At
those meetings, we each
Ana Triliouris, DDS
bring up the things that we
Editor
feel have affected the
practice both positively and negatively during the
week. At last week’s meeting, my patient
coordinator, who is present during all my treatment
plan presentations, contributed that “a positive must
go to Dr. Triliouris for the way she is doing all
treatment plan presentations. They convey her caring
nature and she puts her whole self into them so much
so that there is just no way the patient will not follow
her recommended treatment.”
2
Wow! I was impressed with her remarks and I
thanked her, but most importantly, she got me thinking.
There is so much written about treatment plan
presentation. Each of the gurus has his or her answer
to successful treatment plan presentation and
acceptance. But I think you just have to be there, fully
and with your heart in your hand to show you care.
Of course, you have to be prepared. You have to
talk in language that is clear to somebody that is not a
dentist. You should have pictures of the patient’s oral
condition to show them what you see. You should have
pictures or models of what you are recommending so
the patient can see what you are talking about. You
should be able to offer financing so that they can afford
the treatment. You need to listen to what they want, not
everybody wants a Hollywood smile. But unless you
put yourself in their shoes, show them your human side
and make them believe you have their best interest in
mind, it will not work.
What do I mean by being there? It is exactly
that “touchy feely” thing that most dentists have
difficulty with. You know that your patients don’t
know you really care until you tell them and show
them with your actions as they get to know you and
you get to know them. That is the way you develop
long standing relationships and how you make
treatment plan presentations that are successful and
that are accepted by the patient. This is what we
must all do to keep us all working in these difficult
economic times.

Bond Strengths of All Ceramics: Acid vs.
Laser Etching
Bulent Gökce, DDS, PhD is a scholar from Ege
University School of Dentistry in Izmar, Turkey.
The purpose of this paper indicates that for
surface modification of the inner surfaces of
ceramic restorations, the etching pattern obtained
by dental lasers could be an alternative to
conventional acid etching.
Dr. Gökce is a full-time resident and an Assistant Professor at the
Department of Prosthodontics of Ege University, School of Dentistry,
Department of Prosthodontics and clinical instructor of third year dental school
students. His PhD thesis is on comparison of bond strength to dental substrates
with different surface treatments. His current interests include research of
dental lasers, dental materials and mechanical testing, all-ceramic materials
and zirconia restorations, dental implants, dental adhesives and clinical
applications. He is a member of International Team for Implantology, the
Turkish Prosthodontic and Implantology Association, the International
Association for Dental Research and the Academy of Laser Dentistry. For
this award, Dr. Gökce is mentored by Professor Birgul Ozpinar, PhD,
Prosthodontics Department Chairman.

Glass Ionomer Bond
Strength to Teeth
Prepared with an
Erbium:YAG Laser
Ms. Huma Hamid and
Mr. Stephen M. Tapp are
scholars from the University
of Texas-Dental Branch at
Houston, Houston –Texas.
The objectives of this
study are to provide original knowledge on the interfacial bond strength of
glass ionomer to laser-treated tooth structure by answering the question
“How is the bond strength of a glass ionomer to the laser treated tooth as
6
compared to bur-treated surfaces, and which conditioner - the conventional
etch and rinse conditioner or a non-rinse, self-etch conditioner is more
effective?”
Ms. Huma Hamid holds a Bachelor of Science degree in Biology from
the University of Houston, Texas. She worked at Baylor college of Medicine
as a Research Technician before coming to the University of Texas Dental
Branch-Houston, where she is currently a third year student. Ms. Hamid is
a member of the National Society of Collegiate Scholars (NSCS), American
Student Dental Association (ASDA) and International Association for Dental
Research (IADR). While attending school, she also participates in various
research projects related to dental health.
Mr. Stephen M. Tapp holds a Bachelors of Science degree in Chemistry
from the University of Chicago. He is a registered nurse in the State of Texas
and has experience in clinical pharmaceutical research. He is currently a
third-year DDS student at the University of Texas Dental Branch at Houston.
Mr. Tapp is a member of American Student Dental Association (ASDA)

A Quantitative Assessment of the Effect of Various Antimicrobial
Therapies on Porphymonas Gingivalis Contamination on the Implant
Surface: A SEM Study
Mr. Brian Sang is a scholar from West Virginia University School of
Dentistry, Morgantown, West Virginia.
Previous studies have discussed treatment methods for ailing implants,
but none have compared methods for efficacy. The educational objective
of this study is to determine if a laser is the pre-eminent treatment method
for the elimination of P. gingivalis from the HA coated titanium surface.
Brian E. Sang is a fourth year dental student
at the West Virginia University School of
Dentistry in Morgantown West Virginia. He
attended undergraduate college at Marshall
University in Huntington West Virginia majoring
in Biological Sciences and performing research
in the field of paleontology. After graduation
Brian plans to pursue a career in Oral and
Maxillofacial Surgery. He is proud to say he is
an Eagle Scout, member of Delta Sigma Delta West Virginia Graduate
Chapter Dental Fraternity, and member of Alpha Chi Sigma Professional
Chemistry Fraternity. Brian is also a musician and plays the guitar, piano,
and drums. For this award Scholar Sang is mentored by Harold H. Reed,
DDS, MS, Clinical Professor of Periodontics. Mr. Sang may be reached

Morale... What It Is and How Can We Improve It
by Dr. Alan Goldstein
The American Heritage dictionary defines
morale: As the state of mind of a person or group as
exhibited by confidence, cheerfulness, and discipline.
I will discuss how we can improve morale in a typical
Alan J. Goldstein, DMD dental office. I’m inclined to add the notion that this
New York, NY
is a more difficult task in a period when so many of
our patients are feeling the effects of the economic
downturn. The mood or emotional tone that we often encounter is related to
anxiety, frustration and resignation. I’m not so sure that the current economic
climate is a major determinant of morale in our offices, but it would be silly to
declare that morale exists independent of circumstance.
Can we control something as subjective as morale? Sure we can say to
our staff, “Leave your personal issues at the door, it is Showtime!” but in the
real world can any of us truly do that? A sick child, babysitting issues, an aging
parent in whose care we are entrusted and a myriad of personal issues all can
and do weigh us down. Who knows, I mean really knows, what goes on in the
hearts and minds of our staff. We are not mind readers. Sometimes it is hard
enough to get in touch with our own state of mind.
Creating community is one important way we can stay positive and upbeat,
even in the trying world we live in. To create this sense of community we start
every single day with a morning meeting, commonly called a huddle in dental
practice parlance. In addition to the most common agenda items for the huddle;
the procedures that are planned and the finances associated with them, our
morning meeting becomes the place where we can briefly unpack, as is
appropriate, what is weighing us down. Brevity is essential; this is not a therapy
session. Team members should be listeners, not amateur psychologists. This
activity helps create and maintain a loving and empathic office. It is not easy
but it is essential. It does require a high level of emotional intelligence on the
part of the office leader. Sorry, I knew you were looking for some trick or
magic formula to get everyone radiating joy. There is no silver bullet, only this
piece of team building work each and every morning.
Our profession includes many Dental Consultants who will tell us what
marketing plan to use, or how to script conversations for improved case
acceptance, or even teach us how to reduce our overhead. These are all important
things for our practices. The assumption is that if we improve our production
collection and net profit and then share the wealth via bonuses, the morale of
the office will improve. I maintain that prosperity does not ensure a high esprit
de corps. Relationships among our staff and with our patients have a greater
impact on morale than the finances.
Let me introduce the notion of character as it was recently used by James
Flaherty in his February 2009 Coaches Reunion Workshop in Washington
DC. He is a leader in the Coaching profession and the head of New Ventures
West, a coach training institute. According to him, character can be defined…as
the observable and automatic ways we act, react, express, think and feel that
brings forth reactions from others and the world. Here is the point: our character
is the major determinant of the morale of those with whom we work. If you
want to look for the key to the morale of your office, look to your own character.
Look to the way that your staff reacts to you, look to the gifts you offer; the
praise, gratitude, and appreciation and see if they have been received. Then
look too, to the criticism and see how that is received and the effect that this has
on morale. If we consistently make positive offerings to our staff and make the
necessary adjustments to insure that they are received, then the morale of your
staff, the morale of your patients and pretty much the morale of everyone you
touch will be positive. If this sounds a bit too pie-in-the-sky for you, imagine
the inverse – how a surly, short and hypercritical character torpedoes morale
faster than Nurse Ratchet in One Flew Over the Cuckoo’s Nest.
Morale is a function of small things—a look, a tone, an empathic nod or
just an openness that says I am listening. We can never know with certainty
what life has in store for us. If you, as the leader of your office, stay awake to
the incredible impact that your character has on the morale of everyone around
you, this positive aliveness that you engender can’t help but be infectious.
To recap:
• Create a sense of community every morning by extending the huddle in a
way that gives everyone an opportunity to respond to the implied question,
“How are you doing this morning?”
• Pay attention to finances but don’t be seduced into thinking that the dollar is
the answer to everything. Maybe the secret is staff morale.
• Be seen as a rainmaker by your staff. Create excitement by always having
initiatives on the burner that will expand the practice. For example: a smile
sale for Valentine’s Day, a well advertised discount on whitening and
porcelain veneers, an appreciation initiative for Thanksgiving, perhaps
enhanced hygiene services that include a discount on electric toothbrushes
to clean off all the turkey and stuffing that is left on your teeth after the feast
and all the other possibilities that you and your creative staff can imagine.
• Stay awake! Realize that your character is the biggest single determinant of
the morale of your staff. Be a nurturer for every one of them.
