It is the result of an important and ongoing process of informing patients and consumers about the importance of compliance. This includes following the advice of their eye care professional and the instructions that accompany the product, to ensure the best possible eye health and quality of vision. Good hygiene and using contact lenses and related products in the intended way may seem obvious to us, but two new European studies (led by Dr. Philip Morgan) show that many wearers handle their lenses incorrectly. Not washing hands, not cleaning or changing lens cases, using solutions that are out of date and wearing the lenses longer than they are intended for, are examples of actions that put contact lens wearers at risk of infection. Unfortunately, these practices are all a lot more common than we might think. We discuss these compliance issues in our section on Health. British Blind Sport focuses on encouraging the active participation of visually impaired people (especially youth) in a large range of sporting events. The participants could be either partially sighted or blind and participate in events ranging from archery, shooting, cricket, football and goalball (a sport specifically designed for the visually impaired which has sporting equipment adapted to allow the participants to utilise other sensory functions to guide them). At present British Blind Sport is still a small charity, but does work on a national level, utilising the skill of sports coaches on a voluntary basis. One such event is the Partially Sighted Football League which consists of eleven 5-a-side teams that follow the new international rules for Futsal. Interestingly, some of the players in the league also play internationally for the England Partially Sighted Team. The totally blind Great Britain team will be going to Beijing to attend the Paralympic Games. Bausch & Lomb are proud to sponsor the football season and we attended our first event the Visually Impaired Futsal League Season Finale held on Saturday 26th April 2008 at the Telford Dome. It had been a long six week break since league tournament 5, with the leaders in both Divisions 1 and 2 enjoying healthy gaps to their nearest rivals. The battles seemed to focus on the race for second place in both Divisions, and the anticipated relegation scrap for one of the longest serving clubs within Division 1. Inter Cosmos, who boast England players within the team had, it would seem, a straightforward task to avoid relegation. Bottom of the league at the start of proceedings (due mainly to England training clashing with earlier tournaments) they simply had to score more points than Loughborough B team. Things did not start well. Defeats to Loughborough A team, West Bromwich Albion and RNC put an end to any hope Cosmos might have had of getting out of trouble. After over a decade in the top flight of VI football Inter Cosmos had been relegated and must now contemplate second division football. British Blind Sport (BBS) is a national registered charity (Registered charity number 271500) based in Leamington Spa. Since its beginnings as a registered charity in 1976, British Blind Sport has become the leading voice for visually Futsal is an indoor version of association football. Its name is derived from the Portuguese futebol de salão and the Spanish fútbol sala/de salón, which can be translated as 'indoor football'. Futsal is played between two teams of five players, one of whom is the goalkeeper. In addition each team may have 7 substitutes. Unlike some other forms of indoor football, the court is delimited by lines, and not walls or boards that players can use to rebound the ball. Futsal is played with a smaller ball with less bounce than a football. The emphasis is on improvisation, creativity and technique. Being diagnosed with macular degeneration can be a frightening and lonely time, which makes the work of the Macular Disease Society so valuable to patients and their families. The society aims to build confidence and independence for those with central vision impairment, offering advice and practical support to sufferers who otherwise may struggle to come to terms with their condition. From helpline advice to research, the Macular Disease Society lends a supportive hand to help those affected make the most of their vision, and Bausch & Lomb are a proud supporter of their work. A Helping Hand Helpline The Society runs a helpline for all patients and their families 0845 241 2041 offering advice on the practical impact of life after diagnosis with MD such as new treatments and accessing social care. Counselling A telephone counselling service run by trustee and macular degeneration sufferer Dennis Lewis, is available to help patients come to terms with their condition. The Society is in the process of setting up a network of Community Support Desks which are located in libraries and other public places, and provide a chance for recently diagnosed patients to discuss their concerns. Information Members of the Society receive ‘Side View’ a quarterly magazine, with information on the latest developments in macular degeneration research and news. Local Groups The Society runs a national network of 150 Local Groups which give members a chance to meet other sufferers and share advice and guidance. On joining the Society, members are invited to join their nearest group. There are separate groups for working age members, which give support in the field of training, job retention and technology for the visually impaired at work. Hospital Support In Leicester Royal Infirmary and Mayday Hospital Croydon, Macular Disease Society volunteers attend on macular clinic days to ensure that newly diagnosed patients have a point of contact for help and support. ‘Age-related macular degeneration (AMD) causes severe visual loss and is the commonest cause of blindness in persons >50 years old in the western world. Two main forms of AMD occur: dry and wet. The dry form accounts for 90%, whilst the wet form occurs in 10% of all AMD….There is, unfortunately, no medical or surgical treatment currently available for dry AMD… Ocular nutritional supplements have been shown (in the AREDS Study) to slow the progression of dry AMD to more advanced stages. Such nutritional supplements should therefore be recommended to patients.’ Research The Society actively promotes research into macular degeneration by funding research projects at UK hospitals. Several conferences are held each year at various locations around the UK to enable members to hear the latest developments from acknowledged experts in the field. The Society donated £147,000 in 2007 to research into the causes of macular degeneration. Details of past research projects can be viewed at www.maculardisease.org. (AMD Interim Guidelines: The Royal College of Ophthalmologists Interim Recommendations for the Management of Patients with Age-related Macular Degeneration) The Age-Related Eye Disease Study (AREDS) (Arch Ophthalmology, Oct. 2001) was conducted over 10 years by the National Eye Institute in the USA with 4757 participants. The high-dose supplement formulation of vitamins C, E, beta-carotene and zinc which was tested in this study was found to reduce the risk of progression to advanced AMD by 25% and reduce the risk of visual acuity loss by 19% for patients with moderate to advanced AMD. The supplement used in this study was provided by Bausch & Lomb and is now available in the UK under the brand name ® ‘PreserVision ’. It is widely acknowledged that consumers are not compliant. In 2007 Bausch & Lomb commissioned research conducted by Dr Philip B. Morgan, PhD MCOptom FAAO FBCLA across seven European countries and 1,400 contact lens wearers to uncover the truth about patient attitudes and behaviour towards lens wear and care. UK EYEGIENE RESULTS SURVEY Correct solution Advised received Replace solution Handwashing Shockingly, the research highlighted the true lack of consumer adherence to compliance, with 98% of all contact lens wearers being non-compliant in at least one aspect of their care regimen. The findings published in ‘The Science of Compliance’ quantified the real risks, and Bausch & Lomb is now using this data to endorse our campaign for GREEN BEHAVIOUR – a response considered to be wholly compliant. Hands up for Eyegiene™ Over the years a number of studies have shown a link between inadequate contact lens care and ocular infection. Infections occur most commonly as a result of direct contact between individuals (person-toperson transmission). It was disturbing then to find, that the results from the research found UK patients to be the worst nation for handwashing, with only, just over half (56%) of UK contact lens wearers washing their hands before inserting lenses and only 14% always washing their hands before removing lenses. Evidence from the use of hand washes in hospitals confirms that an element of training in the best methods is required. Hands should be washed with soap, antiseptic liquids or wipes and dried with a lint free towel prior to touching the eye. The verdict on the case An important outcome of the research is that case care is often over looked by lens wearers, and can ultimately result in an increase of the bioburden at the ocular surface during contact lens wear. We still need to reinforce the importance of cleaning cases on a daily basis – with the appropriate solution and leaving the case to air dry. The research found, of those that did clean their lens case, only a third of wearers (34%) cleaned their cases with contact lens solution. The case should be changed at least once a month. When you take all these steps of lens case compliance into consideration, the results from the research showed that 79% of lens wearers are non-compliant when it comes to the care of the case. To continue to educate ECPs on the importance of compliance, Bausch & Lomb have created ‘The Science of Compliance – A Guide for Eye Care Professionals’ training CD. A training CD featuring Dr Philip Morgan and approved with 2 CET points. The presentation is based around the results of ‘The Science of Compliance’ and reports on the increased bioburden and decreased ocular defence mechanisms, which occurs during contact lens wear and which specific risk factors have been linked to contact lens associated keratitis. Shockingly, the research highlighted the true lack of consumer adherence to compliance, with 98% of all contact lens wearers being noncompliant in at least one aspect of their care regimen. B+L Newsletter 2nd edition:Layout 1 1/7/08 17:01 Page 09 HEALTH “Ultimately every eye care professional wants a confident partnership with their patients in order to establish mutual understanding and goodwill that will result in optimum ocular health.” ‘If we, as contact lens practitioners, can understand both the real life behaviour of our patients and the perceptions they have about caring for their contact lenses, we can communicate better with them, and optimise their use of both contact lenses and care systems’ advises Morgan. Dr Philip Morgan concludes from the report, ‘ultimately every eye care professional wants a confident partnership with their patients in order to establish mutual understanding and goodwill that will result in optimum ocular health’. BAUSCH & LOMB WINS GOLD IN THE MEDICAL DESIGN EXCELLENCE AWARDS COMPETITION Akreos® AO Micro Incision intraocular lens receives a prestigious honour. Figure 1 These developments have resulted in high quality lenses that are completely tolerated, stable in the eye, PCO free and have excellent quality of vision. Now that the growing trend is towards MICS™, the boundaries are being pushed yet further and lens design needs to be modified again to accommodate this new requirement. The first Akreos® intraocular lens (IOL) was implanted by Pr. Jean-Louis Arné on 29th June 1998 in Toulouse, France. This lens, known as the Akreos® Disc, was the first of a family of lenses that have spanned a decade and been implanted in over 2 million people worldwide. All disc designs generally stem from the very first implant made by Harold Ridley on 29th November 1949 at St Thomas Hospital, London. These implants are stable but large, so are unsuitable for small incision surgery. There have been 5 generations of design in the Akreos® family (Fig 1); The Akreos® Disc, the Akreos® Fit and the current family trio which consists of the Akreos® Adapt, Akreos® AO and the new Akreos® MI60 Micro Incision lens. The Akreos® MI60 lens, like its Akreos® predecessors, is composed of a biocompatible, hydrophilic acrylic material, which makes it incredibly strong yet flexible enough to be inserted through a 1.8mm incision. The material for this range of lenses was first developed in 1996-1997. It is a copolymer of HEMA (hydroxyethlmethacrylate) and PMMA (polymethylmethacrylate). These polymers have a long record of use in ophthalmology and demonstrate excellent biocompatibility. PMMA has been used in IOLs since the 1960s, whilst HEMA is used both extraocularly in The Akreos® AO Micro Incision lens (Akreos® MI60) wins Gold in the 2008 Medical Design Excellence Award (MDEA), the premier awards programme for the medical technology community. The Akreos® MI60 lens is 30% thinner than its parent the Akreos® AO allowing implantation through a sub-2mm incision. Figure 3 contact lenses and intraocularly for scleral implants. The hydrophilic component, HEMA, allows the lens to be easily compressed to fit through a micro-incision and unfold smoothly when implanted in the eye. The mechanical resistance of the hydrophobic acrylic component (MMA) ensures the lens recovers its initial shape without damage. With the need for a lens suitable for MICS™, the engineers at Bausch & Lomb were faced with the challenge of producing a lens compatible with The novel haptic design is slender, but ensures excellent stability of the implant. The external portions of the haptics, known as the absorption zone, undergoes deformation in 3 dimensions in order to limit any pressure which may be exerted on the optic (Fig 2). The base of the haptics is rigid and thicker, and together with the optic, forms the foundation zone (Fig 2). The Akreos® MI60 lens uses the same anti-PCO properties of the Akreos® AO: the 360º anti-PCO barrier and square edge design (Fig 3). The haptics on the new lens known as the Conforming Tips (Fig 2) are also angled at 10º to improve contact with the posterior capsule. Like its predecessor, the lens uses an aberrationfree aspheric optic. This leaves the eye with its natural degree of corneal positive spherical aberrations, improving contrast sensitivity and benefiting patients with good depth of field. Mr Pillai says Zyoptix® has other advantages over standard laser surgery, “After traditional laser surgery, although good sight usually resulted, patients would complain of night halos and night vision problems. Zyoptix® has virtually eliminated these.” Zyoptix® technology falls into three categories. First, there is Wavefront treatment which is particularly suitable for patients that have large pupils and for all prescription types. It is also used to eliminate night vision problems which can occur with standard laser treatment. The second category is tissue-saving treatment, used when someone has a thin cornea, as the procedure removes less tissue. Rebecca Martin, Before Zyoptix® Rebecca Martin, After Zyoptix® Today, however, she has perfect vision, and it’s all thanks to Zyoptix®, the new laser vision correction surgery technology, pioneered by Bausch & Lomb. Rebecca’s optometrist researched her options and found out that Bausch & Lomb had developed a new type of laser eye surgery technology, called Zyoptix®, which offers a solution for people like Rebecca, who need a personalised vision treatment. Now, even people with high prescriptions, large pupils and/or thin corneas can have their vision restored. Rebecca, 23, who comes from Surrey, endured taunts of ‘foureyes’ when she was at primary school. “If I put my glasses on, it would remind me of the bullying I received as a child and subsequent lack of confidence, which in turn affected my work.” She recalls. “There were all these gorgeous trendy glasses out there, but I just couldn’t take advantage of them. Not only was she extremely shortsighted, but, like many people with blonde hair and blue eyes, she also has large pupils. She was told that the combination of these factors made her unsuitable for conventional laser eye surgery. “Bausch & Lomb has devised outstanding new technology for laser surgery,” says Mr CT Pillai, Consultant Ophthalmic Surgeon and Medical Director at Advanced VisionCare in Harley Street, London, one of the 36 Zyoptix® clinics in the UK. “Laser surgery used to be performed with standard technology that treats only the prescription of the eyes. Patients who had night vision problems, very high or complicated prescriptions, thin corneas or large pupils were simply not suitable. But with Zyoptix®, many more of these patients can now be treated. “The new technology employs a faster laser, iris recognition technology and is programmed to treat every eye individually according to the specific needs of the patient.” Finally, there is the latest Aspheric treatment, which can be used for patients who are not found suitable for Wavefront. Aspheric takes into account the natural shape of the cornea and designs the treatment around it. Aspheric is particularly valuable for patients with high prescriptions, large pupils and thin corneas. Mike says “I was very nervous before the procedure.” Rebecca recalls “I wouldn’t let go of his hand, but it was completely painless and it was over very quickly. I think it only took about 15 minutes for each eye. The first thing I saw was the clock; I could actually read its face clearly without lenses or glasses. It was the first time I’d been able to do that since I was little and I felt a bit tearful. My eyes were a bit sore for a few days and I had to use steroid drops for a week, but they have now fully recovered. My eyesight is 20/20 in one eye and 20/15 in the other which is perfect. I don’t even have any problems with glare when I drive at night. To this day, I can’t believe that I can see without my glasses. I can’t tell you what it means to me to know that, when I marry Mike later this year, I’ll be able to walk down the aisle and see his face, and everyone else’s, clearly. More than 1,000 attended from 48 countries to hear about the latest clinical research and product innovations. Bausch & Lomb were proud to be the Platinum Sponsor of the BCLA again during 2008. Highlights from the 2008 BCLA Clinical Conference began with the opening session on Thursday morning; ‘Patient and Practitioner non-compliance and how to improve it’ a theme that was very much a hot topic throughout the Conference. Despite a power failure lasting throughout most of the morning session, all speakers delivered excellent lectures captivating the audience despite no slides or microphones. The ‘Science of Compliance’ was the session delivered by Nick Pope, Director of Sales Training EMEA for Bausch & Lomb. This lecture explored the powers of influential communication that can affect our patients’ compliance to practitioner recommendations. Not just in following contact lens care and lens replacement instructions, but related to improving the rate of ‘DNA’ (did not The dynamics of the eyelids play a tremendous role in the stabilisation of toric contact lenses. Craig Josephson delivered a paper to assess high speed camera technology and motion detection software in the evaluation and study of blinking dynamics. The physiological descriptors of the normal blink reveal important information which will influence the success of toric lens designs. Such as the almost entirely horizontal motion of the lower lid and that it may play only a minimal role in toric lens stabilisation. Innovation is key to expanding the market. Ravi Sharma, a principal scientist of Bausch & Lomb R&D, discussed antimicrobial silver technology in contact lenses and lens care. Ravi provided a review of current ophthalmic product applications, the methods used to incorporate silver into contact lenses and the challenges related to this technology. Will the industry embrace this as the next step in lens innovation? Merchea concluded that there was no difference in acute inflammatory response with the extent of contact lens solution-associated staining.
