Applications are invited fol engagement of Block ASHA Facilitator on purely contractual basis
under ASHA programme for Bishnupur Sub-Division. The Monthly salary of Block ASHA
Facilitator will be Rs. 7500.00 (rupees seven thousand and five hundred) only. In addition, he
/she will be eligible for a monthly mobility support of, Rs. 1500.00 (one thousand and five
hundred) only. Block ASHA Facilitators will be contractually engaged for a period of one year
renewable based on budgetary sanction and performance.

Selection Criteria
1.1)
Master's degree in Social Science/ Sociology/ Social Anthropology lsocial Work (MSW) /
Business Administration (MBA) / Economics / Rural Development / Mass Communication

OR
Graduate degree in any discipline with minimum2 years experience in health projects
r.2) Preference will be given to candidates having working experience in ASHA programme.

Age should not exceed 40 years as on 01.01 .2}l4.Upper age relaxation will be 5 years for
SC/ST and 3 years for OBC.
2) Selection Process:
The selection will be based on merit (academic result) and experience in health projects,
written test and Computer Skill Test .Weightage will be given as per guideline vide G. O No
HFW / NRHM I 2721 2010 I 875,Dated -26102114.

Mode of application:-

Candidates should submit applications in the format annexed as "Annexure- A" to this
notification. Application form will be downloaded from www.bankura.gov.in.
The application should be on good quality paper (Legal Size) using one side only.
Candidates should ensure that the application is made on a single sheet conforming to the above
specification.
Candidates should fill up the application form in his/ her own handwriting in English with blue
ball point pen only.

{Continuous engagement in a health related project
with an assigned designation, during
the entire tenure furnished as experience will only be considered as "experience in health
project". No certificates specifying that the candidate has worked as volunteer e.g. in Pulse Polio,
social work, leprosy etc will be considered as experience in health related project.)

Self attested photocopy of Caste Certificate from Competent Authority (for reserved category).
Two self addressed envelope pasted with postal stamp @ Rs.5 on each envelope.
Demand Draft of Rs. 100"00 (Rupees One hundred) only for UNRESERVED categories and Rs.
50.00 (Rupees Fifty) only for RESERVED categories, issued from any Nationalized Bank in
favour of "S.D.O. BISHNUPUR" payable at Bishnupur.

A recent passport size colour photograph must be pasted on the application form.
On the envelope of application containing the application, the applicant must write "Application
for the post of Block ASHA Facilitator".
6. Last date and address for submission of application
Application with all relevant documents must be submitted through post or applicant may drop
the application personally in the specific box placed at Office of the SDO, Bishnupur. The last date of
submission of application is 30/10/2015 up to 05:00 p.m.

7. Invalid applications:-
Applications suffering from the following deficiencies
illustrative and not exhaustive.

will be rejected. The list is only
received after the closing date & time.

Application not in prescribed format or which is incomplete in any manner or in non
standard size of paper.

Candidate is not in possession of the required educational qualification on the date
application or is over aged as on 01 .01.2014.

More than one application submitted by the same candidate.

o Application without full signature.
o Copies of required enclosures as mentioned above (under point no 5) are not enclosed.
. Any other discrepancies noticed in the application at the time of scrutiny will be liable
for rejection of his / her candidature.
o Incomplete Application in any point of view will be summarily rejected.
8. General Conditions :-

After verification, scrutiny of applications, the list of valid candidates, fulfilling the
eligibility criteria will be published in ww:ylgilkuaggy$. Only eligible candidates
will be called for Computer and written test.

Valid list of candidates and Notice for date, time, venue for written test and computer
skilled test will be published in rvi,vw.bankura..sov.in. Candidates are requested to visit
the website time to time regularly.
All original certificates will
be verified at the time of computer test/written test.
No TA/DA will be allowed to attend the written test and computer skill test.
Mere selection and empanelment does not confer any right of appointment to the
candidates.
Admission of a candidate at all stages of recruitment will be purely provisional, subject
to satisfiring prescribed conditions.
Sub-Divisional Level Selection Committee of Block ASHA Facilitator reseryes the right
to alter the mode of examinations or conduct re-interview or cancel part or whole of any
process of recruitment at any stage, if needed.
