has been delivered, simply
follow these 6 easy steps
to get your grocery
gift cards!
All steps must be completed within 30 days
of receipt of merchandise. One grocery
redemption certificate per household.
FIRST STEP:
7 days after your merchandise is delivered, log onto
www.mpellsolutions.com/bernieandphylsgrocery500
or call toll-free 800-450-1575, 12pm-8pm.
SECOND STEP:
Print the Official $500 Grocery Registration.
THIRD STEP:
Complete the form on the Official Grocery Registration and register your
Supermarket choice
FOURTH STEP:
Make a copy of a valid picture ID. Driver’s license, passport, or any other ID with
picture can be used. You may cross off identifying numbers.
FIFTH STEP:
Make a copy of your Bernie and Phyl’s Furniture sales order for at least $500
(tax and delivery excluded).
SIXTH STEP:
Enclose in an envelope: The completed Grocery Registration, Copy of your valid
picture ID and a copy of your Bernie and Phyl’s sales order.
Mail envelope to:

Once you have received your five grocery certificates
Every month enclose in an envelope:
-One of the Five Grocery certificates from your grocery book
-A minimum of $200 in grocery receipts from your registered supermarket
(maximum of 2 receipts) and mail to above address to receive one of your five $100
Gift Cards.
Important: All steps listed above must be completed and postmarked within 30 days of receipt of
merchandise. If you fail to redeem your certificate on any given month, the entire offer is invalid and
you will not receive your grocery gift cards. First and last names must match on ID, certificate, and sales
order. Please allow 8-10 weeks for delivery of your grocery book. If you have any questions or concerns, please call 508-286-4000. Employees of Bernie and Phyl’s Furniture are not eligible. Not valid
with any other promotions including long term financing. (Note: If merchandise is returned after 7 days a
20% restocking fee will be charged.) Cannot be combined with any other promotion. Prior sales excluded.
