Dear Friends,
For the past thirty years, homeless families and individuals
have turned to the Colorado Coalition for the Homeless to
help them end their homelessness. Through a combination of
integrated health care, housing, employment, and supportive
services, we have helped tens of thousands leave the streets
and shelters behind for new homes and opportunities.
2014 was a year of continued hope and recovery for those
served by the Coalition. With the opening of our new
Stout Street Health Center, we were able to expand
integrated health, behavioral health, dental, vision, and
pharmacy services for homeless and at-risk families and
individuals. Renaissance Stout Street Lofts added 78
supportive housing apartments, bringing the total number of
supportive housing units developed by the Coalition to 1,600.
In the pages that follow, you’ll learn more about what
our dedicated employees and volunteers do to help
transform the lives of men, women and children across
the state—creating lasting solutions to homelessness.

STOUT STREET HEALTH CENTER
In the summer of 2014, the Coalition opened the innovative and nationally
recognized Stout Street Health Center and Renaissance Stout Street Lofts.
This project delivers fully integrated health care access for up to 18,000
homeless individuals each year while providing supportive housing for
78 formerly homeless households.

INTEGRATED CARE
Bonnie Benjamin, Registered Nurse, Janna Petrie,
Green Suite Coordinator, Kerry Kjellsen, Medical Assistant,
and Kaley Fornelli, Psychiatric Nurse Practitioner, meet
to create an integrated care plan that incorporates
medical and behavioral health interventions designed
specifically for a patient in their care. Skilled screening,
assessment, evaluation, and treatment in one location,
from one care plan, are crucial in serving patients
experiencing homelessness.
Mark is visiting the Health Center to treat a shoulder he
dislocated slipping on ice. Since he lost the construction
jobs he’s held for the last eighteen years, he’s been
sleeping in a tent along Speer Boulevard. “It’s cold. I wake
up with two inches of ice inside my tent every day,
every day.” Mark originally came to the Health Center
because of the pain in his shoulder, but thanks to the
Integrated Model, his care plan includes behavioral
health care, housing assistance, and dental and vision
care tailored to his needs.

DEDICATED TEAM OF VOLUTNEERS
For over a decade, the Coalition has hosted the annual Back to School Healthy Kids Fair (right).
More than 200 health care professionals and community members volunteer to provide
hundreds of homeless children immunizations, vision, hearing, and dental screenings, and
backpacks filled with school supplies, giving the children a healthy start to the new school year.

ACCESS TO CARE
The expansion of Medicaid has enabled newly insured patients to access many services that
they could not previously obtain while uninsured, particularly specialty services, behavioral
health services, and medications. Every day at the Stout Street Health Center, Medicaid
enrollment specialists help patients apply for Medicaid coverage so they can receive the full
spectrum of services and procedures necessary to ensure strong health outcomes.
The Coalition’s mobile medical clinic (far left, bottom) takes health care on the road, delivering
services in the day and evening hours and meeting patients where they are—on the streets,
at homeless shelters and drop-in centers. The Health Outreach Program (HOP) helps address
typical barriers to health care such as lack of transportation or scheduling difficulties for
the working homeless.
A SAFE PLACE TO REST AND RECOVER
Thanks to the Coalition’s Medical Respite Program, Harvey Moon (left) has a place to heal
from an infection that escalated to amputation while he was living on the streets. The Respite
Program offers hospitals an alternative to discharging patients to the streets. Individuals with
complex medical and behavioral health needs are able to recover in a safe environment.

MANAGING MEDICATIONS
In the Stout Street Health Center, medications are prescribed thoughtfully in order to meet
the unique needs of each patient. An on-site Pharmacy dispenses these medications at no
cost to patients.
After a decade of instability, Juanita (left) now lives in Renaissance Stout Street Lofts.
Because she suffers from rheumatoid arthritis, she loves having the Stout Street Health Center
next door. “I pick up my medications, and just made a dental appointment. Plus, because of my
medications, I have to have eye exams every six months which is easy to do right next door.”
Claudine is building a new life. She’s feeling healthier than she has in years, and will finish
her English Literature Degree in the fall. “Even though it’s been 40 years,” she says about
going back to school, “I can do anything. When I say I’m going to get something done,
I work very hard to get it done.” Pictured below (right), she is waiting for an appointment
at the Health Center to treat her thyroid condition.

power all common areas in the 100 unit building. All Coalition properties are built “green” to
promote healthy environments and to save on utility and energy costs. The Lofts demonstrate
that environmentally friendly construction can be the standard for affordable housing.

AFFORDABLE HOUSING TO MEET COMPLEX NEEDS
In over 30 years of service, the Coalition has developed a variety of supportive, affordable housing
developments in Colorado. The Coalition’s integrated housing approach combines high-quality housing
for homeless families with affordable homes for individuals and families with lower incomes.
Services such as counseling, life skills training, financial literacy and employment assistance contribute
to housing stability for those who once were homeless. Quality architectural designs add significant value
to neighborhoods and cultivate pride and well-being among residents and the surrounding community.
Renaissance at Concord Plaza (above, right) in Lakewood, Colorado, is adjacent to the Renaissance
Children’s Center and provides 76 units of housing for formerly homeless families integrated with
mixed-income, affordable apartment homes.

Families with children are the fastest growing segment of the homeless population1, due to a critical shortage
of affordable housing across the country.
Unaddressed early childhood trauma is often an unidentified trigger for mental and physical disease, addiction,
and disability in the United States.2 The Coalition seeks to incorporate interventions that address the impact
of multiple traumas on the children we serve. The Renaissance Children’s Center (below, bottom) provides a
safe, stable environment for children to develop the social and emotional skills necessary to succeed.
Children experiencing homelessness have to change schools frequently, sometimes multiple times a year.
With each transfer, a child loses an estimated six months of academic progress. These children are nine times
more likely to repeat a grade and four times more likely to drop out of school, making it very unlikely that they
will continue on to higher education.3 The Coalition helped this mother (left) complete her education and serve
as a powerful role model for her daughter.

SERVING THOSE WHO’VE SERVED
The Fort Lyon Supportive Residential Community (below) provides recovery-oriented
transitional housing to homeless individuals. The program combines housing with
peer support, and educational, vocational and employment services for homeless
and formerly homeless persons from across Colorado, with an emphasis on serving
homeless Veterans.

VULNERABILITY OF HOMELESS WOMEN
The United States has the largest number of homeless women among industrialized nations and the
highest number on record since the Great Depression.1 Domestic violence against women is the principal
cause of women’s homelessness.2
