TELEMONITORING
Our Technology With the help of hospital-­‐grade in-­‐home monitors, you can check your vital signs daily. These vital signs are then transmitted to a central monitoring station where our nurses review and contact you in the event of any abnormal readings. This technology paired with our clinician’s home visits is our way of caring for you every day. Telehealth Benefits Telemonitoring also assists us in providing disease management services to you if you have a chronic health condition such as diabetes, heart and/or lung disease. We monitor your condition daily and can detect changes quickly. These changes are communicated to your physician. He/she is able to make changes in your care to prevent trips to the hospital or emergency room. Types of Monitors THA Group uses 3 types of telemonitors: Cardiocom, Philips, and Honeywell/HomMed. Your nurse or therapist will explain in detail how to use your telemonitor. The following pages provide instructions for your particular telemonitor. Telestation: Will alert you each day to take your vital signs (may ask you to answer questions as well). Note: If you get an invalid reading on your measurement device, do not enter it in the TeleStation. If you accidently turn on the measurement screen, do not press any buttons.
