2015 Year In Review
Dedication — Commitment — Compassion
Leading to Hospital of Excellence, Growth and Quality Service
O
n behalf of our Senior Management Team, I would like to take
this opportunity to THANK YOU—employees, medical staff,
and volunteers—for your dedication, commitment and compassion
to Lakeview Regional’s patients. We realize that it is not only a
responsibility to serve others, but also a privilege. I am honored to
work with such an amazing group of individuals.
During 2015, we experienced the continuation of positive momentum,
even as we faced many challenges and changes in the healthcare
industry. Thanks to you, we have much good news to report!
Our Employee, Physician and Patient Satisfaction Scores continued
to remain near the top of all HCA Hospitals across the nation. This
is truly exciting news! We will continue to implement and maintain
open lines of communication, enabling us to address all needs and
concerns in a timely manner. We would like to share our “2015 Year
In Review” with our Lakeview family and the community. This report
presents various achievements, awards, accreditations and community
service events illustrating that your dedication, commitment and
compassion are the reasons that we have met and/or exceeded our
goals and objectives for the year, while continuing to give back to
our community.

ER Campaign— “We Are ER”
Lakeview Regional Medical Center is focused on emergency
care and being better prepared In Case of Emergency (ICE).
Imagine waiting at home for an expected loved one, but
they never arrive. You begin calling hospitals and they do
not have your family member there, although they do have
“John Does.” Now imagine you were in an accident on the
way home from work and you were unable to speak. How
would the paramedics contact your loved ones? These reallife scenarios can be prevented with just one ICE Key Tag.
When it comes to receiving true “emergency” care, Lakeview
Regional is now offering an added assurance for patients. The
ICE (In Case of Emergency) Key Tag was recently developed
as a means to alert first responders and hospital personnel of who to
call if a patient is unresponsive during an emergency. That person can
then alert the medical team of any allergies, medications or specific
medical conditions. “When seconds count, the ICE Key Tag can
mean life or death,” states Dr. Charles Muntan, Lakeview Regional’s
Emergency Department Medical Director. “If an unconscious patient
is brought to us, we can certainly provide the necessary treatment,
however, it is most desirable to find out as much information about a
patient in order to deliver the optimum care.”
Lakeview Regional Medical Center is centrally located between
Mandeville and Covington, making it the provider of choice when
an emergency arises. Lakeview Regional is staffed with boardcertified, emergency trained physicians specializing in chest pain,
heart attacks, stroke, and is the first facility to offer 24-hour trauma
coverage, including trauma surgery. Lakeview Regional was also the
first hospital on the Northshore to offer 24/7 neurosurgery services
for patients.
“At Lakeview Regional, we are dedicated to providing the best care
possible,” states Bret Kolman, Lakeview Regional’s Chief Executive
Officer. “Being able to reach emergency contacts helps us achieve
that goal; and in turn offers a sense of assurance for our patients.”

We Fix Broken Hearts
When Jeff’s chest pain got so bad that he knew he couldn’t ignore it, he thought
about driving across the lake to seek care. At the last minute, Jeff decided
instead to drive the two blocks to Lakeview Regional’s ER.* Shortly after arriving
at Lakeview Regional, Jeff collapsed on the floor, unresponsive.
Jeff had a heart attack.
Jeff realizes now that his decision not to drive the Causeway not only saved his life,
but probably the lives of others.
Put your heart in our hands.

In Better Health Patient Testimonial—
April Picou & Dr. Frank Arena story featured in Inside Northside Magazine
Just over two years ago, April Picou was a happy, healthy 40-year-old who
enjoyed a vibrant life and worked offshore for a catering company. But all
that changed one day when she began having chest pains at work and was
taken to a hospital close by. While there, she underwent an angiogram— a
diagnostic tool used to uncover heart and blood vessel conditions. April’s heart
turned out to be fine and her symptoms resolved. But within three weeks of the
procedure she began having horrible pain in her right leg.
“My primary care doctor ordered an ultrasound to see what could be wrong.
I remember the tech stopping in the middle of the procedure to rush to get
the doctor,” April says. The ultrasound showed a major blood clot that had
settled in April’s hip following the angiogram. The complication is very rare
but is a known potential risk of the procedure.
Over the next two years, April underwent multiple procedures to clear the blockages, including three femoral
bypasses. But the clots kept returning, as did the horrific leg pain that came with them. The procedures eventually
made things worse and caused blockages all the way down to April’s lower leg. She couldn’t sleep or stand and was
eventually wheelchair bound as walking was impossible. She lost her job and had virtually given up hope of having
a normal life again. With seemingly no other options, April decided that amputation of her leg was her only choice.
Prior to the surgery, April was told to get clearance from a cardiologist and was referred to Dr. Frank Arena, lead
physician at the Cardiac Vascular and Vein Center at Lakeview Regional Medical Center in Covington. April says
everything changed during that appointment.
“Dr. Arena told me he thought he could save my leg with an angioplasty. At first, I told him ‘no’ because by that
time I already had my mind set on the amputation. I had a second opinion that agreed I should amputate. I didn’t
want to get my hopes up—I hadn’t walked in two years when I met Dr. Arena,” says April. “But he convinced me
to try. He told me not to give up, and that if I was willing to fight, he would fight with me. After he walked out of
the room, I cried. I said to myself, ‘This has got to be my chance.’”
The procedure Dr. Arena suggested would attempt to open blockages in April’s damaged blood vessels to restore
better blood flow. Dr. Arena compares the vessels in the leg to an interstate system—most of the exits on April’s
interstate were closed. Not having blood flow was causing her leg to die, and restoring flow was not going to be
easy. He had to study April’s case and the notes from her past procedures extensively to determine how her native
anatomy had been reworked.
“All we were left to work with really were grafts,” explains Dr. Arena. “We just had to make what we were left
with work.” The angioplasty, which coincidentally was performed on the same date that April’s leg amputation
surgery was scheduled, was successful. After a few months of rehabilitation, April was walking and enjoying life
again. She now works six hours at a time on her feet cooking in her daughter’s café and enjoys playing with her
grandchildren. Her last ultrasound showed good flow in her leg, as well as positive signs that her remaining native
anatomy has maintained since the procedure. April may need a follow up angioplasty every five years or so to
maintain good blood flow, but Dr. Arena is optimistic about her prognosis.
Dr. Arena says that an angiogram is a common procedure that diagnoses real problems and can lead to lives being
saved, but he stresses that all procedures come with risks. He encourages all patients who undergo an angiogram to
recognize the signs and symptoms of blood clots and know where to go to treat them. Most important is choosing
a doctor who uses an experienced team in the catheterization lab, who will listen closely to patient concerns and
who will pay attention to details.
April knows how lucky she is to have found a doctor as passionately aggressive as Dr. Arena. She credits him with
not just saving her leg, but giving her back a normal life. “I thank God every day for him,” she says. “He’s my angel!”

Trauma Coverage Press Release
24-HOUR TRAUMA COVERAGE NOW AVAILABLE AT LAKEVIEW REGIONAL MEDICAL CENTER
Specialized Trauma Surgeons provide around the clock treatment.
Lakeview Regional Medical Center recently announced it has expanded its Emergency Department services to
include 24-hour trauma coverage, including trauma surgery. Dr. Marco Hidalgo has joined the staff at Lakeview
Regional Medical Center as the Medical Director of Trauma, overseeing the treatment of trauma cases involving
motor vehicle accidents, sports injuries, falls, and other life-threatening injuries from traumatic events.
Lakeview Regional Medical Center has a variety of surgeons and specialists on site 24-hours a day to respond
within minutes of a trauma call. “When a patient arrives at a hospital within 60 minutes, or the ‘Golden Hour,’
they have better outcomes,” states Dr. Hidalgo. “Time can be the difference between life and death.” A key piece
of being able to handle trauma in an emergency room is having depth in specialties such as neurosurgery, because
concussions and head injuries are often part of trauma injuries. “Lakeview Regional has six neurosurgeons on staff
and has the highest level of neurosurgical capabilities in St. Tammany Parish, states Bret Kolman, Chief Executive
Officer for Lakeview Regional. We are committed to providing a comprehensive, multi-disciplinary team of
physicians for every patient in St. Tammany Parish.”
Dr. Hidalgo graduated from Tulane University, where he received a Master’s Degree in Cell and Molecular
Biology, as well as his Medical Degree. He served his residency at Tulane as a General Surgery Resident and Chief
Administrative Resident. Dr. Hidalgo worked as a staff surgeon at River Region Hospital in Vicksburg, Mississippi
until the time he came to Lakeview Regional. He has also been trained in wound care, including hyperbarics, and
has received extensive training in Da Vinci robot single site surgery. He is a member of the American College of
Surgeons and has written several peer review articles and presented oral presentations.

an accurate tidal volume and rate.
Alvin Rogers was promoted to Manager of Respiratory Services. Alvin joined
Lakeview Regional 5 years ago and brings a wealth of experience to his new role.
The respiratory department achieved all core measure goals for 2015, which include
initial patient assessment within 24 hours was at 95%, medication scan rate was at
98%, and initial patient education within 24 hours was at 93%.
Micah King consistently looks for opportunities to improve the respiratory
department. He was involved in creating a new code bag process that is concise and
more efficient.
Cardiology received a new Vivid E95 Echo machine that takes 4D ultrasound to a
whole new level—potentially helping reduce non-diagnostic exam and cost of care.
Compared to many conventional echo machines, the system acquires more information up front, enabling True
Confocal Imaging and Adaptive Contrast Enhancement for robust image quality.
Alvin Rogers
Amber Finklea, the Cardiology Supervisor continues to exceed expectations as a leader. She did an incredible job
at introducing and educating all the staff members on the use of the new Vivid E95 ECHO machine.
Cardiology maintained 100% compliance with scope cleaning in 2015. They also attained 90% compliance for
echocardiograms dictated within 48 hours post procedure.
Lakeview Regional had the lowest Employee Turnover Rate
in the entire MidAmerica Division— less than 10%.

ICU Nursing Team,
Stroke Gold Plus Award
Improved communication amongst staff member’s hospital wide—from patient care techs, radiology, cardiology,
lab and nurses. Patients and families voiced enthusiasm of our new process!
Shift change is completed in 15 to 20 minutes in comparison to 30 to 45 minutes prior to implementation of
bedside reporting.
Social Work
2015 was a busy year for the social work department. We added two
new social workers, Alexis Wolcott and Kate Walker. They are
board certified and bring skill and enthusiasm to our department.
Alexis and Kate are both trained in case management and
psychotherapy. Also during this year, three of our social workers:
Beverley Connor LCSW, Jill Chapoton LCSW, and Tessie Rafferty
LCSW, were certified as Advanced Certified Social Work Case
Managers with the National Association of Social Workers. That
is a great honor and takes a good bit of work. We are fortunate at
Lakeview Regional to have such a skilled group of professionals.
Social Work
Social work is integrated throughout our spectrum of care. Social work
does screenings for all high-risk patients at the acute hospital, participates in the interdisciplinary care team that
meets each Tuesday to review patient needs and is active with the Stroke Program. Social workers see all families
with babies in the NICU and screen for abuse of both adults and children.
Social work is also active in community education and screening. Beverley Connor LCSW participated in the
Stroke Symposium in October to lend her expertise and good humor.
Jill Chapoton, LCSW and Debby Kreller, LCSW became CPI certified in July 2015. They join current
CPI instructors Rosa Moses, RN and Danielle Fath, RN.

Departmental News
Debby Kreller, LCSW serves on the board of the National Alliance of Mental Illness, and the social work team is
involved with the St. Tammany Parish Mental Health Task Force.
The social work department, in conjunction with the Case Management Team, has hosted a series of in-service
presentations to ensure that we are up-to-date with the current resources in the community. The social work
team is available both in the hospital and on call 24-hours a day. Social work is instrumental in the placement of
psychiatric patients in the Emergency Department.
As you can see we are an active group and are available to assist as needed throughout the hospital. Social work is
available by physician consult or by employee request.

GIRL’S HEALTH DAY
Lakeview Regional partnered with the Junior League of Greater Covington
to host the Second Annual Girl’s Health Day. Three speakers and 15 vendors
presented topics related to girl’s health issues to an audience of 100 people.
The theme, “Be Healthy. Be Happy. Be You.,” focused on self-esteem, healthy
eating, and internet safety.
“Girls’ Health Day was designed to promote healthy, positive behaviors in girls
between the ages of 6 and 16,” states Dr. Brandee Simon-Davis, Girls’ Health
Day organizer and physician at Lakeview Regional Medical Center. “When girls
are given the tools they need to be strong at a young age, they will grow up to
be strong women, and know how to make responsible and healthy choices.”
Girls took part in discussions presented by Sherrí Hansen (owner, Culinary Kids),
Cathy Williams, Steve Babin,
Christine Howell (local practicing counselor), Emily Ruffino (Franco’s trainer
& Katie Baird
and nutritionist), Catherine Wilburn, MD (owner of Nutrition Company), and
Noah Cockfield, IT Director (social media safety and internet safety). Louisiana
State Troopers discussed seat belt and booster seat safety and distributed materials on bullying. Girls learned
about making healthy food choices from the Junior League Kids in the Kitchen project and participated in
fitness and interactive health activities. Several local health and wellness vendors provided information to help
girls educate themselves on topics relevant to their own lives. Other activities included free health screenings
(vision, hearing, BMI measurements and blood pressure assessments) by
local physicians.
The event also
included an
opportunity to
“Be a Doctor for the
Day,” by performing
simulated surgical
activities using
the da Vinci®
surgical robot.

Lakeview Regional Medical Center hosted the
3rd Annual Kids’ Town EGGStravaganza on “the
Green” on Sunday, March 29th. 3,800 people
attended and $15,800 was raised for Kids Town.
The event, which took place adjacent the pond
facing the hospital, featured three different agelevel Easter egg hunts, special eggs with Elmer
Candy coupons, food and drink, arts & crafts
tents, entertainment, photos with the Walgreens
Easter Bunny, and the Corporate Baby Duck
Beauty Pageant. The highlight of the day was the
Rubber Duck Regatta Race. All proceeds were
dedicated to the construction of KidsTown, a
child-sized, western-style railroad town designed
for interactive play that will include a fire station, sheriff’s office, general store, and more.
“We are thrilled to host this fun, family event that will help fund Kids Town,” states Kim Melvin, LRMC Director
of Marketing. “The Tammany Trace Foundation shares our commitment to the children of this community, and
our combined efforts will assist in continuing to enhance our quality of life in St. Tammany.” The Tammany Trace

AMERICAN HEART ASSOCIATION
NAMES LAKEVIEW REGIONAL
A GOLD LEVEL FIT-FRIENDLY
WORKSITE
Walking Path set as site for Stroller Walk
Meet-Up for Moms with Strollers
Lakeview Regional Medical Center is pleased to
announce it has been named to the American
Heart Association’s annual list of Gold FitFriendly Worksites. This is the second year
Lakeview Regional has received this designation.
The American Heart Association’s Fit-Friendly
Worksites Recognition Program recognizes
employers who champion the health of their
employees by creating physical activity programs within the workplace. In 2009, Lakeview Regional introduced
the Start! Walking Path as part of the national initiative to be a catalyst for positive change in American business.
Recently, the path, which measures 3 tenths of a mile long, was selected as the first American Heart Association
officially designated Walking Path on the Northshore. “We are proud to be recognized as a leader in the national
movement to improve corporate wellness,” states Bret Kolman, Lakeview Regional’s Chief Executive Officer. “On
a daily basis, Lakeview Regional is committed to providing physical activity and wellness opportunities for our
employees to improve their overall health and well-being.” Lakeview Regional’s commitment to a healthier workplace
environment also includes a comprehensive wellness program and, more recently, the addition of an H2U Health
Center, both of which are creating a culture of health within the hospital community. In addition to promoting
healthier lifestyles to its employees, Lakeview Regional is committed to offering opportunities for better health to the
community. On Tuesday, June 23 at 10AM, moms with strollers were invited to a Stroller Meet-up where they can join
other moms for a walk along the Start! Walking Path.
According to the American Heart Association, studies suggest that implementing a worksite physical activity
program and promoting a culture of activity will help increase productivity, reduce absenteeism, lower turnover
and reduce healthcare costs. Lakeview Regional’s recognition as a Gold Level Fit-Friendly Worksite will serve as an
encouragement to other businesses to participate and demonstrate similar activity practices for their employees.

During National EMS Week,

of medicine’s “front line.” At the

event, one patient was reunited with
Jeanne Terrebonne, RN ED
her EMS crew and medical staff
that was instrumental in her care
and subsequent recovery, providing a rare opportunity to say “thank you,” a step that is often a significant part of the
healing process.
“We thought National EMS Week was the perfect time to honor the emergency medical services personnel for their
fast, efficient, and compassionate treatment,” states Bret Kolman, Lakeview Regional Chief Executive Officer. “If
it wasn’t for the immediate response of these individuals many would not have survived or would have faced severe
debilitating deficits.” First responders seldom learn of the outcomes of the patients they treat. This Great Save Event,
which is set to become a quarterly event, will offer a chance for updates and reunions.
EMS Patient Testimonial — Mrs. MARY Folse
During a recent stay with family in Covington, Mr. Folse found his wife
on the ground unresponsive. Paramedics from Fire Department 12 who
responded were expecting a head injury, but upon assessment of Mrs.
Folse’s condition they realized it was more likely a stroke. Paramedics
immediately took Mrs. Folse to Lakeview Regional Medical Center.
During transport, Lakeview Regional activated Code Purple, an internal
alert for a potential stroke patient which mobilizes appropriate staff
and ensures the CT scanner is ready. Mrs. Folse had a significant stroke
involving a blood clot, which required administration of tPA to help
dissolve the clot and restore blood flow to the area of her brain that was
affected. As any medical decision can be frightening, Mr. Folse relied
on the expertise of ER physician Dr. Jay DeSalvo and the neurologist on
call, Dr. Ramy El Khoury. He said, “Do for my wife whatever you would
do for yours.”

her “rebirth” at

on the night of
her stroke.
Although tPA administration is given at Lakeview,
the nature of Mrs. Folse’s stroke necessitated that she
be transported to a higher level of stroke care in order
for her to have the best chance of a positive outcome.
She was transported by Acadian Ambulance to Tulane
Medical Center, a comprehensive stroke center with
interventional capabilities, not currently available on
the Northshore. Following her procedure and inpatient
rehabilitation therapy, Mrs. Folse returned to Covington
to live with her daughter and continue outpatient
rehabilitation therapy and occupational therapy
at Lakeview Regional. She is doing extremely well
considering the severity of her stroke.
Mrs. Folse’s
husband spoke on
her behalf at the
event. He relayed
the story of that
night, his voice

he recounted the
event. He spoke

Administration of the hospital also thanked the EMS
crews for their ability to assess that Mrs. Folse was
having a stroke and not a head injury, and for making
the decision to bring her to Lakeview’s ER, a certified
primary stroke center.
On May 21, Lakeview Regional hosted their firstever EMS Great Save Event, highlighting Mrs.
Folse’s story and re-uniting her with the paramedics
and ER staff who cared for her that night. The
event was held during EMS Appreciation week
as a way of highlighting the importance of the
relationship between first responders and hospitals in
emergency care. Medics from both Fire Department
12 and Acadian Ambulance were present to receive
recognition for their roles, and Mrs. Folse was presented
with a new birth certificate commemorating

6th Annual Boo Fest
The 6th Annual Boo Fest had the largest crowd yet with over 9,000 people,
sixty decorated houses were on display to children of all abilities to visit,
and 20 chili cook off teams competed. We raised over $50,000 for ACCESS
(Adapting and Changing Child’s Environments with Successful Solutions).
Dr. Aaron Karlin was interviewed on FOX News 8, and discussed Lakeview
Regional Medical Center’s involvement in hosting the 6th Annual Boo Fest, an
event benefiting children with disabilities, and open to all children and their
families. The family-friendly day packed with lots of fall-themed activities,
included pumpkin decorating at the pumpkin patch, face painting, all-abilities
games, a photo booth, music and dancing provided by a DJ, a visit by State
Police Troop L, and trick or treating to nearly 60 “gingerbread-style” houses
and castles! Children wore their favorite Halloween costumes and took part in
the annual Boo Fest costume contest for the title of “Scariest,” “Most Original,”
“Crowd Favorite,” and “Best Group” costume.

Breakfast with Santa
We hosted Breakfast with Santa on December 5th. All children ages 3–8
were invited to join in the fun, including breakfast, crafts, and a photo with
Santa. Admission to the event was one unwrapped toy which was donated
to St. Tammany Project Christmas. “Most of us know the joy of Christmas
morning and relish in the excitement of our children when they see that
Santa has come,” stated Bret Kolman, Chief Executive Officer for Lakeview
Regional. “But for thousands of children right here in our own community
that experience doesn’t always happen. We are humbled to be a part of
making those happy Christmas memories for some of the most disadvantaged
families in St. Tammany Parish.”
According to organizers of St. Tammany Project Christmas, 1,500 children
from nearly 500 families across St. Tammany Parish were presented with gifts
in 2015. The organization was established in 2002 to give the needy families
of St. Tammany Parish a complete Christmas. In addition to gifts for the
children, families received food, including a ham or turkey.
