Below is an example of a Memorandum of Agreement (MOA) or Memorandum of
Understanding (MOU) between an academic institution and a community partner (e.g.
community health center, community-based organization, etc.) that is initiated by the
academic institution. The academic institution approached community health centers to
participate in an initiative that aims to build primary care research infrastructure.
Memorandum of Agreement (MOA) Between [Academic Institution] And [Community Partner Organization] This is an agreement entered into on this date, [date], between the [Academic Institution] and [CP Organization]. The [Academic Institution] has received funding from [Grant source] to provide grants to institutions participating in the [Initiative/program]. Program Objectives and Activities: The [Initiative/program] will partner with [CP Organization] to facilitate organizational and individual provider capacity to engage in primary care research. Project activities include working with health center “champions” in primary care research training and proposal development, providing ongoing education to the provider community, and related evaluation activities. Date for this MOA: The project period is [Project start and end date]. This memorandum is intended to cover the entire project period. Responsibilities: In consideration of the mutual agreements set forth herein, the parties agree to the following as a part of their involvement in [Initiative/program]: I.) [Academic Institution] will: • Disburse the grant of [$ amount] to [CP Organization] in two payments of [$ amount] each at the [Start date] and during [Middle date] of project period II.) Below is an example of a Memorandum of Agreement (MOA) or Memorandum of
Understanding (MOU) between a community partner/organization and an academic
institution. This agreement initiated by a community partner to establish and work in
partnerships with an academic institution using a community-based participatory research
approach to evaluate a program targeting men of color to increase healthy lifestyles
particularly around physical activity and nutritious eating.
Memorandum of Agreement (MOA) Between [Community Partner Organization] And [Academic Institution] The [CP Organization] is an initiative which addresses the prevention of cardiovascular disease and type 2 diabetes, particularly in men of color. The initiative has designed integrated programs to increase physical activity, support healthy eating, and educate men about key health topics through a mentorship program. This agreement of collaboration establishes the principles of community participatory research that are generally applied in [CP Organization] projects with academic partners. Under this agreement, [CP Organization] serves as a guardian for these research principles on behalf of community, and facilitates the equal participation in community research of collaborating communities and academic partners. The project period is [Project start and end date]. Partnership Principles Community Participatory Research Principles: This project will follow principles of community participatory research (Minkler and Wallerstein, 2002). The underlying principles are: 1) The project seeks to enhance the community’s wellbeing through empowering the community to address its own health issues. 2) The project will be designed to increase community knowledge of the issue. 3) Community and academic participants will be involved in all project phases, including planning, implementation, research and evaluation, analysis, interpretation, and dissemination. a. Community participants and academic researchers contribute to the development of the project issues and scope. b. Interested members of the community and community agencies will be provided opportunities to participate in the research process. e. All participating members (academic and community participants) are acknowledged as having expertise and commitment that is relevant to the scope of the project. f. conclusions, and supported as needed in the research and scientific methodology. g. Community participants will be partnered with academic participants in identifying the relevant project outcomes. h. The project will periodically assess the experience of participating for community and academic participants and address their concerns. 4) Dissemination of the research results will be the responsibility of all project participants, and academic and community partners will have opportunities for presentations and publications (see below). Rights, Conflicts, and Responsibilities [Academic Institution] responsibility: • Collaboratively develop an evaluation plan to assess the program’s impact on participants health • Assist in providing resources and technical assistance in activities supporting the design and implementation of assessment, data collection, and evaluation • Provide training to program staff on evaluation activities • Participate in analyzing and translating the data collected • Assist in the dissemination of results to the community [CP Organization] responsibility: • Recruit 10 peer mentors and program outreach coordinator through community outreach • Host training sessions for peer mentors on their roles/responsibilities and mentorship skills • Enroll participants into the program • Provide monthly education workshops on men’s health, physical activity and nutrition for participants • Establish partnership with local gym to offer group fitness class once a week for participants • Track attendance and collect evaluation data for mentors and program participants Financial arrangements: [Academic Institution] will receive funds from [CP Organization] for services provided as a part of this agreement in two installments. For this project period, [Academic Institution] will receive a total of [$ amount]. [Academic Institution] will submit an invoice for the first six months of the project period, and an invoice for the second six months five months later. It will take approximately one month from [CP Organization] receipt of the invoice to process and for the [Academic Institution] to receive funding. Dissemination of publications, presentations, and products: Participation of individuals in particular products will be based on participation in the work supporting the product. papers. Authors will be expected to contribute to papers and be responsive to reviewer concerns in a timely manner. Some papers may have a group of participants acknowledged with a shorter list of authors. Research works developed jointly by [CP Organization] and [Academic Institution] shall be jointly owned, and decisions regarding marketing and distribution shall by jointly made by all contributing partners. Data ownership: At the end of the project, [CP Organization] and [Academic Institution], will retain a copy of the full data file, de‐identified appropriately. Any member involved in data collection, review and/or analysis, must review its participation and role through the lead academic IRB and/or sign a certificate of compliance for the project. Internal Review Board (IRB): It is requested that each participating member complete a certification of training for human subjects research and sign a conflict of disclosure/interest form through the lead academic partner’s Internal Review Board. This MOA may be terminated with or without cause by either party giving written notice.
