GE Healthcare Clarient Diagnostic Services Instructions for Shipping Specimens to Our Laboratory (Aliso Viejo) If you would like to track your specimen(s), please contact customer service at 1 888 443 3311 with FedEx® tracking number or courier information. Materials needed: • Shipping Kit • Test Requisition Form • FedEx or our laboratory provided Clinical/Diagnostic Pack • FedEx Billable Stamp (For specimens being sent via FedEx on Fridays, please check the Saturday delivery section of the FedEx Billable Stamp) • Ice Pack (During the months of May – October, or when needed) How to send: • Place specimen in a shipping kit • During the months of May – October use a frozen ice pack (For blood/bone marrows, make sure the frozen ice pack does not come in direct contact with the specimen) • Include a completed test requisition form • Place the test requisition form in the shipping kit, and then place the shipping kit in a FedEx diagnostic pack • Call customer service with FedEx tracking number or courier information at 1 888 443 3311 Send priority overnight to: Clarient Diagnostic Services 31 Columbia Aliso Viejo, CA 9265 Customer Service and Supply Request Phone 1 888 443 3311 Fax 1 888 443 3345 © 2014 General Electric Company — All rights reserved.
