Farm & Ranch Life The people involved in agriculture differ from the general population in the types and frequencies of the behavioral health problems they typically experience. There are commonalities as well, but the differences are the most important for farmers and their healthcare providers to understand. This and next week’s article examine these matters. Today’s message about chronic mental illnesses (CMIs) and personality disorders adopts the National Institute for Occupational Safety and Health’s definition of farmer, which also includes ranchers, farm laborers, seasonal farm workers, commercial fishers and lumber harvesters as farmers. CMIs include any of the recurrent psychoses, such as schizophrenia. Psychosis is identified by its primary symptoms: delusions, hallucinations, and disordered thinking and speech; the latter may seem nonsensical to other persons but not to the person with the illness. Psychotic illnesses usually are genetically inclined, but may be due to other causes, such as neurological injuries, drug addictions and exposures to toxic chemicals. Infrequently, but occasionally, psychotic behaviors accompany severe bipolar disorder, major depression and anxiety disorders, but generally these disorders do not become so severe as to lead to delusions, hallucinations, and seriously disturbed thinking. Prevalence rates of CMIs in the U.S. A National Institute of Mental Health (NIMH) report indicates that 1.1 percent of Americans develop schizophrenia sometime during their lives. The NIMH has indicated a lifetime prevalence rate of about 3 percent for bipolar disorder and prevalence rates for depression and anxiety disorders in the 11-20 percent range, depending on the study. Schizophrenia tends to run in families. Onset usually occurs in late adolescence or early adulthood. A 2015 collaborative report of more than 100 scientists indicated 108 genetic markers linked to schizophrenia. Bipolar disorder, major depression and anxiety disorders also have genetic markers but their phenotypic expressions vary considerably. Depression and anxiety, in particular, are highly associated with the amount of stress experienced by the persons who develop them. Though this type of fraud is tough to prevent against – beyond taking steps to safeguard your personal information – BBB offers some tips to help consumers recognize when it’s occurred and steps they can take to mitigate the situation from there. According to CNBC, taxrefund fraud is expected to total $21 billion in 2016. This type of fraud often occurs without the victim knowing it, until they file their tax return and discover that a return has already been filed using their Social Security number. Another way people learn they’ve been victimized is when the IRS sends them a letter saying they have identified a suspicious return using their SSN. “Various criminal operations have begun focusing their resources on tax refund fraud, which has become a lucrative illicit enterprise,” said Dana Badgerow, president and CEO of BBB of Minnesota and North Dakota. “The IRS is taking increased steps to prevent this type of fraud, but this is an issue that will remain front and center going forward.” BBB seeks to make people aware that this is a growing problem and one they need to be prepared for. Some ways to decrease your risk of becoming a victim of tax-related identity theft include: safeguarding your Social Security number and other sensitive personal information, and filing your tax return as promptly as possible. This year, the IRS began accepting tax returns on January 19. The filing deadline to submit 2015 tax returns is Monday, April 18, 2016, rather than the traditional April 15 date. The IRS offers some additional identity protection tips for taxpayers: Always use security soft- ware with firewall and antivirus protections. Make sure the security software is always turned on and will automatically update. Encrypt sensitive files such as tax records you store on your computer. Use strong passwords. Learn to recognize and avoid phishing emails, threatening calls and texts from thieves posing as legitimate organizations such as your bank, credit card provider or even the IRS. Do not click on links or download attachments from unknown or suspicious emails. Guard your personal data. Don’t routinely carry your Social Security card, and make sure your tax records are secure. Treat your personal information like you do your cash; don’t leave it lying around. Be alert to possible taxrelated identity theft if you are contacted by the IRS or your tax professional/provider about: • More than one tax return being filed using your SSN. • Owing additional tax; your refund being offset; or if you’ve had collection actions taken against you for a year you did not file a tax return. • IRS records indicate you received wages or other income from an employer for whom you did not work. If your SSN has been compromised or you know – or suspect – that you are a victim of tax-related identity theft, the IRS recommends that you: Respond immediately to any IRS notice; call the number provided or, if instructed to do so, visit IDVerify.irs.gov. Complete IRS Form 14039, Identity Theft Affidavit, if your e-filed return is rejected because of a duplicate filing under your SSN or if you are instructed to do so. Use a fillable form at IRS.gov, print, then attach the form to your return and mail according to instructions. Continue to pay your taxes and file your tax return, even if you must do so by paper. Most farmers with CMIs usually cannot hold down permanent and adequate-paying jobs to care for themselves and families. Usually they have to depend on financial resources, like Supplemental Security Income, and on other people for assistance with daily living. Persons with personality disorders also seldom are able to farm successfully. Let’s define personality and personality disorder. Drawing on the research of Dr. Walter Mischel and other psychology experts, personality is a behavior pattern that is consistent across various situations and over time. For example, some people tend to be gregarious and seemingly “talk all the time” wherever they are and with whomever they interact. Personality disorders are enduring behavior patterns that seriously and regularly disrupt personal, social and occupational functioning. Like personality, personality disorders often are formed during childhood, and usually in response to adverse experiences like abuse, rather than the posi- tive childhoods most behaviorally healthy children experience. The latest Diagnostic and Statistical Manual of Mental Disorders (DSM 5), published by the American Psychiatric Association in 2013, lists ten personality disorders: Paranoid, Schizoid, Schizotypal, Antisocial, Borderline, Histrionic, Narcissistic, Avoidant, Dependent and Obsessive-Compulsive. The prevalence rates of personality disorders using the DMS 5 have not been fully established. The NIMH estimated the prevalence rates of personality disorders using its predecessor, the DSM IV, at 9.1 percent of the general population for any type of personality disorder. About half the persons with personality disorders also had one or more co-occurring mental illnesses, such as--but not limited to-- substance abuse, anxiety and depression. My colleagues and I also used the DSM IV when we followed the diagnoses of 7,238 farm people in the Upper Midwest. We found a 1.5 percent prevalence rate for any type of personality disorder. Farm people and their behavioral health caregivers need to be aware of the differences in CMIs and personality disorders for farm people and the general population. Next week’s article will be devoted to the most common behavioral health issues of farm people, which are stress-related and usually manageable. You also need to determine if your message does what you want. Run some test messages on a variety of platforms to see how people respond before spending your entire marketing budget all at one time. Understand that each marketing method may require a somewhat different way of presentation. A colleague ran a Facebook ad test and found people respond differently whether they were viewing the message on a desktop computer or on a mobile device. Your message must connect with your intended audience. It must encourage people to read more, embed your company and products/services into their mind, or elicit action. Again, what is your goal? The fourth issue for many small-business owners is this: What does your marketing do for your bottom line now or in the foreseeable future? In part, this means determining the most cost-effective way to reach your goal or goals. Numbers are great, but aware, engaged customers taking action are what you want. Finding an idea that will make money and is sustainable is hard. Ideas for businesses are found in many different ways. The idea may an old family recipe or a new recipe you create. Ideas also come from imagination and observation, and may pop up as you solve a problem. Or maybe your idea develops as you see something and feel you can do it better. Building on trends is another often-used method for idea generation. The idea of developing a business or a product or service around a trend has merit. Often the news media help with the marketing because they keep the idea in front of people with a continuous stream of stories. Building on a trend also helps bring the first customers, those who want to be on the “cutting edge,” in the door. Yet when building on a trend, money, the competition already is established and has substantial inventory it wants to sell. What is important to remember is that trends will bring competition. If you see the idea, the chances are that others also are ready to jump. Businesses involved in building around trends tend to find themselves with more competition than others. Trends can be great opportunities for a business or a product or service. Look around and see what’s trending. Then consider how you can capitalize on it. In 1996, the Animal Drug Availability Act created a new category of products called veteri- called a veterinary feed directive, or VFD, providing for such use,” says Gerald Stokka, North Dakota State University Extension Service veterinarian and livestock stewardship specialist. The purpose of this rule is to limit the use of medically impor- nary feed directive (VFD) drugs. These are livestock antibacterial drugs (antibiotics) intended for use in or on animal feed. “In other words, for animal feed containing a VFD drug to be used in animals, a licensed veterinarian first must issue an order, tant antibiotics in livestock to those antibiotics considered necessary for assuring animal health. These drugs are for treatment, control and prevention. They are not for food production purposes, such as to enhance growth or improve feed efficiency. Medically important antibiotics used in livestock include aminoglycosides (Neomycin), penicillin, lincosamides (Lincomycin), macrolides (Tylosin, Tilmicosin), sulfas and tetracyclines (chlortetracycline [Aureomycin], oxytetracycline). The U.S. Food and Drug Administration is implementing the VFD rule, which will go into effect on Jan 1, 2017. “In practical terms, the VFD is a permission slip granted by your licensed veterinarian who is familiar with you and your operation; provides health, management and treatment recommendations; and knows and understands your type of operation and management capability,” Stokka says. “In addition, your veterinarian is available for diagnosis and/or follow-up in cases where response to the drug does not meet expectations or adverse reactions occur.” This type of professional relationship is known as a valid veterinary client patient/producer relationship. In some cases, this relationship can be with a number of veterinarians in the same practice, or it may be with veterinarians from different independent practices. In every case, the VFD must be granted prior to making any purchases of VFD drugs. The VFD order cannot be granted over the phone to the producer or the seller of the products. However, it can be transmitted electronically or via paper copy. The VFD will contain specific information: * Veterinarian’s name and address * Producer’s name and address * Approximate number of animals to be fed the medicated feed * Name of the livestock drug, the reason for the drug’s use, the dose and withdrawal time Feed additive medications have a number of approved claims and uses; however, the use of VFD drugs does not permit any extra-label drug use. This means a product that has label claims for specific conditions, such as the prevention of respiratory disease in beef cattle, cannot be used to prevent foot rot in pasture cattle unless an approved claim for this use exists. None exists at this time. “In addition, VFD drugs have a number of uses that may not seem obvious to producers and veterinarians,” Stokka says. “For example, some brands of calf milk replacer may contain a preventive level of a VFD drug such as chlortetracycline to prevent enteritis or calf scours. Before you can purchase this product, you will need a VFD order. Work with your veterinarian to assist in purchasing the appropriate products.” The main impetus for these changes is the need to address the issue of humans’ increasing resistance to antibiotics used in livestock feed that are considered medically important in human medicine. These changes also provide more accountability in the use of feed additive antibiotics. “While this connection is weak, we must all recognize that whenever antibiotics are used, whether in livestock, companion animals or people, resistance by susceptible bacterial populations is likely to occur,” Stokka says. “Interestingly, reversion to a susceptible or nonresistant state seems to take place once antibiotic use ceases.
