PERMISSION SLIP
Dear Teacher:
To facilitate your Field Trip Factory Field Trip, we have prepared the following
permission slip. Please reproduce it, pass it out to students planning to attend
the Field Trip, and ask their parents to sign and return it two weeks prior to
your excursion. When you call to confirm your tour, please let the tour leader
know if there are any students in your group who should not be given food
samples or should not be included in potential photographs or media
coverage.

we are planning to take an in-store nutrition tour of our
local Stop & Shop store.
During the tour, your child will sample some healthy foods. If your child has any food allergies, please list
them below. If your child has any physical limitations, please list them below. Your child may be
photographed for store promotion and/or local media coverage. Your signature below indicates permission
for your child to be included in the photos, which may be displayed in the store, used for promotion, and/or
shown in media coverage.

I hereby hold Stop & Shop harmless for any accidental injury that my child may suffer while on the premises of
