WELCOME LETTER
Welcome to the 66th Congress of Delegates and installation dinner of the
New York State Academy of Family Physicians!
Since its inception in 1948 the Academy has been a leader in advancing the
specialty of Family Medicine, advocating for members and patients, producing educational programs to enhance the skills and knowledge of family
physicians, and cultivating interest in Family Medicine among medical students.
The Congress is our annual business and policy meeting. Members discuss
important issues regarding public policy and Academy programs and make
decisions that our board of directors and staff implement throughout the
year.
Among the important decisions delegates to the Congress make are the election of officers and directors. Individuals elected at today’s session of the
Congress will be installed tonight by Dr. Michael Munger of Kansas who is
a director of the American Academy of Family Physicians.
After installation of our officers and directors, we will honor our incoming
and outgoing presidents – Drs. Mark Josefski and Ray Ebarb, respectively.
We will hear from Dr. Josefski regarding his plans for the next year and we
will pay tribute to Dr. Ebarb for his year of service as president.
We are grateful to have you with us tonight and hope you enjoy this
evening’s program.

Welcome
I’d like to welcome everyone to Troy for the 2014 Banquet and Installation of
officers of the New York State Academy of Family Physicians. We are a
broad and diverse coalition of individuals, who come together to debate, discuss and decide on our collective approach to health care issues that affect
the people of New York. We are tasked with the responsibility to represent
more than 4000 Family Physicians in the state, sharing our wisdom, ideas
and policies with a wide spectrum of interested parties.
Our academy has become a valued source of information and leadership for
basic and important decisions being made in Albany. Our patient-centered
advocacy efforts are becoming more sophisticated and respected, as we improve visibility, effort and savvy. This has been a joint effort of the Advocacy
and Public Health Commissions, involving the “collective wisdom of a very
smart group of physicians” (apologies to Phil Kaplan for borrowing his
phrase). The recent decision to more fully engage the services of Reid,
McNally & Savage, allowing Marcy Savage and Vito Grasso to coordinate these
efforts, has been a very positive move. As we continue to move forward, we
will face the challenges of an ever evolving (and never simplifying) healthcare
delivery system and advocate for the changes that our specialty offers.
Tonight we celebrate…thank you for coming to share in our progress, which
is built upon the efforts of a long tradition of leadership. I am honored to be
a part of that legacy and I challenge everyone here to be a part of our future
successes!

!
Welcome to our Annual Inauguration Dinner. It certainly is hard to believe an entire year
has passed since I was inaugurated as president a short twelve months ago. It has been
a year filled with successes and, shall we say, ongoing challenges. But there is a bright
side to the ongoing challenges. Even on those projects where we would have liked the
outcome to be different, we were still out in the front of the crowd and recognized as
leaders in patient advocacy. The political leaders heard our name more often and our
members got to know a little more about us. Hopefully, with our increased efforts of
transparency, our members are a little more educated as to the efforts of their academy.
This year our academy delved into areas we were never involved with before. We pushed
the envelope a little more. We were at the State Capital, the U.S. Capital, in politician’s
offices, quoted in newspapers and interviewed on radio. We took a stand and were out
in front of the crowd on controversial public healthcare issues.
We will push hard again this year. Under the experienced leadership of Mark Josefski,
the Academy will stand on the shoulders of this past year’s accomplishments and reach
new heights. It is an exciting time for family physicians in the state and it will be an exciting time in the Academy. It will not be easy for there is still a lot of work to be done.
I want to wish Mark my heartfelt congratulations on his presidency and best of luck with
the challenges that lay ahead.

2014 FAMILY MEDICINE EDUCATOR OF THE YEAR
LINDA W. PRINE, MD
Linda Prine is senior faculty at the Beth Israel Residency Program in Urban Family Practice and at the teaching health center Harlem Family
Medicine Residency. She is the Women’s Health Director for the Institute for Family Health and the Medical Director of the Reproductive
Health Access Project.
She graduated from Cornell University Medical College in 1987 and completed her residency training at Montefiore Medical Center’s Family
Practice Residency Program in Social Medicine in 1990. She is board certified in family medicine. Dr. Prine has presented at grand rounds
around the country and at many academic meetings: at the Society of
Teachers of Family Medicine, American Public Health Association, Association of Reproductive Health Care Professionals and at American Academy Family Physician on topics in Women’s Reproductive Health. She
has authored many articles and research papers in this field.
In 2012 she was awarded the Rashbaum Award from Physicians for Reproductive Health for mentoring, teaching, and inspiring students and
physicians-in-training.

Excerpts from nominators…..
One day, I asked Dr. Prine about her thoughts on her career, her legacy, and how she views her impact on women's healthcare. I remember her look of surprise, and she told she me would have to think about it. Later that
night, I received an email. She said she never thought about it in that way- she just went to where she felt she
was needed, and her work is something she felt she had to do..."I didn't think about what it would (do) for or
against my career, I just plowed ahead."
Recognizing Dr. Prine as the NYSAFP Family Practice Educator of the Year has so much meaning for me as her student because the same passion that has driven her career as a clinician has also propelled her as an outstanding
mentor. Dr. Prine is the physician we as residents hope to become: a role model who excels in clinical care, with
poignant teaching abilities, and a relentless energy for advocacy and mentoring!

Beth Israel Residency in Urban Family Practice, Phillips Family Practice- The Institute for Urban Family Health
--------Although I practice in New York now, I was influenced by Dr. Prine’s mentorship and teaching as a new residency
faculty member in Oregon and California. As someone who also has an interest in women’s health, Dr. Prince was
always available to answer questions and provide education. In fact, when my job in California ended, Dr. Prine
was a major influence on my decision to return to New York, where I had attended college and medical school.
She is extremely generous with her time and knowledge, and I was eager to follow in her footsteps as a residency
educator.
Once I returned to New York, Dr. Prine was responsible for getting me involved with the NYSAFP. As a former
chair of the Public Health Commission and Board Member, she suggested that I come to the Congress of Delegates,
introduced me to other delegates, and then suggested that I join the Public Health Commission, of which I am
now chair. Dr. Prine’s advocacy for the NYSAFP has a ripple effect; by involving me, she has encouraged me to

recruit medical students and residents to come to meetings and serve on commissions. As chair of our New York
County chapter, she is the major reason that our chapter is so active and has involved more junior faculty, residents and students. She is also an active educator within the NYSAFP and has given many conference talks on women’s health topics. Her fellowship is actively producing more family physicians with strong women’s health, reproductive health, and advocacy training each year.
Heather Paladine, MD, Program Director
NY Presbyterian/Columbia family Medicine Residency Program
--------Linda has made a mark in the field of family medicine and women's health in research, activism, patient centered
care, and most importantly, in education. She has trained a cadre of doctors from whom she expects, and elicits,
outstanding levels of competence. She leads by example, dedicating long hours and precious time to her patients,
involving residents in the research that drives her, mentoring medical students in free clinics and reproductive
health electives, running a fellowship in reproductive health, and mentoring doctors from all over the country in
health advocacy. Through the RHAP project, which she founded, she inspires AmeriCorps volunteers, researchers,
and clinicians throughout the country to provide patients with the best information for them to make an informed
decision about their health. She constantly examines the nuances that her own language, thoughts, emotions and
behavior may convey to a patient, thus encouraging her mentees to do the same in order to provide patients with
the most non- judgmental, patient centered care possible. She makes her patients feel as though there is no one
else in the world when she is in the room with them. She listens to their thoughts, feelings, and makes them feel
safe. She evokes the same feelings in her students, whether they are male, female, medical students, medical office staff, residents, fellows, or young attendings. Her passion for her work shows in everything she does and pushes
others to perform at the best of their ability. She finds students who are lagging behind and helps them fill in the
gaps and excel, and equally finds students who already excel and nudges them to do more, smiling like a proud
mother when her students have accomplished more than they set out to do.
Her advocacy in the NYSAFP also provides an amazing example to countless mentees as she fights for what she believes is right, from reproductive health rights, to equal salaries for women doctors. She encourages young doctors
from all over the country to engage in advocacy and activism of their own to make their voices, and their patients'
voices, be heard. Linda gains nothing by all of this tireless work, except to know that people are working hard all
across the country to advocate and to provide the best possible care for their patients and for themselves.
Linda cannot help but touch every life with which she comes into contact. Her concern for us as people, as professionals, and now as colleagues shows in everything she does. I can only hope to continue a small piece of her work
in my daily life to carry on her message, her energy and her enthusiasm.
Rachna Kaul
Faculty, Mid-Hudson Family Practice Residency
--------Linda’s defining characteristic as a master educator is how generously, instinctively and consistently she trains
those around her to be educators themselves. She takes the “see one, do one, teach one” concept to another level
and encourages everyone around her to teach each other, constantly. Right after she shows a medical student how
to do a pelvic exam, she will seek out the other medical student in the office and ask the first student to teach her
peer. She arranges clinical time and implements scheduling structures to specifically foster peer education, with
the more experienced residents partnered with interns, who are partnered with medical students. She unfailingly
invites her residents to participate in her myriad research projects and presentations, posters and papers. Furthermore, she fiercely curates their participation to be meaningful; her students are not doing endless chart reviews or
data entry, but learning how to write research proposals and how to structure a clinically relevant, testable research question.
Linda’s unflagging passion and drive for providing evidence-based, patient-centered care are what defines her as a
caring and knowledgeable physician; her generosity in sharing her knowledge and skills as both a clinician and
teacher is what defines her as a master educator.

2014 FAMILY DOCTOR OF THE YEAR
ROBERT P. NIELSON, MD
Dr. Nielson is a board certified primary care sports medicine specialist. An Ohio native, he received his medical degree from The Ohio
State College of Medicine in 1980 and has lived in Saratoga Springs
since 1991. He is a Fellow of the Academy of Family Practice and a
member of the American College of Ringside Physicians.
Dr. Nielson has served as a high school football team physician for 30
years and is school district physician currently in three Saratoga and
Washington County school districts. He maintains a primary care practice in Saratoga as well. He is a Certified ImPACT Consultant for management of concussions and performs computerized neuropsychological for school districts and in his private office.
Dr. Nielson served as a volunteer United States Olympic Center Physician in 1996 and now travels internationally with the United States Boxing Team. He has covered many
winter sports for the USOC including short track speed skating, hockey and luge. In the past he has
covered hockey with the Albany River Rats (AHL) and Skidmore College. He has been a member of the
New York State Athletic Commission responsible for professional boxing coverage.
He is current President of the American Board of Ringside Medicine and serves on the American College of Medicine's special task force on ringside physician certification. He is co-chair of the Saratoga
Hospital Medical Department and a member of the Executive Committee of Saratoga Hospital. He has
raced bicycles competitively in the past and still is an active participant in many sports including ice
hockey.

Excerpts from nominators…..
Dr. Robert Nielson, whom we lovingly refer to as "Dr. Bob", has been our family physician since 2000. We
have three children ages, 20, 17, 11 and have turned to Dr. Nielson to guide us through numerous childhood
illnesses and serious medical issues through the years.
Dr. Nielson first examined our son when he was only a week old. He was so excited for our new addition. Eighteen months later, Dr. Nielson was who we turned to when our son stopped speaking, started
spinning in circles and no longer made eye contact. Dr. Nielson had the sad task of telling us that our sweet
son, our baby, was Autistic. Those were dark days indeed but ultimately life propelled us forward and our
son recovered and was upgraded to Aspergers. I do believe our son's recovery is in part due to Dr. Nielson's
expediency in making the appropriate referrals for sources of support that we needed so many times along
the way.
A few years later, my husband began experiencing some very serious medical problems. We made numerous
visits to Dr. Nielson; multiple tests; referrals; and hospitalizations. Over one year later, Dr. Nielson made
the call to my husband personally to confirm what he had suspected all along; it was Multiple Sclerosis. It
was two days before my husband's 46th birthday back in 2007.

My family has been consoled and supported by Dr. Nielson multiple times over the course of this difficult journey. Together with his office staff, he even attended a local fundraiser that was held for my husband.
After my husband's diagnosis, life took us down another treacherous path when our oldest daughter became
seriously ill at the young age of 13 and was hospitalized four times in five months. Dr. Nielson was the first
to evaluate and treat her. I am happy to report that she will begin her senior year of college this August. Dr.
Nielson showed great care and concern for our daughter during that difficult time. We trusted him and knew
just how much he cared, and still does.
A few years later, our younger daughter developed a serious illness as a result of bullying at school coupled
with her anxiety over her father's battle with M.S. Again, it was Dr. Nielson who got help for our daughter immediately and still follows her closely.
One Christmas I received a call from Dr. Nielson at home on a Saturday telling me that I needed to be available for a call from a radio station the next day. He did not tell me "why" only that they had a "surprise" for
our family. I got the call while the program was on the air and was told that Dr. Nielson had written about
our family and our difficulties. As a result, our family was given a $500 shopping spree courtesy of the radio
station to Toys R Us. In addition, he "secretly" put several gift cards in our mailbox to allow us a wonderful
Christmas that year.
While life is still challenging for us here at home as we battle M.S., we are so grateful that we have found
such a wonderful family physician in Dr. Robert Nielson, whom we also consider a family friend. He has gone
above and beyond the realm and responsibilities of a physician. He has shown great care, immense concern
and deep compassion for our entire family. Words just cannot express how grateful we are to have "Dr. Bob"
as our family doctor. "Thank you" is simply inadequate to show our appreciation.
Mrs. Amy Brimmer, Gansevoort, NY
--------Dr. Nielson has a great reputation among both doctors and patients, and it is very much deserved. He is
knowledgeable, informed, a great listener, always gives me all the time I need to ask questions and discuss my
condition, and is connected to other physicians such that he can usually make a great referral when other doctors (in my experience) cannot. I have severe pain from an old accident, and have seen so many doctors,
many excellent ones, over the years. Dr. Nielson has helped me through multiple trials, physical, mental and
emotional. He has helped me find therapies that have enabled me to manage my pain without having to take
excessive NSAIDs and opioids. He was able to refer me to a center of excellence for my back, found me a
"personal trainer" to help me with my comprehensive home physical therapy plan, and recently located a P.T.
who understood myofascial pain and trigger points, which has been very important as I have multiple, constant and severe spasms, and other massage therapy only increases my pain, whereas myofascial release has
been effective in helping manage my pain. Dr. Nielson even finds me coupons and special deals on therapy
equipment that he knows I could use! He goes above and beyond what any reasonable person could expect
from their physician. Dr. Nielson never stops looking for ways to improve my care. I am always moving forward. For the first time in my 55 years, I have confidence that my doc is managing my total healthcare in a
comprehensive fashion. Previously, I have always largely had to manage my own healthcare, even though I
saw my other doc regularly, probably because I have a lot of health problems.
Please consider Dr. Nielson. If you look into him, I'm confident you'll find a lot of very impressed doctors and
patients who know and respect him very much. He is a humble man with a quiet demeanor, but his actions as
a physician deserve recognition.

The NYSAFP mourns the loss of past-president George Hart, MD who died at home on
Tuesday, May 13, 2014, in his 98th year.
The NYSAFP had the pleasure of featuring Dr. Hart with a reception in his honor at
Winter Weekend in 2013 with much of his family, friends, patients and the Lake
Placid Community in attendance.
The NYSAFP and the NYSAFP Foundation proudly honored George Hart, MD, for his
years of service to patients and his exemplary leadership within the Academy of
Family Physicians. The NYSAFP Foundation established a fund in Dr. Hart’s name to
support our work with medical students and residents in Family Medicine and to develop the leadership
and advocacy skills of family physicians in the model which Dr. Hart and so many of his colleagues in the
Academy have done over the years.
Dr. Hart served as president of the Academy in 1970-71 and was an active and respected leader in the formation and growth of the Academy and the specialty of Family Medicine. His distinguished career in medicine and his extraordinary achievements as a community leader in Lake Placid epitomize the traditional
role of the physician and established Dr. Hart as a role model for young physicians.
Dr. Hart’s CV is replete with notable achievements and exemplary contributions to medicine, family and
community service. His career is an example for Family Physicians everywhere.
Dr. Hart was the son of Edward and Sarah Gremple Hart. Edward Hart was an orphan in
London, England, and arrived in the New World on the S. S. Numidian from Liverpool at
Montreal in 1895 at the age of 14. Sarah Gremple came to Saranac Lake from New York
City to cure for tuberculosis. Edward and Sarah married, and Dr. Hart was born July
22, 1916, in Lake Placid. He graduated from Lake Placid High School in 1934. He attended McGill University in Montreal, and he received the degrees of Bachelor of Science, Doctor of Medicine and Master of Surgery. McGill is one of the few universities
that awards the two degrees of Doctor of Medicine and Master of Surgery to the graduates of its medical school. At McGill, he was a member of the Osler Society and the Nu
Sigma Nu fraternity. He interned at St. Luke's Hospital in Cleveland, Ohio, and after
World War II, he took training in obstetrics in Providence, Rhode Island.
In 1942, Dr. Hart joined the U. S. Army Medical Corps as a flight surgeon and was
attached to the 46th Fighter Squadron for over three years, all service being in the
Pacific area of combat. His last post was the bivouac area on Iwo Jima where many
casualties ensued, and Dr. Hart was the only medical officer on duty following a
Banzai attack. For his actions, he was awarded the Silver Star for uncommon valor.
After World War II, Dr. Hart returned to Lake Placid, took more medical training and
undertook the general practice of medicine. During his years of practice, he was a
staff member at Placid Memorial Hospital, where he served several terms as President of the Medical Staff, and Saranac Lake General Hospital. He served as Presi-

dent of the New York State Academy of Family Practice in 1970-71. He was a member of the American
Medical Association and the New York State Medical Society. During his years of practice, he delivered
generations of babies born in Lake Placid, was the doctor on call for many of the hotels, made house calls
as a major part of his practice, had office hours and was available to the community for their medical
needs 24 hours a day. He covered school, community, and international sporting events. He was the doctor for North Country School. In 1975, he was appointed Medical Director of Uihlein Mercy Center and
served until 1987, when he retired from the practice of medicine.
Dr. Hart was an active member of the community. He was Medical Director of the Lake Placid Club, and
Chairman of Medical Services for the 1980 Olympic Winter Games. He served on the Lake Plaid Board of
Education from 1955-1970, and was President of the Board for nine years. He was a member of the Lake
Placid Hall of Fame, the Kiwanis Club of Lake Placid, and the Lake Placid Sports Council.
Dr. Hart was a life-long member of St. Eustace Episcopal Church. He served on the Vestry and was Senior
Warden. He sat the board of St. Francis Academy (Camelot) for many years. He was a member of the Lake
Placid Lodge of Freemasons, #834, and of the Scottish Rite, the Shriners. He was a member of the Lake
Placid Institute and the Lake Placid Historical Society.
Dr. Hart was an accomplished photographer, gardener and musician, playing
the French horn and the violin. He compiled a genealogy of the History of
Aberdeen Angus breed, and at one time, he had sixty head of Angus cattle
on his farm, Placidmere, in Averyville. He was widely read, with a special
interest in history and music. He often said his favorite things in life were
medicine, music, cows, and bridge.
Dr. Hart’s devotion to family is evidenced in his 67+ years of marriage to Ruth, their four children, nine
grandchildren and five great grandchildren.
We honor Dr. Hart for all of his many contributions and for the high personal and professional standards he
remained dedicated to. His innumerable contributions to the establishment and growth of the specialty of
Family Medicine and to the community of Lake Placid are and will remain worthy goals for future generations of physicians and community residents to emulate.
We will miss you, Dr. Hart.

NYSAFP COMMISSIONS
Advocacy
This Commission monitors government actions and the potential impact of legislation and
proposed regulations on Family Medicine and Family Physicians. Commission members discuss legislation and make recommendations for action to the board of directors. The Commission plans the annual lobby day and works with the Academy’s government relations
counsel – Weingarten, Reid & McNally.
Education
This Commission develops educational programs for Academy members. It oversees production of the annual Winter Weekend & Scientific Assembly and the Regional Family Medicine
Conference. It also manages the Advanced Life Support in Obstetrics (ALSO) course and reviews proposals for educational programs and collaborations. This Commission serves as the
selection committee for student and resident awards, research externships and the Family
Medicine Educator of the year Award.
Leadership
This Commission supports leadership training for members and serves as the nominations
committee for selection of candidates for Academy offices. It also develops and presents
leadership training for residents at the Congress of Delegates.
Membership
This Commission is responsible for recruiting and retaining members and making recommendations for member benefits. It also coordinates panning for the Congress of Delegates and
oversees the Academy website.
Operations
This Commission is responsible for all Academy operations including preparation of the annual budget, financial management, personnel, headquarters operation, leases, contracts
and bylaws.
Public Health
This Commission is responsible for education of the public regarding health, policy development to promote public health initiatives and increasing access to care for special needs
populations such as the aged, children and those in underserved areas.
