“Celebrating Our Past . . . . . . Planning for the Future” Many changes have occurred in the health care delivery systems in our nation and our area since the Omega House first opened in 2005. One thing that has remained constant is our organization’s commitment to serving our community by providing compassionate end of life care in a home-like setting to those in need. We have stayed true to our mission and have provided care for over 370 Upper Peninsula residents. We employ a unique model of care that comforts our residents by utilizing hospice staff working in concert with our aides and volunteers and family and friends to provide individualized person centered care. To proactively plan for the future, our board and staff have been actively engaged in strategic planning sessions to assess our strengths as well as our shortcomings and to identify how to meet the growing end of life care needs in our community. In this process we have engaged in dialogs with regional and national experts in end of life care, as well as financial planning. I am pleased to share that our mission has not changed but our vision has expanded beyond the walls of the house, and I believe we are charting a bright course for the future. I invite all of our supporters to join with us in celebrating the Omega House ten year anniversary at the Finnish American Heritage Center in Hancock this September. We will be sharing details of our new strategic plan and the new initiatives that we feel can help to transform our community into a role model for We hope that you feel inspired to continue to support this work and share in our joy of service. Dr. McCullough is a native of Dollar Bay and has won national recognition for his work in elder and end of life care. Our Vision The Next Best Thing to Being Home Omega House 2014 Annual Report Page 1 Rayna Racine — Dee Vancato To the Staff and Volunteers, My family would like to extend our deep appreciation for the kindnesses and caring nature you have all provided to our Aunt Rayna Racine. She was in our care for eight years after she could no longer live alone in her home. We thought as a family we could provide all the comforts of home and meet her needs until around-the-clock care became too much for us. Our network of family members were more than willing to be available to carry the load of care. However, it became so much more than we could handle. Then, when Aunt Rayna had pain enter her life, we realized that our best intentions were never going to be adequate to give her the type of care she needed. It was with sadness that my family and I decided it was time to allow another group of people to take over her care – she had been surrounded by our loving care for eight years. And although we promised her we would never place her in a nursing home, that day in the hospital changed those promises. Rayna Racine My son (who is a social worker) and daughter (a registered nurse) were with me and suggested Omega House. I was not sure Auntie would meet the criteria to become a resident – but I was so wrong. We are grateful to those who took the helm that afternoon and helped the process along: my children; Carolyn Norman, hospital social worker; an ER physician on staff that day; the ambulance transport crew; and finally all at Omega House for helping us with an easy transition. You were an answer to our prayers. We reached Omega House that evening, and found our aunt settled in, content, and surrounded by loving staff, who made sure our own needs were met, as well – so comforting after an emotional day. "Supporting Omega House over the past decade has been a pleasure with over 370 community members being served during this time. Volunteering at the house has kept me close to hospice care in my retirement and for that I am grateful." Your care is the blessing residents are looking for during the last days of their lives. With sincere respect to each of you, and may you have peace and comfort in your own lives and know that your care gives you the same peace and comfort. Wanda Kolb Board Member Written by Dee Vancato, niece Omega House 2014 Annual Report Loving Hands — A Grand-daughter’s Reflections I lived in Scottsdale, Arizona while my Grandma Irene lived in Lake Linden, Michigan. Last year on June 21st, she celebrated her 95th birthday. My husband and I knew that we needed to take our three year old Luke and five week old Ryan to celebrate her birthday. It’s not all that common for your kids to have greatgrandparents, let alone, that they are celebrating their 95th birthday. I knew we had to be there. And now I am so thankful that we made the trip and have many wonderful memories. I had grown to think my grandmother was going to be around for ages and that she would be a part of my children’s lives while they grew up. I loved bringing my boys “Up North” to visit her and wanted them to experience some of what I did on my annual summer visits to see my grandma’s beloved Upper Peninsula of Michigan—the place where she grew up and never wanted to leave. wasn’t to be. When many of my cousins and friends of my family were all coming to Omega House in her final days, I didn’t think I could make the journey to say goodbye. My husband has a traveling job that is scheduled out months in advance. I focused on the happy memories and family reassured me of my grandma’s plans and this wonderful home called Omega House they chose for her to spend her final days. After praying for days about what to do, I woke up one morning and knew where I needed to be. I needed to make that journey to see her—immediately. Arriving at Omega House, I found myself surrounded by peace. Walking into the home, I was greeted with the smell of fresh baked cookies, friendly smiles welcoming me and introducing me to the loving hands that were caring for my grandmother. The home is beautiful and not only provides the I was shocked when I heard she was not only very sick, but that her health was failing. Grandma was a huge part of my life and I grew up very close to her. The thought of her not living any longer was devastating. If you have a strong woman in your life, then you know a little of my grandma’s spirit. She was STRONG. Even though her prognosis meant her days were numbered, I still held onto the hope that, perhaps, a miracle would happen and she would wake up well. She had survived so much in her life, even when things didn’t look in her favor, and I held onto the hope that it might be that way this time. Here, we gathered as a family, like you would in someone’s home. Her room was right across the hall from a meditation room where I think we all used to pray, collect our thoughts and process what we were going through. I sat at my grandma’s bedside, holding her hands, finding peace in her strength. Here, this woman who had loved me through the years knew that her life was ending and was now gracefully welcoming her journey to her Maker. Facing death is never easy. Whether someone dies instantly or they have weeks, months or even years, the family and loved ones left behind are still grieving and hurting with the loss. Having the support of family and friends around you is essential. We are a people who thrive in community and we need to know we are not alone. At Omega House, we each had time to visit with my grandma in the loving support as a group and also in the personal, intimate moments, one on one. Omega House provided this for my family and supported us as we prepared to say goodbye. We were able to gather and visit with one another, to spend hours laughing and telling stories with my grandmother, to gather our thoughts in stillness and peace. We were able to celebrate the life of this STRONG woman and find peace in our hearts as we said our goodbyes…until we gather again in the peaceful hands of God. Best wishes, Renee Genetti ■ Page 3 Remembering Reverend Christine Fryxell “For the Christian, God is the Alpha and Omega and a constant companion throughout life. Christians believe that Jesus Christ is with them always. A feeling of contentment springs from trusting in God’s constant presence. The end of life can therefore be seen as a rainbow bridge spanning the gulf between two worlds rather than a dark door to be feared.” Reverend Christine Fryxell Bohnsack, known to many as “Tina,” joined the Omega House board of directors in the winter of 2005 just months before the house opened. She served faithfully with a deep sense of commitment to the Omega House mission. During her ten years of service she endeared herself to many with her wonderful sense of humor and her dedication. She served as board secretary, vice president, board representative from Baraga County and chair of the We Honor Veterans committee. finished, she rolled up her sleeves, went to work, and got the job done." She served for three years as vice president during Mike Abbott’s tenure as president. Mike remembers, “Christine was always ready and willing to do anything that was needed. From representing the organization and board of directors at events, to the hard work of committees to helping at our golf outing, she did it all with grace and a smile. At the same time, she was very effective at making sure issues were addressed. If I forgot or ignored something she thought was important, I would get a phone call from her, gently reminding me I needed to take action. Invariably, she was right and I was better in my role as president because of her.” Wanda Kolb remembers: “Christine was a wonderful pastor to my mother, visiting her frequently at home when she was sick and dying. My mother looked forward to her visits, which were a great comfort to her. She provided a wonderful opportunity for my entire family to meet after her death to talk about our mother’s life, sharing memories, laughter and tears. The funeral she officiated at was so personal, loving and perfect in every way. She was truly a great pastor.” Christine worked with Ray Weglarz on the We Honor Veterans program. She knew Omega House was a wonderful resource to help care for dying veterans and loved to share that news. Ray recalls working with Christine as some of his fondest memories related to Omega House. “We worked together on many projects In the late 1980s, Christine became a member of the but mostly things related to veterans. During the past year, Omega House has taken a lead role promoting advance care planning (ACP), in our community. It is a “gift” you can give your family. It helps ensure your wishes for health care and treatment are known by your doctor and family, and the health care you receive is consistent with your wishes. We are grateful to the Keweenaw Community Foundation, whose grant for $10,320 last fall allowed us to fund the advance care planning initiative at Omega House. When I learned about the Omega House involvement in advance care planning, my immediate reaction was: “how can I become involved in this program?” Last month, I was asked to attend an ACP training session in Marquette at the offices of the Upper Peninsula Health Plan, where I became a certified ACP and MI-POST facilitator. Moving forward I will take over the role at Omega House that has previously been led by Shelby Hamar. There are two key parts to the ACP process: 1. Discussing and documenting your health care wishes before a time of crisis; 2. Naming an advocate or “health care agent” who can speak for you if you are not able to speak for yourself. For the past year Omega House volunteers, Shelby Hamar and Kathy Abbott, have been busy with ACP work; talking to the community – small groups and private citizens – introducing them to ACP and to Five Wishes, an easy-to-fill-out ACP document that addresses your wishes for physical, emotional and spiritual care near the end of life and your wishes for medical care. It is hard on your family when end of life decisions need to be made, and you haven’t discussed them. Our Omega House volunteer and ACP co-facilitator, Kathy Page 6 “End of life decisions are not what we like to think about in our fifties. During 2014, two of my brothers died within 3 ½ months of each other. When my 53-year-old brother dropped of an apparent heart attack in July, I tried to help my mother with final arrangements. But we didn’t know for sure if our decisions were what my brother would have wanted. He wasn’t in great health for a few years, he had not chosen a patient advocate, and he hadn’t discussed his wishes for care. Nothing was in writing.” “My oldest brother, age 58, who learned he had cancer, passed away in October. And, it’s easy to use.” Gerald Primeau – Mercy Ambulance EMS director Through a partnership with Omega House and Katriina Hopper, MD, Upper Peninsula Health ServicePortage, we are now able to assist patients on Dr. Hopper’s referral, with the use of a new Michigan initiative called, MI-POST (Michigan Physician Orders for Scope of Treatment). MI-POST is designed for people who may be in their last 12 – 18 months of life. Unlike other forms of advance care planning, MIPOST, when signed by the patient and physician, becomes a set of physician’s orders that are based on your personal preferences for end of life care. You will be hearing more about this program in the coming months. The Keweenaw Community Foundation grant has allowed us to provide a large number of printed resources for healthcare professionals and the public. Page 7 Toni White: 2014 Volunteer of the Year — During the Omega House Volunteer Appreciation lunch in April, Deb Young presented the Volunteer of the Year award to Toni White, citing her years of dedication. Toni has volunteered with Omega House since before the House doors opened in 2005. During those first couple of years, she volunteered two to three times a week, saying “I helped out where I was needed. In the beginning I helped move equipment from Dr. [Sigurds] Janners’ office. Later, I became involved with our residents. I love helping out with resident care. “ Before her time spent at Omega House, Toni volunteered with Keweenaw Aspirus Home Health and Hospice. Now she splits her time between Omega House and Little Brothers Friends of the Elderly, where she and her husband, Bill, who plays the accordion at LBFOE Tapiola events, have spent the past 10 years helping out on holiday and Sunday afternoons. She said, “These people become like a family to you.” When asked why she is dedicated to hospice, Toni told the story of living in Maine, years ago, when her stepfather the time and should commit to volunteering, if only for a few hours each week. “We have so many wonderful resources for the elderly in the Keweenaw, and people are so friendly. It’s great to see so many of our friends and neighbors receive such good care and to witness first-hand the generosity of our community and the dedication of our volunteers, and employees. Recently a close relative was a resident, and for the first time since joining the board a decade ago I experienced Omega House from this perspective. Reflecting on my loved ones passing I can say Omega House is vitally important to our community and the support it receives is well deserved. It continues to amaze me when we need volunteers for anything from shoveling snow, to mowing the lawn, to picking weeds from the gardens, we just have to put the word out to our volunteers and the work gets done. We have a fantastic group of people that will volunteer to do whatever is needed to help Omega House. The community support both with the volunteers and financially for Omega House is incredible and I want to personally thank all of you for your help. It is a great place with a great staff and a great volunteer group of people and I am very proud to be a small part of Omega House. We have invited Dr. Dennis McCullough, a native of Dollar Bay and nationally known family physician to help us celebrate. Dr. McCullough teaches at The Giesel School of Medicine at Dartmouth and is affiliated with The Dartmouth Center for Health and Aging. He is the author of “My Mother, Your Mother: Embracing Slow Medicine. His own mother, Bertha, died at 92 at Omega House. In his book he talks about how “Slow Medicine” was born from the Italian “Slow Food” movement. “This family-centered country first conceived of the Slow Food movement to counter the invasion and industrial excesses of American fast foods. Later, they extended the concept to their urban environments, designating Slow Cities, where automobiles were banned from central plazas, helping grandmothers to be safe and welcome on the streets. Slow Medicine’s ultimate goal is a practical and qualitative change in care directed by a more complete respect for and fuller understanding of the particularity of each late-life elder. There will be three aspects to our celebration: A Memorial Hike for the House, a public celebration with a community educational event, and a health care provider presentation. Omega house is such an asset to our area. The support it receives in so many ways, demonstrates how caring the people of this area are. Clare Gaff, Board Member Page 14 Health Provider Education: On Friday September 18th Dr. McCullough will provide a talk open to all UP health care providers, volunteers and health science students in the late afternoon at the Finnish American Heritage Center in Hancock. We are inviting the health care community from across the Upper Peninsula for his presentation: Slow Medicine for Elders. Dr. McCullough shares about his talk: “For the past 50 years as scientific and technological advances have re-shaped medical practice, health profession educators have regularly mounted efforts to maintain humanistic and other non-science elements in curricula. As high-tech Fast Medicine has pushed for more and more “efficient” use of time, with disease protocols and financial business models taking attention away from contact with patients and families (and with one another!), many patients and professionals express dissatisfaction with these new regimented conditions. Slow Medicine, especially for elders and their families, affords an opportunity to re-engage in humanistic care, at the same time improving effectiveness—including cost effectiveness—for the rapidly increasing number of frail older patients. Slow Medicine offers an approach for improving care, embracing humanistic values, and making our current medical systems sustainable.” Memorial Hike for the House: On Saturday morning, September 19th at 9 am join us at the Kestner Waterfront Park in Houghton for our annual Hike for the House. Dr. McCullough will be participating in the hike. This year the hike will be a memorial walk. We welcome hikers of all ages and physical abilities to walk with us and to remember loved ones who have died, including the 375 area residents who have lived and died at Omega House. Flower bulbs will be provided with the name of those being remembered attached to them and planted by hikers and volunteers along the public pathway that is accessible to those in wheelchairs and walkers. If you need assistance getting to or from the hike or at the Omega House 2014 Annual Report House Ten Year Anniversary hike let us know, and we will arrange assistance. If you would like a bulb with the name of your loved one, let us know and we would be happy to accommodate your request. The person you are remembering did not have to receive hospice care or be a resident of Omega House to be remembered. Community Celebration and Educational Program on Elder Care: Saturday afternoon September 19th we will host a public celebration from 5-7 pm at the Finnish American Heritage Center, hors d’oeuvres and refreshments will be served. Dr. McCullough will be present for a book signing in partnership with Omega House and North Wind Books and we will share our new strategic plan with the community. Dr. McCullough will present a program on elder care for the entire community: In his own words, “We are facing a future where we foresee limited numbers of elderoriented medical care providers (primary care MDs, NPs, and sympathetic specialists) in most rural settings. How might we use these circumstances as an opportunity to re-structure both our thinking and our care systems to use scarce resources more wisely? This discussion will consider obstacles in Omega House 2014 Annual Report our common attitudes and practices and suggest how they might be overcome as a community." In the past twenty years it has been my great privilege to work in hospice and end of life care. During that time I have been blessed to accompany many great souls in their final months and days of life. The most courageous of all the people I have met are not the ones who fought to find a cure to the bitter end when no cure was possible. Nor were they the people who became depressed and resigned to their fate. But rather the most courageous have been those who accepted the reality of their situation and lived life as fully as possible in the face of terminal illness. These courageous souls shine as the brightest stars in the sky and the light they share is filled with love, curiosity, humor, laughter and tears. This covers the expenses of 24 hour care from trained Resident Care Assistants, home cooked meals, personal care, laundry services, housekeeping, all house utilities, volunteer services, snow and waste removal and other essential services. Omega House provides care to anyone in need on a sliding scale basis so residents are charged a daily fee based upon their income and assets and are billed monthly. SHF Grant to Help Fund Room and Board for Low Income Residents in 2015 The largest grant ever awarded to Omega House came from the Superior Health Foundation in the fall of 2014. The gift of $25,956 will be used solely to help cover the cost of room and board low income residents during 2015. Omega House board members Don Van Uum and Brian Geshel are heading up a committee this spring, reviewing the sliding scale fee system for room and board charges, with a goal of trying to make our care as assessable and affordable as possible to those in need. Ray Weglarz, said, “We are thrilled to receive this award and grateful to Jim Lajoie, SHF Executive Director, and their entire board for this generous gift. This gift helps us fulfil our mission of providing care to anyone in need, regardless of their financial circumstances.” Thank you Superior Health Foundation. Omega House 2014 Annual Report Omega House Legacy Society You can become a member of the Omega House Legacy Society and leave a legacy of compassion by including Omega House in your will or estate plan. A Legacy Gift to Omega House may provide tax advantages for you and your heirs. Your Legacy Gift to Omega House can be recognized or you may remain anonymous. Omega House has created a Planned Giving Advisory Council with experienced professionals from our community who help advise us in the area of planned giving. We encourage you to consult with your tax advisor, attorney, or financial planner regarding such gifts. The options listed in this brochure give you flexibility, control and the enjoyment of your assets during your lifetime. Bequest A bequest allows you to support Omega House in the future without gifting away your assets during your lifetime. You can make a bequest donation in your will or living trust by designating Omega House as the beneficiary of cash, real estate, securities or by leaving us a percentage of your estate after all other obligations have been fulfilled. A bequest remains revocable until your passing, allowing you to change your mind at any time. Beneficiary Designation The simplest way to leave a Legacy Gift to Omega House is through a beneficiary designation. Some assets like retirement plans, life insurance policies, and annuities are not distributed as part of your will or trust. The beneficiary is determined by you on the “beneficiary designation form” provided by your account administrator. You can request to update your designation form at any time and simply name Omega House as the full or partial beneficiary of your plan’s assets. You may change your mind at any time during your lifetime. Real Estate A gift of a remainder interest in your home, camp, cottage or vacant land allows you to enjoy and control the property during your lifetime, and transfer ownership to Omega House upon your death. Such an arrangement avoids probate, saves your heirs from having to maintain or sell the property after your death, and may even provide lifetime income tax benefits. This type of gift will not impact your property taxes, as Omega House 2014 Annual Report the principal residence exemption will be unaffected, and the taxes on your property will not be “uncapped.” This gift can also be changed or revoked if your circumstances and needs change. Contingent Gifts Contingent Gifts only come into effect should something unforeseen happen to your primary beneficiaries. Contingent gifts can be made by a will, trust or a beneficiary designation form. Other Gift Options More complex gift options include: Charitable Remainder Trusts, Charitable Lead Trusts and Charitable Gift Annuities. These gifts can provide tax advantages for you and your heirs but do require the assistance of an estate or financial planner experienced in these matters. The only qualification for eligibility to Omega House is to be in a hospice program or qualify for hospice care. The diagnosis can be related to any life-threatening illness: stroke, heart failure, kidney failure, degenerative diseases and cancer, to name only a few. Omega House is for those individuals who need additional care that prevents them from residing in their own homes. Residents are supported physically, emotionally and spiritually, while maintaining their independence, dignity and respect.
