creditcardnumber.Pleaseincludetwoidenticalcopies(nomorethan15pageseach)of
anexcerptfromyourworkofnon-fiction,fiction,poetryorplayforassessment.Please
ensurethatpagesarenumbered,double-spacedin12-pointfont,andthatyourname
appearsoneachpage.Includetwocopiesofacoverletterthattellsalittleabout
