This has been a year of exciting changes for Mental Health Partners in our pursuit
of the Triple Aim for healthcare improvement:
• excellence in quality,
• excellence in client experience, and
• excellence in cost efficiency.
The grand opening of our Wellness Education Center in Longmont in December was the
culmination of an intensive collaboration with staff and external partners to redesign our services
with the Triple Aim goals in mind. The Journeys program located there offers state-of-the-art
services for adults with mental illness by supporting them in managing their illness and building a
meaningful life in the community, frequently through pursuing educational and vocational goals.
The vision for the Center is to integrate wellness and physical health awareness, and so reduce the
25-year disparity in life expectancy for adults with serious mental illness.

With the launch of the Wellness Education Center, we also announced a change in our name to
Mental Health Partners. Partnership is core to our values and our approach to providing services.
We emphasize partnership not only with clients, families, and staff, but also with a large number
of human services, health care, education, and criminal justice partners, as well as local funders,
advocates, and stakeholders. Our collaboration on joint planning at both the individual client/
family level and at the larger healthcare systems level is essential to achieving the Triple Aim goals
in our community. We continue to strive to be the center of excellence and provider of choice for
mental health and wellness services for residents of Boulder and Broomfield counties.
Our next phase will be to implement the redesign and the Wellness Education Center in Boulder.
We look forward to continuing to work with all of our partners to ensure that our vision of
Healthy Minds, Healthy Communities becomes the reality in our community.
We are grateful to our community and to our partners for the unparalleled support that has
contributed so much to the success of Mental Health Partners.

Journey to a Culture of Recovery and Excellence
Our Clients tell us…

Our strategic plan begins with our vision Healthy Minds, Healthy Communities, and focuses on
activities to reach more people, enhance the services they receive, improve the client’s experience,
and increase efficiency. These activities prepare us for the changes coming with healthcare reform and
our goal to become the provider of choice for mental health and wellness services. New programs are
already producing positive results:
Journeys, a best practice psychosocial rehabilitation program for adults with serious mental illness,
served more than 250 clients in its first year. Clients work with recovery care coordinators on selfidentified goals and skill-building and have access to benefits specialists, employment specialists and
other services.
Journeys clients seek our crisis services significantly less than clients in our more traditional program
for adults with serious mental illness.
Our new Assessment program has a strong customer focus, ensuring
clients are able to directly and easily access the program that best
meets their needs and minimizing the number of times
clients have to “tell their story” before meeting their
primary clinician.
The time to first appointment has steadily
decreased since the implementation of our new
Assessment Model.
Our new Employment program supports
clients to find work, a significant way to
support recovery for adults with serious
mental illness.

Our Clients tell us…
More than 50 clients have found jobs through our Employment services.
Our new Wellness program offers a variety of services including classes, workshops, and wellness
coaching. In the past year, the Wellness program has served more than 100 clients and has offered
numerous classes focusing on wellness, employment and self-sufficiency.
“They [benefits specialists] are really helping
me out and one thing that’s nice is that
unless you just really can’t do it by yourself,
they leave it on you and they make you be
part of the process. And if you honestly just
can’t figure out the next step on your own,
they’ll definitely give you a hand, but they
want you to feel like you’re independent and
not that ‘We’re babysitting you and we don’t
think you know what you’re doing.’ And so
that’s nice.”
Another important goal is to provide cost-effective services. By improving efficiency and decreasing
costs – while maintaining quality of care – we are able to serve more clients and be responsible in our
use of resources.
We compared the cost of care for Journeys to the previous adult intensive services program. Many
of the same clients were first served in the intensive services program and then moved into Journeys.
The cost of their care decreased an impressive 42%.
We are proud of our success in making a difference in our community and for our clients!

I decided to work at the Wellness Education Center because I realized,
due to first-hand experience, that recovery is not only possible but
probable. As a Peer Support Specialist I have a unique and special
connection to the client. I am a strong believer in a self-directed recovery
so I talk a lot about goals and dreams. By making a strong connection
based on shared experience, mutual trust and respect happen in the
relationship. This is where the real work begins. There are many different
roads to Recovery and each individual, deep down, knows what their
need is. Sometimes they just need someone who is on their side and will
support them through the ups and downs.

I have started two ongoing groups: Coffee Talk and Spirituality Awareness, each group speaking to different
needs of the client. Coffee Talk is purely recreational. It gives clients a safe space in the community to share
and enjoy each other, sometimes making new friends. Spiritual Awareness helps clients to look within,
concentrating on topics like faith and forgiveness.
There are many other responsibilities for the Peers and it is quite a fascinating profession. If coupled with
creativity it can be a great pleasure to serve the common good and a great way to give back in so many ways.

I never imagined in my wildest dreams that I would be working as a Peer Support Specialist here at the
Wellness and Education Center, with all the mental and physical constraints I have had to learn to live with,
and considering I used to be my own wildest nightmare. Working in this environment has opened my eyes
and my heart to what it means to be a better person or, perhaps I should say, learning to become a better
PEER. Being a PEER means helping others to try to better themselves. If I can help someone for a few
moments in time or, better yet, make someone feel better about where they are in their lives at the moment,
then a day’s work is complete.
A Peer Support Specialist does require training, but the best training comes from my own life experience.
Clients and I may share an experience, which often softens their first impression of me and allows them to
open up. It’s not only a Peer’s life experience that allows for the success of an individual’s wellness, but also
working in a building that encourages all who enter to “feel comfortable” when they open the doors to the
Wellness Center. The open environment allows for nurturing of relationships and to stay connected to people
in general. It applies equally when going to a movie, to church or perhaps a concert; you just feel better being
around people, no matter what your business may be. We all are the movement of positive energy working for
the same goal, which is to simply do and be the best one can be while we are here.

Charitable support provides opportunities for recovery and wellness to our friends and
neighbors coping with mental illness.
Your gifts to the Foundation support programs and services provided by Mental Health
Partners in more than fifty locations throughout Boulder and Broomfield Counties. Whether
through funding an expressive arts program for adolescents to foster creativity and self-esteem,
creating community garden space for adults to enhance exercise, social engagement and
nutrition, or offering regular programs to care for and work with horses under the guidance of
equine professionals, your gifts create the opportunities that help change people’s lives for the
better.
Following the lead of our sister organization, our Trustees changed our organization’s name
to the Mental Health Partners Foundation to reflect our shared commitment to the principle
of partnership. Our most essential partnership is with you, our donors, and we are dedicated
to ensuring that this partnership reflects your determination to provide hope and support for
people experiencing mental illness.

A community focusing on wellness, work and recovery.
Chinook Clubhouse is a vocation and social rehabilitation program of Mental Health Partners
with the mission “To provide a warm, inclusive community with the goal of encouraging adults
with mental illness to proceed at their own pace toward recovery and independence, using their
unique strengths, talents and abilities. Through a partnership among staff, Clubhouse members, and
volunteers, we create opportunities for volunteering, paid employment, education, and social and
wellness activities.”
Employment Services
The vocational component of Chinook Clubhouse assists members in preparing for, securing and
retaining employment. The first step is accomplished through participation in the Clubhouse
units, which increases stamina, self-confidence, problem-solving skills, and the ability to interact
and work cooperatively with a variety of individuals. With increased opportunities a priority, we
were able to meet outcome goals with the Colorado Division of Vocational Rehabilitation, as well
as navigate a difficult economy and transition of vocational staff.
Employment is a major step in becoming reintegrated into society, which is why it is one of
the primary functions of the Clubhouse. Through job training, transitional and supported
employment and eventually independent employment, our members are able to “work through
their mental illness.”
Whether an individual is entering the workforce for the first time, or reentering after a long
absence, the Clubhouse offers a variety of options designed to assist with this important
transition.

Transitional Employment (TEP)
Members are placed in part-time entry-level jobs with local businesses for a period of six to nine
months. The Clubhouse staff selects, trains, and provides on-the-job-support. Additionally, if the
member can’t make it to work, the Clubhouse staff fills in for the member at no cost to the employer.
Nineteen Members participated in Transitional Employment for FY 2010/11.
Supported Employment
The Clubhouse helps the member obtain a job in the community and provides the support necessary
to maintain the position. These jobs have no built-in time limits and members must go through
the regular application and interview process. Forty-four members participated in Supported
Employment for FY 2010/11.
Independent Employment
The member obtains a job in the community without assistance from the Clubhouse. Support is
provided to assist the member to sustain and upgrade the employment.
Examples of support provided to our members: resume preparation, interview practice, referral to
Colorado Division of Vocational Rehabilitation, job search, obtaining and filling out applications,
problem solving, transportation to the interview. Eighteen members participated in Independent
Employment for FY 2010/11.

Beautiful Art for Beautiful Minds
On May 13, 2011, Chinook Clubhouse presented its eighth annual Beautiful Art for Beautiful
Minds (“BABM”) benefit and auction at Macy’s Twenty Ninth Street, a festive evening with music,
wine and delicious food provided by the restaurants of Twenty Ninth Street and desserts by Elvis
in the Oven. Holly and the Husbands entertained with delightful music, as did Member Laurie
Dameron who also told of her experience with mental illness and what Chinook Clubhouse has
done for her recovery. Over 200 guests bid on the 150 art and auction items, which raised over
$30,000 for Chinook programs. We are grateful to the Boulder artist community and other
contributors who donate their pieces and services to assist Chinook Clubhouse in serving the
mental health community.

Spotlight on Recovery and Inspiration
Rita
Rita has been a member of the Clubhouse for two years. She is
from West Virginia and came to Colorado in 2009 to be closer to
her teenage daughter. Rita has struggled with bipolar disorder for
ten years, is a cancer survivor and has been homeless twice in her
life, living in shelters in Kansas and West Virginia. Rita was not in
treatment and was guided to Mental Health Partners by the Boulder
Shelter for the Homeless staff. She lived on the streets and in the
shelter from April to December of 2010 when she was awarded a
housing subsidy. Rita came to the Chinook Clubhouse not knowing
what to expect. She found support, friends and self-confidence. She
stated that at the Clubhouse she is “around others struggling with
something similar and you don’t feel alone.” Rita regularly participates
in the Art Group and social activities offered at Chinook.
Rita recently completed a six-month transitional employment (TE) placement in the Human
Resources department at Mental Health Partners. She managed the filing system and provided
payroll and administrative support. When asked how the transitional employment opportunity
aided in her recovery she said it helped to “build self-confidence and I am more self-assured.”
She not only gained new skills, but she also built stamina and “felt a part something.” Rita was
given an Everyday Hero Award by the ladies with whom she worked in the HR department
in recognition for her efforts, stating that she “embraced and fulfilled all of the TE job duties
with a positive attitude. We appreciate your growth and willingness to jump in and assist at
any moment.” Rita is currently looking for a job in the community, aided by the Clubhouse
and the Division of Vocational Rehabilitation, and is also volunteering at Ares Thrift Store
where she is organizing and displaying donated items. Rita is a source of inspiration and an
example of perseverance. The process of recovery is different for everyone, but it can happen!
