Welcome to Southwest General
Southwest General Health Center is a private, not-forprofit, 358-bed acute care facility located in Middleburg
Heights, Ohio, serving Southwestern Cuyahoga, Eastern
Lorain and Northern Medina counties. Founded in 1920
by the citizens of the surrounding communities, Southwest
General has a rich history of community partnership and
a deep commitment to providing personalized care and
building a healthy future for the patients, families and
communities it serves. Additionally, Southwest General
is home to nationally recognized physicians with full
access to state-of-the-art technology.
In 2011, the decision was made to expand the
hospital’s Main Campus facility. The “Building
for Your Future” expansion project was undertaken
with our community members’ and patients’ needs
and preferences in mind. The project includes
a new Emergency Room (ER), Critical Care Unit,
96-room private patient bed tower and a 250-car
underground parking facility. We anticipate an early
2014 grand opening for the new ER and Critical Care
Unit, and completion of the bed tower is slated

Condition Help
Southwest General, like many
hospitals nationally, is focusing on
eliminating problems that affect optimal
patient care. As a result, we’ve created
a ‘Condition Help’ line for patients,
family or visitors to call for assistance.
When to Call
You can call for Condition Help:
n If, after speaking with a member of the
health care team (i.e. nurses, physicians),
you continue to have serious concerns
about how care is being given, managed
or planned.

phone. This is a special line just for
Condition Help. The hospital operator

During Your Stay
We have policies and procedures in place to help you
and your family work with our doctors, nurses, and staff
to get the most from your hospital stay. Please take a few
minutes to review these guidelines with your loved ones.
Visitor Guidelines
We encourage visitors for emotional support and
recovery. To provide a restful and safe environment, we
ask that all visitors comply with the following guidelines:
n Be considerate of other patients by keeping noise
to a minimum.
n Refrain from visiting if you have a cold, sore throat,
or any contagious disease.
n Observe “No Visiting” and precaution signs before
entering the room.
n Leave the room during tests or treatments if asked.
n Aged 16 and older can visit in the ICUs.
n No children under aged 12 in Maternity, except
siblings of newborn.
n Geriatric Behavioral Health Unit accepts visitors
11 a.m.-noon and 6-8 p.m.
Smoking
Smoking and/or the use
of any tobacco product is
not permitted anywhere
in the hospital or on
hospital grounds.
No Photography
To protect the privacy of
all patients, please refrain
from taking personal
pictures and video while
in the hospital buildings.
ATM
For your convenience,
an automated teller
machine (ATM),
sponsored by Fifth Third
Bank, is located in the
Main Lobby.
Patient Information
To obtain updated condition reports a patient (good,
fair, serious or critical), call the Nursing Station of the
hospital unit directly, and designate one family member
to act as the “spokesperson.” The spokesperson can then
relay the information to other family members. Please
avoid calling during shift change times: 7-8 a.m., 3-4 p.m.
and 7-8 p.m. and 11 p.m.-midnight. Patients have the
right to sign a form requesting that no information about
them or their condition be released to anyone.
Public Restrooms
Please do not use the bathrooms in patient rooms.
They are reserved only for patients in order to protect
their health. Public restrooms are located throughout
the hospital. Male and female restrooms are located near
the Main Lobby elevators on all floors.

Shuttle Service
On the hospital’s Main
Campus, the long walk
from the South parking
areas to the Main Lobby
can be eliminated by
accessing our shuttle
for patients and family
members. The shuttle,
operated by hospital
volunteers, is available
7:30 a.m.-4 p.m., Monday
through Friday, and is
located just inside the
first floor parking garage
entrance. This area also
is in front of the L. Jon
Schurmeier Medical
Pavilion (Building C).

access the service.
Parking
For patient and visitor convenience, Southwest General
provides free parking both in the Garage attached to the
hospital and on surface-level lots adjacent to the Main
Campus buildings.
Valet Parking
Those needing to drop off patients at the Emergency
Department entrance and accompany them into the
hospital can have Protection Services (Security) officers
park their vehicle for them. The car will be parked in
either the Emergency Department parking lot or an
adjacent visitor lot. This service is available 24 hours
a day, seven days a week. Southwest General also offers
other patients and visitors a valet parking service.
The service is operated by ParkMed, a company that
specializes in valet parking for health care facilities.
The service is offered Monday through Friday at the
following locations:
n Buildings A & B (Outpatient Services entrance)
8 a.m.-6:30 p.m.
n Building C (South entrance) 8 a.m.-4:30 p.m.
The fee for valet service is $3 per car and is free for
anyone with a handicap placard, any ED patients
or expectant mothers. For more information, call

Security Assistance
Southwest General’s Protection Services (security)
staff members provide 24-hour-a-day service within
the hospital and surrounding campus areas. They
will be glad to help visitors in emergency situations
or those requiring an escort to parking areas around
the hospital. They also provide vehicle unlocks,
jump starts and tire inflation. For assistance, call

Van Transportation
To assist patients who live in the hospital’s taxing
district (Berea, Brook Park, Columbia Township,
Middleburg Heights, Olmsted Falls and Strongsville),
free van transportation is available for residents
who need transportation to and from the hospital, LifeWorks (available for Physical Medicine & Rehabilitation
Services only), Southwest General’s ambulatory facilities
in Brunswick and Strongsville, and the UH Seidman
Cancer Center at Southwest General. This service can
be scheduled by calling 440-816-4046.
Wheelchairs
Wheelchairs are available for use within the hospital. Ask
the staff and/or volunteers at the Information Desk in the
Main Lobby, and they will be happy to provide access to
a wheelchair, or call extension 8940 from any phone.
Valuables
If you cannot send your valuables home, please speak
with your nurse about storing them in the hospital
safe. Southwest General cannot be responsible for
any valuables left in your room.
Lost and Found
There are two Lost and Found areas within the hospital.
The area for valuables such as jewelry, cell phones
and wallets is located within Protection Services,
which it is open 24 hours a day, seven days a week.
If you (or a family member) need to locate a missing
article, call 440-816-8884. For items such as
clothing, books and other less valuable items,
contact Environmental Services at 440-816-8660.

All medications you take while a patient at Southwest
General have been prescribed by your physician and
administered by a trained professional. Patients are not
permitted to administer their own medications or to
keep personal medications at the bedside.

these agencies to make
individual arrangements.

the costs associated
with this service.

Southwest Community Pharmacy
Located in the Main Lobby, the retail Southwest
Community Pharmacy makes it convenient for patients
to fill a prescription when leaving their physician’s
office or upon discharge. The pharmacy offers the
Good Neighbor Pharmacy Prescription Savings Club,
e-vouchers and manufacturer loyalty and discount cards
to help with out-of-pocket costs. The pharmacy also
offers some durable medical equipment, Medela Breast
Pumps, a variety of over-the-counter medications and
health products, and employs pharmacists certified
in immunizations. Southwest Community Pharmacy
accepts most third-party insurance cards and offers
a 10 percent senior citizen discount. Pharmacists
are available to discuss your medications and answer
questions before going home.

TTY-TDD telephone service for the hearing-impaired
is available by calling the operator or extension 4054.
Additionally, sign language interpreters are available

Telephones are provided in all patient rooms.
Incoming calls will not be accepted before 7 a.m.
or after 9 p.m. Local calls are provided free of charge.
To make a call, dial “9” to obtain an outside line,
followed by the local number. Outgoing calls can
be made at any time. Long-distance calls can be made
by dialing “0” for the hospital operator. A credit
card must be used for long-distance charges, or the
call must be billed to the patient’s or family member’s

to take phone calls.
Fire Safety
We periodically conduct fire drills. If you hear an alarm,
stay where you are. In the event of an actual emergency,
hospital staff will notify you.
Electrical Appliances
Electrical appliances, including hair dryers, curling irons,
razors, radios, heating pads, portable heaters, VCRs,
computers and other devices are not permitted in patient
rooms. You may use only battery-operated devices.
TV
Your cooperation is appreciated in using the complimentary headset for TV viewing in an effort to provide
a quiet environment for patients.
The on-demand health education channels are 37
through 48. Channel 49 is the 24-hour continuous-play
Relaxation Channel. These channels can be viewed by
our patients at no cost. Please see the TIGR VideoOn-Demand patient health education program guide
for your selections.
Mail and Flowers
Mail and packages will be delivered to you by a hospital
volunteer. To ensure that mail is efficiently delivered
to patients, family members and friends must use
your full name. The correct mailing address for

Postage stamps are available in the gift shop, ATM
in Main Lobby or hospital cashier (located on the
Basement Level). Please note that flowers are prohibited
in Intensive Care Units. Latex balloons are NOT
permitted in the hospital.
Room Service Meals
Patients who are hospitalized on the Medical/Surgical
and OB units have the option to order their meals
through Room Service. A Host or Hostess from
Nutrition Services will visit each patient to take their
meal orders close to meal time from our larger selection
of food items.
Guest Meals
If a family member or friend would like to share a meal
with a hospitalized loved one, they can call Nutrition
Services from the patient’s room at extension 8842 or
from home at 440-816-8842 to order a meal from the
guest tray menu. There is a $6 charge for the meal that
must be paid by credit card when the meal is ordered.
Guest trays will be delivered with the patient’s regular
meal service.
Southwest Community Diners Club
Through this club, you can enjoy the wonderful food
from the Courtyard Café AT A DISCOUNT! If you
are aged 60 or older, you will receive a 10 percent
discount in the Courtyard Cafe on any item or meal,
any day of the week! Simply stop by and fill out a short
application to receive a Southwest Community Diners
Club card to activate the discount at the register. If
interested, let us know on your application if you’d
like to be e-mailed our weekly menu. Additionally, our
menus are posted at many community centers and senior
residences. For more information on the Southwest

7 a.m.-3:30 p.m., weekdays.

The Corner Shop is located off the Main Lobby.
The shop features a variety of gift items as well as fresh
and silk flower arrangements. Please note that flowers
are restricted in certain areas of the hospital.

This is a non-profit organization offering free web pages
to patients receiving care in a hospital. It allows family
members to communicate the condition of their loved
one while hospitalized.
Spiritual Care
Clergy members are always welcome to visit patients
at Southwest General. Additionally, Spiritual Care
staff members provide emotional and spiritual care
and support for patients, family and staff members
and are available 24 hours a day. Ask your nurse
to call the hospital operator who will page one
of the Spiritual Care team members. For information,
call 440-816-8648.
Why is my loved one confused?
It is not uncommon for our patients to become confused
during their hospital stay. Patients may become agitated
and difficult to orient. There are many possible causes
of delirium (confusion). Some of the most common
causes are medications, surgical procedures, acute
infection and unfamiliar environment. Occasionally,
the nursing staff may contact you to see if you can
spend more time at your loved one’s bedside. Your
familiar presence can be a calming influence and may
actually help them recover faster.
The level of care specified
for a hospitalized patient
is either “observation”
or “inpatient” and is
determined by the
patient’s diagnosis
and treatment. The

transporters. The following information will help make
your hospital stay safe and comfortable.

While you’re in the hospital to get well, there is the
possibility of developing an infection. The single most
important thing you can do to help prevent infections
is to wash your hands and make sure that everyone
who touches you—including your doctors and
nurses—wash their hands, too.
You, your family and friends should wash hands:
1. after touching objects or surfaces in the hospital room
2. before eating
3. after using the restroom
It is also important that your health care providers
wash their hands with either soap and water or
with an alcohol-based hand cleaner every time,
both before and after they touch you. Health care
providers know to practice hand hygiene, but
sometimes they forget. You and your family should
not be afraid or embarrassed to speak up and ask
them to wash their hands.

Preventing Falls
Hospital patients often fall because they are weak or
unsteady due to illness or medical procedures, their
medications make them dizzy, or because they’ve been
sitting or lying down for too long. Southwest General
cares about our patients’ safety. Please help us keep
you safe by following these guidelines during your
hospital stay:
n Do not get out of bed by yourself. Your hospital
bed is probably higher and narrower than your bed
at home and you can easily fall while trying to get
in or out of it. Please use your nurse call button
and ask for assistance.
n Keep often-used items—call button, tissues, water,
eyeglasses, telephone, TV remote—within easy reach.
n Do not walk in bare feet. Wear non-skid socks
or slippers.
n Make sure your robe or pajamas don’t drag
on the floor; they can cause you to trip.
n Use the handrails in your
bathroom and throughout
the hospital at all times.
n Ask your nurse to show
you how to properly
walk with your IV
pole, drainage bags or
any other equipment.
n Be sure your
wheelchair is
locked when
getting in or out
of it. Never step
on the footrest.
n If you see a
spill on the
floor, report
it at once.
DVT: Lower Your Risk
Deep-vein thrombosis
(DVT) occurs when blood
clots form in the legs and
block circulation. The clots
can lodge in your brain,
heart or lungs, causing
damage or even death.
When you’re hospitalized
and in bed with limited
physical activity, your risk
of DVT increases.
Ask your doctor about
using compression boots
or stockings and/or blood
thinners to prevent DVT
during your stay. And be
sure to tell your doctor
or nurse if you have

What are Your Advance Directives?
What Are Advance Directives?
A living will, health care proxy and
durable power of attorney are the
legal documents that allow you to give
direction to medical personnel, family,
and friends concerning your future care
when you cannot speak for yourself.
You do not need a lawyer in order to
complete Advance Directives.
Patient Services Advance Directives
You have the right to make decisions
about your own medical treatment.
These decisions become more difficult
if, due to illness or a change in mental
condition, you are unable to tell your
doctor and loved ones what kind
of health care treatments you want.
That is why it is important for
you to make your wishes known
in advance.
Here is a brief description of each
kind of Directive:
Living Will
A set of instructions documenting
your wishes about life-sustaining
medical care. It is used if you become
terminally ill, incapacitated or unable
to communicate or make decisions.
A living will protects your rights
to accept or refuse medical care
and removes the burden for making
decisions from your family, friends
and medical professionals.
Healthcare Proxy
A person (agent) you appoint to
make your medical decisions if you
are unable to do so. Choose someone
you know well and trust to represent
your preferences. Be sure to discuss
this with the person before naming
them as your agent. Remember that
an agent may have to use their
judgment in the event of a medical
decision for which your wishes
aren’t known.
Durable Power of Attorney
For health care: A legal document
that names your health care proxy.
Once written, it should be signed,
dated, witnessed, notarized, copied
and put into your medical record.
For finances: You also may want
to appoint someone to manage your
financial affairs when you cannot.
A durable power of attorney
for finances is a separate legal document
from the durable power of attorney
for health care. You may choose the
same person for both, or choose
different people to represent you.
You must provide a copy of your
Advance Directive each time you are
admitted. In this way, you ensure
that the hospital has your most
current information.

Preparing to Leave the Hospital
When You Are Discharged
Your physician determines when you
are ready to be discharged. Your physician,
pharmacist or nurse will give you discharge
instructions and answer any questions you
have about managing your treatment and
medications once you are home. If you are
confused or unsure about what you need
to do, what medications you must take or
if you have to restrict your diet or activities,
don’t be afraid to ask and take notes.
Be sure you understand any instructions you have been given before you
leave the hospital.
After you are home, you will receive
a phone call to “check in” and be certain
you have the information you need to
be healthy and safe.
Going Home
When your doctor feels that you are ready
to leave the hospital he or she will authorize
a hospital discharge. Please speak with your
nurse about our discharge procedures.
Here are few tips to make the discharge
process run smoothly:

n Be sure you and/or your caregiver have
spoken with a discharge planner and that
you understand what services you may
need after leaving the hospital.
n Verify your discharge date and time with

and bedside table carefully for any
personal items.
n Retrieve any valuables you have secured
with Protection Services.
n Make sure you or your caregiver has all
necessary paperwork for billing, referrals,
prescriptions, etc.
Homegoing Prescription Services
Save stopping at a pharmacy on your way
home. You may have your prescriptions
filled here before you leave. They will be
ready to pick up on the way out in the

Billing and Insurance
Billing: What a Hospital Bill Covers
The hospital bill covers the cost of
your room, meals, 24-hour nursing care,
laboratory work, tests, medication, therapy and the services of hospital employees.
You will receive a separate bill from your
physicians for their professional services.
If you have questions about these separate bills, please call the number printed
on each statement.
The hospital is responsible for submitting
bills to your insurance company and will do
everything possible to expedite your claim.
You should remember that your policy is
a contract between you and your insurance company and that you have the final
responsibility for payment of your hospital

Geriatric Behavioral Health Unit
This is an 18-bed inpatient unit at Southwest General designed to help with the
mental health needs of patients aged 65
and older and provide support for their
families. The unit is directed by psychiatrists experienced in geriatric care, while
family physicians and internal medicine
specialists address any existing medical
conditions. Individualized treatment
plans are developed for patients to help
them through difficulties with behavior
that threatens the safety of themselves,
others or property; depression; talk of
hurting one’s self, suicide or attempted
suicide; behavior that interferes with their
caregiver’s ability to care for them at home
or in other living situations; confused
thinking or disorientation not related to
an acute medical condition; hearing or
seeing things not present or having bizarre
thoughts; and sudden onset of memory
impairment and/or confusion, or dementia requiring behavioral stabilization. For

Driving Program for Seniors: As the
population continues to age, the number
of senior-aged drivers also is increasing.
Slower reaction times, reduced vision and
coping with a loss of independence are
some issues that older drivers cope with
daily. Their families often struggle with
how and when to discuss these issues
with their senior loved ones. Southwest
General has a program called Beyond
Driving with Dignity through which
social workers, certified as Beyond Driving with Dignity Professionals (BDDP),
are trained to help families address the
key issues of safety and independence
while driving. Through this program, the
BDDPs are able to assess the senior driver
and provide tools necessary to help them
and their families work through the complicated issues related to diminishing driving skills. The program is not built around
the premeditated goal of “taking the keys
away” from an older driver. Rather, the
program is designed to assist the older
driver and concerned family members
make appropriate decisions on the future

identifies isolated and at-risk elderly
through the use of nontraditional referral sources known as “gatekeepers.”
Gatekeepers are individuals who are in

Managing Your
Medications
Keep track of all medications you are prescribed while in the hospital.
The best way to avoid medication errors is by keeping
up-to-date records of all the medications, supplements,
and herbs that you take and sharing that information with
any doctors you visit. If you are hospitalized, you will be
asked to list all the medications you are taking—including
prescription medications, over-the-counter drugs, and
vitamin/herbal supplements.
Double Check!
Be sure that all of your doctors know what medications
you have been taking. They also need to be aware of any
allergies you may have. And whenever any of your doctors
prescribes new medications, be sure to ask:
n Which of my original medications/supplements
should I still be taking?
n Which of my original medications/supplements
should I no longer take?
n Will any new medications I am being prescribed
interfere with my original medications/supplements?
n What side effects do I need to be aware of ?
n Who should I call if I have questions or problems
with my medications?
