Welcome to the SAOA’s 61st Annual Scientific Meeting. Welcome to the beautiful Drakensberg mountains, another exciting tourist destination. I would like to extend a warm welcome to the Carousel Presidents, their partners, the African Orthopaedics Association Presidents present and to our international guest speakers: Peter Choong from Melbourne Australia, Ian Harding from Bristol in the United Kingdom and Rodrigo Pesantez from Columbia. They will all participate and contribute to ensure that this conference will be a memorable one. The theme of this meeting is: “Doing right the first time”. It is aimed at taking us back to the basics with regards to clinical skills, technical skills, ethics and professionalism while sowing the seed of social accountability. We look forward to a thought-provoking engagement with the speakers and our colleagues. FP Du Plessis assisted by the Congress Committee, the SAOA secretariat and ICE Solution have put together a stimulating and exciting educational and social programme. I thank them for their effort. Regards, Mkhululi “Mac” Lukhele President: SOUTH AFRICAN ORTHOPAEDIC ASSOCIATION Although the main purpose of the meeting is education, we also have the rare opportunity for group discussions of professional and business matters of mutual importance. As a bonus we can relax at the end of the day with friends and colleagues in this retreat from busy urban life, with its clear air and magnificent scenery. The scientific programme has something for everyone. Presentations covering a wide range of topics will keep us up to date in areas of orthopaedics which we may seldom practise, but are interesting and still need to know. The Instructional Courses have excellent programmes and guest speakers, with Ian Harding for Spines, Peter Choong for Tumours and Infections and Rodrigo Pesantez for Trauma. CPD has been well catered for in 3 sessions providing 6 Ethics points. We are grateful to the delegates submitting free papers as well as our Visiting Presidents who are taking part in the programme by giving lectures and conducting panel discussions with registrars. To round things off, there is an interesting variety of activities on the accompanying persons programme. My thanks to the rest of the Congress Committee, Leana of SAOA, and the ladies of ICE Solution; they did all the hard work while I made approving noises. He is the clinical head of department of Orthopaedics at Charlotte Maxeke Johannesburg Academic Hospital. He is the head of the spine unit at CMJAH where he has been training spine surgeons. He is the adjunct Professor of Orthopaedics at the University of Witwatersrand where he is also the Head of the School of Clinical Medicine. – Mac Lukhele He is one of the past presidents of the South African Medical Association and also a past president of the South African Spine Society. He is a member of the National Osteoporosis Foundation of South Africa. His interest is paediatrics spine deformity, osteoporosis and medical education. He is married to Grace Motswana Lukhele and has 4 children. He enjoys road running and occasional cycling. Visiting Presidents American Orthopaedic Association Dr Kevin Black • • • • Professor and C. McCollister Evarts Chairman Department of Orthopaedics and Rehabilitation Director, Penn State Hershey Bone and Joint Institute Vice Dean University Park Regional Medical Campus Dr. Black received his undergraduate degree from the University of Pennsylvania in 1977, and then graduated from the University Of Rochester School Of Medicine. He completed his orthopaedic residency at the University of Rochester in 1986, and a fellowship in Sports Medicine at the Cleveland Clinic Foundation. After 6 years at the Medical College of Wisconsin he joined Penn State College of Medicine and Hershey Medical Center and in 2002 he became the C. McCollister Evarts Professor and Chair. He continues to serve as Chair of the department and on July 1, 2013 he assumed responsibility as Vice Dean of the Penn State Hershey University Park Regional Medical Campus in State College, Pennsylvania. In 2001 he was a European Travelling Fellow of the AOSSM. He has served on the Board of Directors of the AOSSM and the AAOS, as well as the Executive Committee of the AOA. He has been president of the Pennsylvania Orthopaedic Society and will become President of the AOA in 2015. He served as Course Director for the “Orthopaedic Educator’s Course” from 2007 through 2013, the longest running CME program of the AAOS. His clinical practice focuses on knee ligament and patellofemoral instability. From 1990-1992 he was orthopaedic consultant to the Green Bay Packers, and since 1993 has been team physician for the Hershey Bears hockey team. In addition to his ongoing research related to orthopaedic sports medicine, in the past decade much of his effort has focused on resident education. He is a two time recipient of the Woodward Endowment for Medical Sciences Education Grant and is currently investigating methods to teach and assess resident knowledge and performance in the Systems Based Practice competency and cognitive simulation relative to surgical decision making. American Academy of Orthopaedic Surgeons Dr Frederick Azar Frederick M. Azar, MD, is an orthopaedic surgeon specializing in sports medicine in Memphis, Tenn., and the past president and treasurer of the American Academy of Orthopaedic Surgeons’ (AAOS). Dr. Azar is chief-of-staff at the Campbell Clinic, as well as a professor and director of the sports medicine fellowship program in the University of Tennessee-Campbell Clinic Department of Orthopaedic Surgery. He also was director of that department’s residency program for 10 years. Dr. Azar is the team physician for the NBA Memphis Grizzlies and was selected as the 2012 Team Physician of the Year by the trainers from all 30 NBA teams. He also serves as a team physician for the University of Memphis and Christian Brothers University sports teams. After earning his medical degree from Tulane University of Medicine, Dr. Azar completed a residency in orthopaedic surgery at the University of Tennessee-Campbell Clinic and a fellowship in sports medicine at the American Sports Medicine Institute in Birmingham, Alabama. He also was an American Orthopaedic Society for Sports Medicine (AOSSM) Traveling Fellow. Dr. Azar is active in numerous national and international groups and professional societies, including the American Orthopaedic Society for Sports Medicine (AOSSM), the Arthroscopy Association of North America (AANA), and the American Orthopaedic Association (AOA). Dr. Azar has volunteered with 7 the Academy for nearly 20 years and was a graduate of the inaugural AAOS Leadership Fellows Program class in 2003. An active participant in a number of community philanthropic organizations, Dr. Azar serves on the Board of Directors of ALSAC/St. Jude Children’s Hospital and as member of several committees. He was part of the Haiti Disaster Relief Team in 2010. Dr. Azar and his wife, Julie, have two children, Isabel and Nicholas Australian Orthopaedic Association Dr John Tuffley Dr. John Tuffley MB,BS [Queensland], FRACS, FAOrthA Doctor Tuffley’s medical and orthopaedic training was undertaken in the State of Queensland, Australia. After obtaining Fellowship of the Royal Australasian College of Surgeons [Orthopaedic Surgery] in 1984, he spent time in Switzerland and Ireland gaining experience in large joint arthroplasty, paediatric orthopaedic surgery, and in scoliosis surgery. On return to Australia, he commenced practice in Brisbane, the capital city of Queensland, initially undertaking full time public work at the Royal Brisbane Hospital and Royal Childrens Hospital, and later serving as a visiting consultant orthopaedic surgeon at both these hospitals. His current public appointment is at the newly opened Queensland Childrens Hospital where he is appointed as a consultant to the spinal deformity unit. He continues to run a comprehensive private orthopaedic practice including medico-legal work. Doctor Tuffley served as an examiner for the Royal Australasian College of Surgeons in orthopaedic surgery for eight years before assuming governance roles with the Australian Orthopaedic Association including chairing the AOA’s Digital Imaging Committee and the AOA’s Audit and Risk Committee. He became President of the Australian Orthopaedic Association in October 2014. Doctor Tuffley is married with four adult children. His non-orthopaedic interests include cycling, fishing, and sailing. British Orthopaedic Association Mr Colin Howie Trained at Edinburgh and Exeter and was originally appointed a consultant in Inverness. In 1995 moved to Edinburgh (Princess Margaret Rose Orthopaedic Hospital) to continue the renowned arthritis service and to provide a specialist arthroplasty service. Clinical and research interests are focused on joint replacement, revision joint replacement (hip, knee, ankle, shoulder and elbow) and arthritis surgery in general with a special interest in patient derived outcomes. BOA President: 2014-2015 Canadian Orthopaedic Association Dr Robin Richards Robin Richards was born in Guelph, Ontario and attended McMaster University where he obtained BA (Geography - 1973) and MD (1976) degrees. He completed the University of Toronto Postgraduate Training Program in Orthopaedic Surgery and received his FRCSC in 1982. Dr. Richards was a MRC Fellow at Duke University in 1983 and joined the staff of St. Michael’s Hospital in 1984 where he was Head of the Division of Orthopaedic Surgery, Medical Director of the Mobility Program, Chair of the Medical Advisory Committee and served on the Board. Dr. Richards was Head of the Department of Surgery and Co-Director of the Operating Room and Related Services at Sunnybrook HSC from 2001 - 2012. Dr. Richards maintains an active surgical practice focusing on arthroplasty of the shoulder and elbow, upper extremity reconstruction following trauma, soft tissue procedures to control joint instabilities in the upper extremity and the surgical treatment of irreparable brachial plexus injury. Dr. Richards is Vice Chair – Clinical of the University of Toronto Department of Surgery, Chair of the Awards and Internal Evaluation Committees, Medical Director of the Sunnybrook Working Condition Program and Sunnybrook Centre for Independent Living, Past Secretary-Treasurer of the American Shoulder and Elbow Surgeons and Past Deputy Editor for the Upper Extremity of the Journal of Bone and Joint Surgery (American). Dr. Richards is a member of the Ontario, Canadian and American Orthopaedic Associations and is Past-President of the Ontario Orthopaedic Association and Past-Secretary of the Canadian Orthopaedic Association. Dr. Richards enjoys stream fishing, cycling and wind-surfing. Dr. Richards has four children ages 26 – 34 and one grandchild. 8 New Zealand Orthopaedic Association Mr Brett Krause Brett Krause is a graduate in Science and Medicine from the University of Otago. After completing his Orthopaedic training in New Zealand and receiving the FRACS, he undertook post fellowship training at the RNOH in London.. Passing the FRCS(Ed Orth), he returned to a consultant post in Lower Hutt, outside Wellington. He has a general Orthopaedic practice with an interest in Shoulder Surgery. He has been President of the NZ Shoulder and Elbow Society, Secretary of the NZOA, Chairman of the Wellington Medico-legal society and currently sits on the Health and Disability Disciplinary Tribunal. He is also Clinical Senior Lecturer, Otago University Clinical School in Wellington and an examiner for the Royal Australasian College of Surgeons. He is the current President of the New Zealand Orthopaedic Association. Invited Guest Speakers Ian Harding – Ian Harding studied and did his Orthopaedic training in Oxford before doing his fellowship with Daniel Chopin at the Institut Calot in France. He returned to the UK in 2004 and commenced his Consultant post in Bristol where he is also a honorary lecturer. In 2012 he was the SRS travelling fellow. Research interests include the basic science of disc degeneration, the sagittal plane and adult spinal deformity on which he has extensively published. Peter Choong – Prof. Peter Choong is the Sir Hugh Devine Chair of Surgery, and Head of Department of Surgery University of Melbourne at St. Vincent’s hospital Melbourne. Prof. Choong is also the Director of Orthopaedics at St. Vincent’s Hospital, and Chair of the Bone and Soft Tissue Sarcoma Service at the Peter MacCallum Cancer Centre. Prof Choong completed sub-specialty training at the Mayo Clinic and the University Hospital in Lund Sweden. Prof. Choong’s research (NHMRC, ARC, RACS, AOA, Cancer Australia) focuses on 3 areas: (i) outcomes of arthritis surgery, (ii) the molecular regulation of bone tumours and (iii) advanced limb reconstruction. Prof Choong is widely published with over 260 peer-reviewed articles (H score 37) in these areas. Rodrigo Pesantez – Studied medicine and did his residency in Colegio Mayor de Nuestra Señora del Rosario in Bogotá. After that he spent 3 months in Boston (Massachusetts General Hospital under Dr William Harris supervision) and 3 months in New York (Hospital for Special Surgery under Dr Eduardo Salvati Supervision), 5 months in Sacramento, California at UC Davis under the supervision of Dr Michael W. Chapman, Steve Olson and David Mohering. Then he did a 3 month AO Trauma Fellowship at Universitatspital in Zürich under Prof Otmar Trentz supervision. He did a 3 months rotation in the Lindehonf Medical Center in Bern with Prof Diego Fernández. Dr Pesantez has worked at Fundación Santa Fe since 1995, and started its residency program in 2000 and directed it until 2008. He was the president of the AOAA Colombian Chapter from 1996 to 2002 and the President of the Colombian Orthopaedic Trauma Chapter of the Colombian Orthopaedic Society from 1997 to 1999. Dr Pesantez has been involved in the AO since 1995 and was an Elected Trustee from 2004 to 2011, a member of the AO Trauma Research Commission from 2007 to 2012, and a member of the Principles and Advances Task Force since 2009 to 2012, currently a member of the Lower Extremity Expert Group.
