The California Dental Association (CDA) Foundation helps
us obtain happiness for a lifetime by allowing our collective
contributions to serve a greater number of people than we
would be able to help individually.
As dentists, we know that oral health is not just about teeth.
Oral health is about quality of life, self-esteem, dignity and
changing lives.
The CDA Foundation represents our association and us as
members in making a difference in California. Our tagline
“Creating Smiles…Changing Lives” expresses succinctly just
what the Foundation does through the three main focus
areas: the Student Loan Repayment Grant Program, the
CDA Foundation Grant Program, and Direct Programs for
the Underserved. All three of these initiatives align with the
CDA Foundation’s mission to improve the oral health of all
Californians by supporting the dental profession and its
efforts to meet community needs. They include programs
such as the Pediatric Oral Health Access Program (POHAP)
and the new Geriatric Oral Health Access Program (GOHAP),

which have touched people throughout the state. The CDA
Foundation has also been enormously successful in getting
water fluoridated to help prevent dental disease — to date
more than 26 million Californians are receiving fluoridated
water because of the Foundation’s efforts.
To keep these and future programs available to those who
need them most in California, the Foundation depends
on charitable donations. Every contribution helps the
Foundation to leverage that money to benefit greater
numbers of people. Eighty cents of every dollar donated
goes directly to Foundation programs and services.
I am asking you to join me in continuing to support the
CDA Foundation and help sustain present and future
access to care programs. I can’t think of many other ways
to have happiness for a lifetime!

Nam Hoang, DDS
This unique grant program is available
to recently graduated dentists and
awards up to $105,000 over a three-year
period toward dental student debt.
In exchange, these dentists serve for
three years in a California practice
setting providing oral health services to
undeserved populations. The Student
Loan Repayment Grant Program is
one of the Foundation’s strategies to
encourage recently graduated dentists
to begin working in, and providing
dental services to, underserved
communities in California and keep
them caring for these populations
throughout their professional career.
In 2007, the CDA Foundation proudly awarded this grant
to two deserving recipients, distributing $70,000 to the
selected recipients’ financial institutions to apply toward
their dental student loans.
One of the recipients, Nam Hoang, DDS, says he read
about the grant in the CDA Update, the association’s
monthly newsmagazine. “I was already working at the
Chapa-De Indian Health Program, Inc., in Auburn, when
I read the article,” recalls Dr. Hoang. “I knew I wanted to
stay here, but because of my student debt load, I questioned how much longer I would be able to.” Chapa-De
serves a large population of local Native Americans
and underserved individuals for whom Dr. Hoang has
much compassion.
Having come from a family of 11 from Vietnam, Dr. Hoang
fully understands the challenges of providing health and
dental care for a family. “I care deeply for my patients
and insist they be treated with respect and dignity,” said
Dr. Haong “It is so fulfilling to me to see my patients smile.
That’s why I chose this profession — to serve others. The
CDA Foundation Student Loan Repayment Grant made
it possible.”
Dr. James Forester, the other 2007 recipient, serves at
La Clinica de Tolosa in Paso Robles, specializing in pediatric dentistry and caring for underserved children in the
area. Both Drs. Forester and Hoang are committed to
making a lasting difference in their communities.
The CDA Foundation Grant Program supports efforts to respond to community oral health needs by providing grants to
nonprofit dental organizations that increase access to dental care for underserved populations in California, including oral
health education and preventive and restorative treatments.

Grant Program
The CDA Foundation received grant requests totaling
more than $1.1 million in 2007. Although we cannot yet
fulfill all these requests, the CDA Foundation was able to
provide funding in 2007 for the following nine nonprofit
dental organizations’ oral health projects that have
served thousands of patients:

Keeping the Doors Open
One of the 2007 grant recipients, the University of
California, San Diego (UCSD) Student-Run Free Dental
Clinic, reported that the funding provided by the CDA
Foundation kept the doors open to one of their three
clinic sites that otherwise would have been forced
to close. The significant community impact from this
funding allowed the clinic site to remain open and
provide dental care to more than 500 underserved
individuals for the year.
Direct
Programs
for the
Underserved
Access to proper oral health care is
essential for all Californians. Unfortunately, many families, including
children and the elderly fall between
the cracks and do not receive the
care they deserve and need. The CDA
Foundation has implemented several
programs that directly target these
underserved populations.
POHAP is a free training program for general dentists to increase their skill and comfort level in treating young children,
including children with physical and/or developmental disabilities. POHAP enlists general dentists who practice in
underserved areas, treat uninsured patients and/or currently accept publicly funded dental insurance programs.
Pediatric Oral Health Access Program
In 2007, the Pediatric Oral Health Access Program (POHAP) trained 170 general
dentists from underserved areas in 30 counties across California. Two of the
dentists, Drs. Candice Yee and Andrew Chen, quickly jumped at the chance to
participate in the free training to better serve patients in their respective community clinics. Having met at University of the Pacific Arthur A. Dugoni School
of Dentistry, both Drs. Yee and Chen had a heart for underserved populations
and apparently, each other. They are now married, living in Chico and practicing
at separate community clinics. Dr. Yee practices at the Northern Valley Indian
Health Clinic, which treats a large population of underserved children, elderly
and migrant families. Dr. Chen practices at Chico Family Dentistry, which serves
a similar population, with designated days specifically set aside for migrant
children. “The POHAP training has been invaluable to me and my patients. In
the year since I had the training, I have been able to treat about 40 percent more
kids in my clinic rather than sending them to a specialist in Sacramento or the Bay
Area,” said Dr. Chen. Dr. Yee says since the training, she has been able to see and
treat 50 percent more children at the Northern Valley Indian Health Clinic. Both
doctors expressed their gratitude to the CDA Foundation for offering POHAP
and wholeheartedly encourage other dentists to participate.
Year-end data reflects that in 2007, POHAP dentists have increased access to
dental care by seeing more than 9,200 new patients and more than 200 children
with special health care needs. Additionally, more than 960 children received free
restorative dental care. Since inception, POHAP dentists have treated more than
16,000 children. The California Endowment and L.A. Care Health Plan provided
funding for the Pediatric Oral Health Access Program.

Geriatric Oral Health Access Program
Are you aware that up to 78 percent of the 1.65 million seniors over the age of
65 in long-term care facilities in the United States have untreated caries? And
that more than 40 percent have periodontal disease?
To address this epidemic, the CDA Foundation has embarked on the Geriatric
Oral Health Access Program (GOHAP). This pilot project is a partnership with the
San Mateo County Dental Society and California Dental Hygienists’ Association
to develop solutions to complex issues confronting seniors in long-term care
facilities. The project partners with local dentists to bring Registered Dental
Hygienists in Alternative Practice (RDHAP) into long-term care facilities to perform oral health assessments and develop a daily oral hygiene regimen for
residents. In support, The San Francisco Foundation awarded a three-year grant
to the CDA Foundation to implement GOHAP. Additional funding has been
awarded from the ADA Foundation as well as Mills-Peninsula Health Services.
2007 saw much progress toward successful implementation of this innovative
pilot project, including securing a facility — Brookeside Skilled Nursing Hospital in San Mateo. The CDA Foundation and its partners have high hopes for
the residents of the Brookside Skilled Nursing Hospital and, in the future, for
seniors across the state and perhaps the nation. “This program has enormous
implications,” says Tippy Irwin, executive director of the Ombudsman Services
of San Mateo County and member of the GOHAP Advisory Committee. “The
Foundation’s good name brought the project credibility.”
The long-term goal of this program is to increase dental care to this underserved population. “Brookside is very excited to partner with the CDA
Foundation’s GOHAP to provide our residents with increased dental services
and our employees with more training in dealing with the dental needs of
long-term care facility residents,” says Brookside Administrator Greg Bogart.
Migrant Head Start
Training Program
In partnership with the California Head Start Association,
the CDA Foundation implemented an oral health-training
program intended to educate staff at Migrant and Seasonal Head Start programs on the importance of oral health
and how to lower the incidence of caries. Migrant and
Seasonal Head Start programs work primarily with Spanish speaking low-income families who earn their income
mostly from agricultural labor.
The CDA Foundation worked in collaboration with the
California Head Start Association to expand the impact
of this far-reaching program. Migrant and Seasonal Head
Start centers in California provide care to more than
5,100 children up to five years of age. These 11 Migrant
and Seasonal Head Start programs employ 1,193 education services staff and 133 family service workers who
are in contact with the program’s 5,100 children and
4,000 families.
This program was made possible through funding provided by the Aetna Foundation, awarded to the CDA
Foundation to advance its commitment to increase
access to care.
Mobile Dental
Grant Program
The CDA Foundation’s Mobile Dental Grant Program is
one of several initiatives designed to increase access to
care to the state’s most vulnerable populations, including supporting the Veterans Administration’s (VA) Stand
Down events. The events are typically one to three days
and provide homeless veterans with services such as food,
shelter, clothing, health screenings, dental treatment, VA
and Social Security benefits counseling, and referrals to
other necessary services, such as housing, employment
and substance abuse treatment.
The CDA Foundation supported free dental treatment for
105 homeless veterans by providing a mobile dental clinic
at three 2007 VA Stand Down events.
This is the fourth year that the CDA Foundation has been
able to provide the Mobile Dental Grant Program, thanks
to funding by The Dentists Insurance Company.
First Smiles:
Dental Health Begins at Birth
2007 marks First Smiles’ fourth year, with the following accomplishments
to date:
•
More than 12,000 dental professionals and 4,000 medical professionals
participated through live lecture or web-based learning. In addition,
28 trainings were held for 438 Head Start staff and 19 trainings for 349
Women, Infants, and Children (WIC) program staff.

The First Smiles Web site First5OralHealth.org, an online clearinghouse,
has been visited by nearly a half million viewers and is accessed by an
average of 20,000 visitors each month. It offers the latest science and
research on Early Childhood Caries (ECC) prevention, a resource guide
for parents and caregivers, and a variety of on-demand continuing
education opportunities.

More than 1 million parent education brochures titled “Healthy Teeth
Begin at Birth” have been distributed in the 10 languages most
commonly needed in California.

The publication of manuscripts addressing the complex issues in
implementing Caries Management by Risk Assessment (CAMBRA) were
featured in peer-reviewed and scientific oral health media such as the
October and November issues of the Journal of the California Dental
Association. Two companion web casts, “CAMBRA: A Team Approach,”
and “The Practitioner’s Guide to CAMBRA,” were facilitated by
the Journal’s co-editors and authors and were presented in January
and December 2007. Archives of both webcasts are available at
First5OralHealth.org. Richard Schoenbaum, DDS, of Culver City, states
that CAMBRA has great value, but that it is difficult to implement
without full staff participation. He recognizes that the CAMBRA science
is evolving, and understands that acceptance and implementation is a
process rather than an “on-off” activation. He has been working toward
fully implementing this with his staff for about two years and found the
web casts “very helpful in bringing the staff to the level where they can
actively promote and implement CAMBRA protocols.”
The First Smiles Education and Training Project is the result of a four-year, $7 million grant from First 5 California. The goal
of this project is to reduce the incidence of Early Childhood Caries (ECC) affecting children ages 0–5, including those with
disabilities and other special needs. First Smiles prepares dental, medical, and early childhood education professionals
to work with parents and caregivers to promote oral health, establish a dental home by age 1, and provide guidance in
preventing ECC.
Fluoridation
The CDA Foundation strongly supports water fluoridation programs as one of
the most effective preventive oral health measures available to a community.
In 1999, the CDA Foundation joined a statewide collaboration called the California Fluoridation Workgroup. The goal of the coalition, now the California
Fluoridation Advisory Council, is to develop statewide fluoridation programs
using funds provided from The California Endowment and other local funding
partners to increase the number of Californians receiving fluoridated water.
Tremendous strides were made in 2007 to expand community water fluoridation to approximately 18 million additional residents in five California counties.
The Metropolitan Water District of Southern California completed its capital
improvements at five treatment plants to increase the fluoride content of its
treated water within Los Angeles, Orange, Riverside, San Diego and Ventura
counties. The Metropolitan Water District received funds for fluoridation from
the CDA Foundation as a project sponsored by the California Fluoridation
Advisory Council.
The CDA Foundation has successfully facilitated funding for fluoridation,
serving more than 26 million Californians. Even with that success, more work
is needed to reach the Healthy People 2010 goal of 75 percent of all Californians receiving fluoridated water.
Grants and Scholarships
The CDA Foundation believes that supporting student and faculty achievement
helps provide opportunities to succeed in their academic programs and increase
career opportunities in both traditional and nontraditional practice settings,
such as clinics in underserved communities.
Dental Hygiene
Education Program Grant
The Dental Hygiene Education Program Grant supports
dental societies, community organizations, or educational institutions interested in expanding current dental
hygiene programs, establishing a new dental hygiene
education program, or creating innovative methods of
providing dental hygiene education.
Research has identified that the market demand for dental
hygienists in San Mateo is high, indicating there is a great
need to increase the number of graduating hygienists to
serve the area. In 2007, the CDA Foundation supported
the College of San Mateo by providing $50,000 toward the
establishment of a new dental hygiene program that will
be located in the future (90,000 sq. ft.) Wellness and Allied
Health Building. Dental assisting and dental hygiene will
share part of the facilities located on the third floor. The impact of the CDA Foundation funding is the
increase in the number of individuals who can enroll in
the accredited dental hygiene education program to
60 students, beginning in fall 2008. By supporting the
College of San Mateo’s new dental hygiene program, the
CDA Foundation has helped meet the need for dental
hygienists in the San Mateo area. This improvement
contributes to our mission to improve the oral health of
all Californians by supporting the dental profession and
its efforts to meet community needs.
Maurice Amado Dental
Assisting Scholarship Program
Allied Dental Health
Student Scholarship Program
One of the ways that the CDA Foundation supports the
dental profession and helps to ensure a sustainable oral
health workforce is through educational scholarships like
the Allied Dental Health Student Scholarship Program.
This scholarship program is in partnership with local
dental societies. It is available to students enrolled in
state-approved dental hygiene, dental assisting, or dental lab tech programs, and provides awards to selected
individuals toward completing their education.
In partnership with local dental societies, the CDA
Foundation supported student achievement by providing
scholarship awards to 44 recipients through this program
in 2007.

Dental Hygiene Program
The CDA Foundation partnered with the Maurice Amado
Foundation to establish a dental assisting fund that offers
scholarships to students. The program was established by
Drs. Mark and Sam Tarica, on behalf of their great uncle’s
foundation, the Maurice Amado Foundation. The program
is available to students enrolled in state-approved dental
assisting programs, and provides financial awards annually
to selected individuals toward completing their academic
education. Recipient selection is based on individual
demonstration of financial need, volunteer community
service/leadership, and academic achievement.
The CDA Foundation awarded scholarships to 10 dental
assisting student recipients in 2007. By supporting student
achievement through the Maurice Amado Dental Assisting
Scholarship Program, the CDA Foundation has made a
significant impact on the recipients of this award.

Dental Assisting Program
Latinos for Dental
Careers Scholarship Fund
In 2007, this advised fund awarded scholarships to 14 students of Latino descent.
There are minimal resources available and positive role models to mentor in the
Latino community. The CDA Foundation believes that supporting opportunities
for ethnically, and often socio-economically, disadvantaged individuals in pursuit
of their studies in dentistry will pay off in the return of these individuals contributing back to underserved communities once they enter the dental profession. “To
me, being of Latino descent signifies never forgetting where one comes from…it is
my sincere hope that with my dental degree I can contribute to these communities
and work with others to improve them.”

The Latinos for Dental Careers (LDC) organization, whose mission is to increase
the number of Latinos in the dental profession, partnered with the CDA Foundation
to establish an Advised Fund that provides financial support through scholarship
awards to students of Latino descent studying in the field of dentistry. The CDA
Foundation offers the LDC Scholarship Program to California dental students,
dental hygiene students and international dental students of Latino descent and
provides awards annually to selected recipients.
Dental Materials and
Supplies Grant Program
The CDA Foundation Dental Materials and Supplies Grant Program works
in concert with the Henry Schein Cares Program to provide dental materials
and supplies to nonprofit dental organizations. This grant program provides
in-kind donations of dental supplies to each of its recipients in the amount of
$5,000–$25,000 of goods per year for two years. Since this program’s inception, Henry Schein Cares has provided in-kind donations to more than 65
nonprofit dental organizations.
The CDA Foundation and Henry Schein Cares awarded 20 California nonprofit dental organizations this in-kind donation in 2007. The grant program
makes a profound impact on recipient organizations, donating a broad selection of dental products and materials to help enhance the quality of dental
care to underserved individuals and families in California communities. The
in-kind donations are estimated at a cumulative value of up to $2 million per
year to the receiving California organizations. Ultimately, recipients of this
grant are provided with the tools they need to increase access to care within
their communities.
CDA Foundation Relief Grant Program
When life’s difficulties impose a financial burden, dental professionals and their
families can turn to the CDA Foundation for assistance. The CDA Foundation
Relief Grant provides both emergency, and long-term financial support to California nonprofit dental organizations and dentists in the event of a catastrophic
disaster such as a flood, fire, or a dentist’s illness, disability or death. This grant is
intended to help reestablish the delivery of dental care through nonprofit dental
organizations in a community affected by a disaster, or help dentists and their
families impacted by a disaster with emergency food, clothing and shelter costs.
The 2007 Southern California fires are an example of the Foundation’s readiness to provide relief to California nonprofit dental organizations and dentists.
Emergency relief assistance was offered to California dentists and their family
members, dental staff and component staff impacted by the disaster, as well as
affected nonprofit dental clinics.
In 2007, the CDA Foundation also provided financial assistance to dentists facing
situations of undue financial burden caused by illness or disability. Since 2001,
more than $195,000 in relief assistance has been provided to eligible dentists
throughout California.
Dr. Arthur A. Dugoni
Faculty Award
Susan Hyde, DDS, MPH, PhD, UCSF School of Dentistry
faculty member, was the 2007 CDA Foundation Dr. Arthur
A. Dugoni Faculty Award recipient. Dr. Hyde’s historical
trail of accomplishments in dental education and
organized dentistry, including her display of exceptional
leadership, collaboration, and compassion, over the years
proves her to be deserving of this faculty award.
Dr. Hyde is assistant adjunct professor at the University of
California, San Francisco School of Dentistry, Department
of Preventive and Restorative Dental Sciences, a member
of the UCSF Center to Address Disparities in Children’s
Oral Health and is the Project Director for its Acceptability
Study of Preventive Interventions for Reducing Early
Childhood Caries.
Colleagues describe her as “an exceptional, awardwinning teacher and mentor” who “has contributed
extensively to the new dental school curriculum and
clinical activities.”
CDA established the Dr. Arthur A. Dugoni Faculty Award
in 2004 in recognition of Dr. Dugoni’s accomplishments
in dental education and organized dentistry. The purpose
of this award is to recognize and support individual faculty
for their exceptional leadership, innovation, collaboration, compassion, philanthropic spirit, and integrity in
dental education.
