ANNUAL GENERAL REPORT Our Mission As a leader in community health services and programs, we empower residents to improve their physical, emotional and social well-being. We are dedicated to building a strong and healthy community "Your Wellness, Our Mission". Our Vision We are instrumental in the creation of a caring and respecting community in which all people have dignity and an optimum quality of life with access to a broad range of programs and services. Since our founding in 1982 by volunteers, we have proudly worked with our residents to build a strong and health community. Our services include family medicine, social work, counseling, diabetes education, Hepatitis C education and outreach, a GAIN (Geriatric Assessment & Intervention Network) Team and a variety of health promotion, Aboriginal and community development programs. All of our services and programs are free (except for a nominal registration fee for our Very Important Kids Day Camp). Oshawa Community Health Centre utilizes a team of supportive and skilled professionals including child and youth workers, doctors, social workers, counsellors, nurse practitioners, diabetes educators, and health promoters to deliver services. Our centre is unique for the reason that we are responsive to the health needs as defined by our community. Oshawa Community Health Centre is fully accredited by the Canadian Centre for Accreditation through 2015. Message From the Board Chair & Executive Director Oshawa Community Health Centre (OCHC) is now two years into operationalizing our five year strategic plan. We continue to focus in two broad arenas aligned to achieving our strategic vision: 1. At the service level, ensure that dedicated efforts continue to focus on providing responsive, effective, quality care to those in our community that need it most; those most negatively affected by determinants of health. Over the past year OCHC has continued to reach out and build our capacity to provide supports and services to our growing Aboriginal community in Durham, a community that is significantly more likely to be negatively affected by the determinants of health. We are working as the lead agency on two successful Ontario Trillium Fund Grants, the first one consists of collaborating on the creation of an Aboriginal Secretariat and the second one; the Youth Opportunities Fund, will focus on strengthening programming and mental health supports for Aboriginal youth living in and around Durham Region. In addition, with support from the Central East Local Health Integration and many new partners, OCHC established the first Geriatric Assessment and Intervention Network (GAIN) Community Team in south Durham to support the frail elderly in our community. 2. At the strategic and system leadership level, OCHC spearheaded an intensive integration planning process between OCHC and The Youth Centre, proposing a value added/ integrated CHC service delivery model across the communities of Ajax, Pickering, Whitby and Oshawa. A robust integration plan, including opportunities assessment, risk mitigation analysis, and due diligence review was developed in partnership between the two CHC’s and supported by the Central East Local Health Integration Network. Recognizing the clear potential to enhance services for all those in-need across our communities, OCHC’s Board voted in favour of a full merger of the two organizations. Although the two Board’s decisions were not united, OCHC will actively use the planning information gathered throughout the integration as a solid blueprint for our growth strategy. We are currently undertaking a Strategic Planning refresh to ensure alignment in our goals over the next 3 years. In closing, we would like to sincerely thank all of our board, staff, volunteers, partners and supporters for believing in, and committing to, the mission and vision of our centre. We are excited about our immediate and long term future and look forward to working with our stakeholders on bringing a bold vision to reality.
