Status relates to the changes that can and cannot be made to a dictation or document. 8 Dictation and Message Tabs IntelliScript IntelliScript is the server located at your institution that records high-quality, digital audio dictations. Clinicians call IntelliScript and enter their ID and other identifying information such as patient medical record number and work type. Using IntelliScript, clinicians can dictate, review, and edit their dictations. AutoScript AutoScript performs speech recognition for each dictation. AutoScript learns how a physician speaks and creates a voice recognized model for each clinician. Models are then used to create document draft of dictations. EditScript Server The EditScript Server manages the life cycle of a dictation as it transitions from an audio recording (including demographic information), to a draft document (generated by AutoScript), to an item in a prioritized work queue, to a completed and distributed document. EditScript MT EditScript MT is the software that eScriptionists (MTs) use to transcribe or to edit reports. eScriptionists download dictations, perform editing or transcription, and upload completed work using EditScript MT. EMon EMon (eScription Monitoring) is the administrative console that provides visibility into the dictation workflow in eScription. This software is installed locally on the user's PC. NetScript NetScript is the web portal www.escription.com where clinicians search for, modify and sign dictations. Transcriptionists and administrators also have access to NetScript to search for and review other dictations. 2011 EditScript for Administrators All rights reserved. Overview of eScription and EMon 9 Overview of EMon EMon is the administrative console that provides visibility into the dictation workflow in eScription. Primarily, you will use EMon to find dictations, modify dictations and assign them as needed. You can also view dictation history to facilitate troubleshooting. This guide focuses on information related to these functions only. EMon is organized by Tabs located across the top of the screen and by Tools menu options. Tabs Tools EMon Tabs User tabs eScriptionist – manage MT accounts. Clinician – manage provider accounts and contacts required for CCs. Administrator – manage EMon user accounts. Dictation tabs Dictation – search for and change the properties of dictations. Message – manage dictations on hold or pended. Distribution – search for, manage and troubleshoot distribution. 10 Dictation and Message Tabs EMon Tools EMon Tools help you understand how dictations flow through eScription. Security Groups – define who has access to which dictations. Distribution Rules – manage distribution of completed reports. Validation Rules – define when a dictation is automatically pended. MT Instructions – create messages for MTs working in EditScript MT (see Appendix for more information). Turnaround Time Rules – wizard for automatically defining dictation priority. Templates Distribution – define templates for text, print and fax distribution. Normals – manage normals (pull templates). Application Features – turn on/off new functionality Options – configure EMon behavior on the local PC. Dictation and Message Tabs 13 Searching for Dictations When you log into EMon, it defaults to the Dictation tab. Define your search for finding dictations by using one or many of the fields shown. Click Search throughout the day to refresh the data Dictation ID – a system generated document number for the report; search for more than one document by entering the exact ID of each dictation, separated by a comma or a space Confirmation number – given to the Clinician at the end of recording the dictation; similar to a job number Dictation Period – specify a period such as yesterday or last week; or identify a specific date or date range Status – drop down menu shows all possible statuses; the actions that you can perform on a dictation depends on the status of the dictation Do not modify dictations that are in a status of Editing, Transcribing, Checked out for Editing or Checked out for Transcription. These documents are on the MT's PC, ready to be worked on. Ready Statuses Ready for Recognition – recorded dictation, waiting to be voice recognized Checked Out Statuses Checked out for QA – pended dictation being corrected in EditScript MT. Ready for Transcription – standard dictation not yet downloaded by an MT Checked out for Transcription – standard dictation in an MT’s work queue Ready for Editing – editable draft not yet downloaded by an MT Checked out for Editing – editable draft in an MT’s work queue Active Statuses Recognizing – audio file being speech recognized Transcribing – standard dictation currently being transcribed by an MT. Editing – draft currently being edited by an MT Pending – uploaded by MT and now on hold See the Appendix for the definitions of the Aggregate Statuses 14 Dictation and Message Tabs Work type – hold the CTRL key to select more than one work type Business Entity – hold the CTRL key to select more than one business entity Clinician name – enter the entire or partial last name, comma full or partial first name Medical Record – MRN is a unique identifier; enter the exact number when searching including any prefixes or suffixes (e.g. 1234BRC) Patient Account – visit or encounter; enter the exact number when searching including prefixes or suffixes (e.g. W610000123) When searching on any numeric value, such as Dictation ID or MRN, you must enter the exact value. You can search by more than one value by adding a space or comma between each numeric value. Each time you want to search for different dictation(s), click Clear and enter your new search. Clear removes the search criteria only; it does not clear the dictations. 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 15 Sorting Search Results To organize the search results, click on the column title. 1. Click the column heading to sort by ascending order. Click the column heading a second time to sort by descending order. Click a third time to sort by default order. Click any column title to Sort by that field Number of dictation found in the search 2. To sort by a second column title, press CTRL and click a second column title. Reposition the columns by dragging the column heading to the left or right. This layout is saved for EMon on this PC only. Action Buttons Located at the bottom of the Dictation Tab, action buttons are active when a dictation is highlighted. Depending on the status of the highlighted dictation, different buttons are active. For example, if a dictation is in a status of Ready for Transcription, the Assign button is active. Properties button - used to modify the dictation properties including CCs and text. You can also double click the highlighted dictation to view the dictation properties. History button – view pends, transmissions/distributions, audit trail and document versions. This is explained more fully at the end of this chapter. Print button – local print to the printer connected to your PC. Assign button – click this button or right click highlighted dictation to assign a dictation. Distribute button – used for ad hoc distribution by customers using PrintScript and/or Electronic Signature in eScription. Used to distribute to institution (your EMR) if you update one or many dictations from pending to unsigned/transcribed from the Dictation Tab. 16 Dictation and Message Tabs Saving a Search If a search is done on a daily basis to manage work flow, you can save the search as follows: 1. Enter the search parameters. Save Search Criteria 2. Right click Save Search Criteria. 3. Enter the Search Name. 4. Click OK. 5. Click OK. Right click in the top of the Dictation Tab to Retrieve or Delete a saved search. 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 17 Updating Dictation Properties From the Dictation Tab you change the dictation properties of one or many dictations. 1. Click the dictation to highlight it. Hold the CTRL key to select more than one dictation. 2. Right click on any one of the highlighted items and select Update dictation properties. Here you can change Priority and/or WorkStatus. 18 Dictation and Message Tabs When you change the WorkStatus to Pending, you can also add a pending reason if needed. Pending reasons are associated with functionality that pends documents with pre-defined reasons. This is discussed later in this chapter. If you unpend dictations in this manner from the Dictation tab, you must then select the dictations for distribution by clicking the Distribute button. . 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 19 Changing Dictation Properties and Saving Changes When you click the Properties button (or double click a specific dictation), the Properties for dictation dialog opens. Here you can change dictation properties, patient information, CCs and/or institution properties. Although it is easy to change the properties of a dictation, the status of the dictation defines what can and cannot be changed. Also, If you are working with NTS or another MTSO, that relationship may define who can change which fields and when. Select value from the drop down menu To change Patient Information Save changes on the Properties screen Note that the Priority number defines the turnaround time of the dictation. For dictations not yet worked on by an MT, you can change the priority number to a lower numeric value. By doing so, you move the dictation ahead in the queue. Patient Information Buttons Change button – used to change patient information to other data that is in your ADT (e.g. to select a patient if the current patient is Unknown) 20 Dictation and Message Tabs Edit Data button – used to edit ADT for situations such as misspelling of patient names or to add a visit date. Most changes made using this button will change all dictations for that patient (e.g. DOB, spelling of name). Most often, this function is performed in your registration system. Advanced button – used if there is a new patient, new patient visit or new order. (e.g. T , temporary visit or MRN). You can add a patient, a visit or an order. You can change relationships such as Dictation and patient, Dictation and patient visit and Dictation and order. Most often, this function is performed in your registration system. Making a Dictation a STAT 1. Open the properties window 2. Click the Priority Drop Down 3. Select 1 (or 2) 4. Click Save High Priority configuration is a setting that can be set as a default. Most institutions choose to have 1 or 2 as STATs. 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 21 Changing Patient Information - Change Button 1. In the Properties for Dictation screen, click the Change button (under Patient Information) to search for a new Patient Name, Medical Record Number, Account Number or Order Number. If there is an Account Number/Visit, that value will populate automatically for a search. If there is no Account Number/Visit and there is a MRN, then that value will populate automatically for a search. Clear to define a different search 2. Use the Clear button to define a new search and enter new search criteria. 3. Click the Search button to retrieve all visits (that are in EMon). 4. Highlight the correct visit and click the OK button. If the correct visit is not here, contact your manager. You may have to have the ADT resent or use the Advanced button. If using the Advanced button, be sure to enter the visit exactly as it appears in your ADT. 22 Dictation and Message Tabs Adding or Changing a CC 1. To modify the CC, click the Change button (next to CC). Modify CC 2. On the Change CC screen, enter the last name of the Clinician (or partial last name) and click Search. Selected clinician posts here To add a New Contact To delete CC To remove the selected clinician, highlight the name and click Delete. 24 Dictation and Message Tabs To add a new CC contact, click the New button.. Enter as much information as you have about the contact. Then click OK to save. 'Add to clinician list' – adds the contact to the database for future use. This is similar to adding a new user in the Clinician Tab which is discussed in the next Chapter. Save changes to Dictation If you are through making changes on the Properties for dictation dialog, click Save. Any changes made in EMon will carry forward into EditScript MT for dictations that have not yet been worked on by an MT. 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 25 Assigning Dictations You can assign dictations that are Ready for Editing or Ready for Transcription. 1. From the Dictation Tab, highlight a dictation. You can assign multiple dictations by using SHIFT or CTRL and clicking the desired dictations. 2. Click Assign (or right click the dictation and click Assign to eScriptionist.) 3. In the Assign dictation to an eScriptionist dialog, enter the first few letters of the eScriptionist account last name or click the drop down arrow, scroll and select the name. 4. Select the priority or leave the default. At most institutions, priority defaults to 1 or 2 for a STAT dictation. Institutions working with an MTSO or NTS usually have a process in place to assign dictations or to move dictations ahead in the queue. 5. Click OK. If you want the MT to work on the assigned STAT immediately, you must notify him or her that there is a STAT in queue. If not, the STAT will download upon upload of the document that she is currently working on. 26 Dictation and Message Tabs Pended Dictations The Message Tab shows all Pended Dictations. The Problem Note shows why the dictation is on hold and not complete. Dictations can pend automatically based on validation rules defined by your institution. MTs can also manually pend dictations and add their own reason such as inconsistencies in the report. For dictations manually pended in EditScript MT, there are two possibilities: Free form text where the MT explains the issue. Pre-defined reasons which the MT selects from a drop down menu. Manually Pending with Free Form Text: If you are working with both in-house MTs and those from an MTSO, it is best to add a prefix to the message identifying your institution or entity (e.g. Inst_reason). Likewise, the MTs from the MTSO will prefix their messages with their acronym, such as MTSO_reason. Furthermore, when entering a pending reason, in-house MTs and those from the MTSO can select the 'owner' of the pending note (e.g. Inst or MTSO). This defines who is responsible. Manually Pending with Pre-Defined Reasons: MTs select pre-defined pending reasons/criteria which are prefixed with the name of her billing group (e.g. MTSO_reason1, Inst_reason2). These reasons/criteria are grouped together into buckets known as Pending Document Tasks. Each bucket is assigned to the responsible security group (In-house or MTSO). Your Institution will use one type of Pending List Management; either free form text or pre-defined reasons and pending document tasks. The latter is generally used by larger institutions that work with an MTSO and need to closely monitor turnaround time. 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 27 Handling Pended Dictations: Pre-Defined Reasons This section explains how to handle pended dictations with pre-defined reasons. See the Appendix for Handling Pended Dictations: Free-Form Text. The steps to unpend a dictation are essentially the same whether using free-form text or pre-defined reasons. Pre-defined pending reasons/criteria are prefixed with the name of the billing group that is primarily responsible for handing the issue (e.g. MTSO_reason1, Inst_reason2). These pre-defined pending reasons/criteria are grouped together in buckets called Pending Document Tasks. These buckets are prioritized and belong to specific billing groups. Based on security, the bucket is assigned to specific reviewers who are responsible for specific types of issues. Users are assigned to security groups whose work queue is limited to documents tagged with one or more specific Pending Document Tasks. In EMon you can see all Pending Document Tasks. When handling pended documents in EditScript MT, you can see only those for your security group. Each billing group can create its own Pending Document Tasks or the institution may chose to configure the workflow for all billing groups. If a document has pending reasons from more than one Pended Document Task (more than one security group), the document routes first to the security group with the higher priority PDT. Pending Document Tasks You can also search for and handle pended dictations from the Dictation Tab. 28 Dictation and Message Tabs Highlight the dictation you want to work on. Click the Respond button (or double click on the dictation) to open the Properties for dictation screen. Click Pending Reasons to see all Problem Notes associated with the highlighted dictation Click Respond to handle the Pended dictation Respond opens the same screen you saw in the Dictation Tab. You can Add, Edit and Resolve each Pending Reason 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 29 If you can solve the problem(s): 1. Make the change and Resolve the pending reason(s) 2. Change the Status to Transcribed (or Unsigned if signing in eScription) 3. Click Save/Distribute Note the Dictation ID of the document so that you can follow-up on it. 2. Change Status to Transcribed (or Unsigned) 1. Solve the problem(s) Resolve 3. Save/Distribute If you need to review the document, click the Edit Document button. With the report visible, you can listen and make changes as needed. You can also listen to the report without opening it by clicking on the Speaker Icon 30 Dictation and Message Tabs When addressing pending issues, you have a few options based on the buttons shown below the Pending Reasons. If you can resolve all pending reasons, you can unpend the dictation without using these buttons. Use them to provide additional information or if required to do so by your institution. Add button – Edit button – 2011 EditScript for Administrators All rights reserved. Dictation and Message Tabs 31 Resolve button – There is no need to Resolve each issue individually. When you change the status from pending, and save those changes, the system will prompt, asking if all pends have been resolved. When all pending issues have been resolved, click Save/Distribute at the bottom of the Properties for dictation screen. 32 Dictation and Message Tabs Save and Distribute If the dictation is in a status from which it can be distributed such as Unsigned or Transcribed, then you can click Save/Distribute. Save and Distribute The Distribute screen provides 2 main options: 1. Distribute the documents subject to the rules that have been set up OR 2. Use these settings and this original indicator (used only when Printing or Signing in eScription) Option 1 follows predefined distribution rules Option 2 for printing from eScription 2011 EditScript for Administrators All rights reserved. Distribute the documents subject to the rules that have been set up: Select: Distribute transcription to institution (EMR) Distribute deficiency information Distribute to Print/Fax/Email/Signature Recipients (only if Printing or Signing in eScription) These distribute according to predefined Distribution rules. 34 Dictation and Message Tabs Option2. Use these settings and this original indicator (only if Printing or Signing in eScription) Make selection: Print at printer – list of printers Fax to this phone number – enter phone number (usually with area code) Send to contacts(s) – Select button to identify contacts/preferences per the Clinician Tab Send to electronic signature queue Click OK. Follow up on the distribution of the unpended report. Search for it in the Dictation Tab to verify that it is unpended. Dictation and Message Tabs 35 Dictation History The History Button is used to: view Pending Reasons. The Comment column shows the issue Resolved by. The Problem Status column shows the original issues as closed and repeats those same resolved issues with a Problem Status of resolved. view or troubleshoot distribution (Transmitted) of the dictation. A status of Completed means that eScription did its part; follow up in your EMR to ensure completion of the distribution. view the Audit Trail which shows everytime that the dictation has been 'touched'. compare Document Versions to see changes made to the dictation. Select two versions by holding the CTRL key, then click Compare Documents (the View Document button dynamically changes when two versions are selected). 36 Dictation and Message Tabs 2011 EditScript for Administrators All rights reserved. The Clinician Tab 39 Clinician Tab The Clinician Tab is used to maintain and add clinicians and contacts. Clinicians are speakers and/or signers in your organization. Contacts are providers and institutions that may not be members of your organization, yet they receive copies of reports. During the Installation Process, Providers were downloaded for you. Post go-live, you will maintain your Provider list by adding new Clinicians as needed. Prior to adding a new clinician, make sure that the clinician does not exist in the system. Search by entering the full or partial last name in the Last Name field. You can also search using the Speaker ID in the Clinician Code field. The system warns you if you add a new clinician who has a name similar to an existing clinician. After you add a new clinician and click OK to save, the follow dialog box appears: 40 Clinician Tab Adding a New Clinician: Click the Add Clinician button at the bottom of the screen (in the Clinician Tab). The Name and Address tab is used for all Clinicians and Contacts. 1. Complete as much information as you can in the Name and Address tab. The following may be required only for speaker and signers. Physician Code _____________________________ Insertion Text 1 _____________________________ Signature contains the signature for the clinician which will appear on all printed and faxed documents. 2. If this 'Clinician' is a contact used for CCs only, click OK. If you are printing and faxing with eScription, you may identify a distribution preference for this entry. You will also have the option to select various distribution options based on Business Entity and/or Work Type. This option appears after you have saved the new entry. 2011 EditScript for Administrators All rights reserved. The Clinician Tab 41 Dictating Clinician Configuration If the clinician is also a dictator, click the Dictating Clinician tab. Enter values in the screen below. Speaker Code is the Speaker ID Status - Ready ensures that the clinician's name is visible in drop down boxes in EMon and EditScript MT. Business Entity - __________________________________. Export Code _____________________________________ Speaker Profile ___________________________________ Specialty –select one that most closely matches that of this provider; this is used for voice recognition Resident Clinician Workflow Role - used for speakers who are not final signers (i.e. PAs). 42 Clinician Tab If the clinician is a signer, click the Groups tab. The Groups tab lists all of the security group access that is available. For all signing clinicians, select the appropriate Group - People who can sign documents. Highlight the Group and click >>>Add. If your Clinicians are signing in eScription, they will be doing so in NetScript. You also need to select a security group that gives them access to NetScript. If your Speakers are not electronically signing in eScription, click the OK button. 2011 EditScript for Administrators All rights reserved. The Clinician Tab 43 For clinicians who sign in eScription, a login and password are required. Click the Security tab, to identify a login and password. To keep it simple, make the Login for NetScript the same as the Login for IntelliScript. Password expiration date – set to yesterday's date (if you want to force the Clinician to change his password). 3. When you are through entering all the required data, click OK. If the new Clinician is a speaker, he will be ready to dictate within 10 minutes. If you are unsure about the data to enter in the Clinician Tab for a new clinician, search for an existing clinician in EMon who has a similar role. Use the set up for the existing clinician as the basis for entering data for the new clinician. 44 Appendix 2011 EditScript for Administrators All rights reserved. Appendix 47 Searching for Reports in NetScript www.escription.com 1. For CUSTOMER LOGIN, enter your institution name. 2. Click Log In. 3. Enter your EMon login and password and click OK (or Enter). 48 Appendix 4. Select Administrators>Search for Reports. The Search for Transcriptions and Dictations page opens. Search for dictations using one or more of the fields shown on this page. 2011 EditScript for Administrators All rights reserved. Appendix 49 5. Enter search criteria such as Patient Last Name. You can search by full or partial last name. For numeric values, you must enter the exact value. After you define your search criteria, you can also filter by using the Full Text Search at the bottom of the page. Here you can enter words and phrases found in the reports. 6. Click Search. Completed Dictation Audio File Only 7. Check the box next to the dictation you want to view. A speaker icon without a check box on the Search Results page means that the dictation has not been transcribed or edited. In this case you can only listen to the incomplete report by clicking the speaker icon. 50 Appendix 8. Click View to open the actual dictation. 9. The dictation opens. View and listen to the dictation as needed. Patient and dictation information are not editable. The body of the report is if you have security permission to edit. The buttons on the bottom of the screen vary based on your security access and how NetScript is set up at your institution. 2011 EditScript for Administrators All rights reserved. Unsigned, Partially Signed, Signed e-signature statuses Unsigned – completed dictation waiting to be signed Partially signed – completed dictation requiring two signatures but having only one signature Signed – completed dictation that has been signed and is final 52 Appendix Handling Pended Dictations: Free-Form Text This section shows how to unpend dictations if the MTs at your institution write free-form text in their pending notes. All pended dictations are in queue in the Message Tab. The Problem Note shows why the dictation is on hold and not complete. Number of pended dictations Filter by Owner Refresh the queue throughout the day Sort by the Problem Note to group dictations with the same messages. You can also search for and handle pended dictations from the Dictation Tab. However, the concept of 'owner' does not apply there. To work on a pended report, double click it or highlight it and press the Respond button. This opens the Properties for dictation screen (as seen in the Dictation Tab). 2011 EditScript for Administrators All rights reserved. Appendix 53 If you can solve the problem: 1. Make the change/solve the problem 2. Change the Status to Transcribed (or Unsigned if signing in eScription) 3. Click Save/Distribute Note the Dictation ID of the document so that you can follow up on it. 2. Change Status to Transcribed (or Unsigned) 1. Make change The message 'grays out' when the status is changed from Pending 3. Save and Distribute If you need to review the document, click the Edit Document button. With the report visible, you can listen and make changes as needed. You can also listen to the report without opening it by clicking on the Speaker Icon 54 Appendix If there are several pending reasons, you can correct the ones for which you are responsible and click Save. If you are using 'owners', the application will prompt you to select an owner to handle the remaining issues. Distribution Options for Completed Dictations are: 1. Distribute the documents subject to the rules that have been set up OR 2. Use these settings and this original indicator (used only when Printing or Signing in eScription) Option 1 follows predefined distribution rules Option 2 for printing from eScription Follow up on the distribution of the unpended report. Appendix 55 Distribution Tab Through the Distribution tab, you can monitor and manage the distribution process across multiple interface types. You can search for documents in any phase of distribution, and you can assign a resolution status to them. You can easily filter and manage these documents by status. Options include canceling scheduled distributions and creating new distributions. The Distribution Tab is great for troubleshooting failed faxes. Media Type selection Distribution Status Action buttons and the right click menu give you access to actions which can be performed: Repeat Distribution – redistributes the original document and the original date. When repeating distribution, you will also be asked to select the fax target. Create New Distribution – create a new distribution of the current version of the report Set High Priority – set fax documents to a high priority for quick delivery Cancel Distribution – cancels the distribution of documents that have been stuck in the distribution process for too long or contains incorrect distribution information (e.g. incorrect fax numbers) History – view the history of a dictation. You can select more than one dictation when using these action buttons. . It is suggested that you sort by Dictation ID. If there is more than one line (failed fax notice) for a specific Dictation ID, select one and Create New Distribution. 56 Appendix MT Instructions MT Instructions allow you to send a message to MTs working in EditScript MT. These instructions are defined by Speaker, Work Type or Business Entity. To create or modify MT Instructions go the Tools Menu option, MT Instructions. Add new MT Instructions 1. Select your parameters by Dictating Clinician, Work Type or Business Entity; click Next. 3. Add your criteria and click Finish. You can determine if the Instructions will automatically be visible to the MTs and you can define how long the message will be deemed new. Go to the eScriptionist Tab > Profile Button > Transcription Tab for the following: Always display Instructions (or have the MT select the instructions from the status bar) Define the number of days that the instructions are marked as New. These are not global settings; they are defined per MT. Delete the MT Instruction when it is no longer needed.
