merchandise must be purchased at a Sony authorized dealer using your SonyCard Credit Card account in order to be eligible for the
purchase credit. Please complete and return in an envelope to the address below within 90 days of the purchase date of your Sony
product from a Sony authorized dealer. Before submitting, please make copies for your future reference.
