at 888-884-2424 for an application.
In addition to payment, please e-mail, mail or fax us your evaluation or appraisal (completed within the last 18 months and
provided by a jeweler) for each piece of jewelry you wish to insure within 10 days of submitting your application. Your appraisal
should note the current retail replacement value, along with any applicable sales tax. If we don’t receive this information within 10
days, your application may be declined.
