INF INITY ORMER Definitive Service to the Chemical, Petrochemical and Refining Industries May 2013 Infinity Maintenance Services at Johann Haltermann Achieves Nine Years of OSHA Recordable-Free Service Infinity Maintenance Service’s crew at Johann Haltermann was recognized at a safety breakfast for achieving nine years OSHA Recordable Free Service. Representatives from Haltermann and Infinity’s La Porte and Corporate offices were present to give the crew the accolades they deserved. Stacy Johnson, Safety and Health Manager for Haltermann asked the crew what they attributed their success to. Everyone responded that safety is not about the individual, it’s about everyone looking out for each other. IN THIS Infinity’s Regional Safety Manager, Rick Voyles stated “We’ve achieved this goal by maintaining heightened safety awareness all day, every day. Safety recognition is nice, but don’t let your guard down. Let’s come back next year and celebrate ten years recordable free.” Issue 2 HBR Safety Excellence Awards Process 3 The Infinity Group and WorleyParsons Offer EPC Capabilities 3 Brazos M&E Concrete Batch Plant 4 Doing The Right Thing: Management Recognition Program 6 Employee Recognition 7 Letters We Like 2 HBR Safety Excellence Awards Process Infinity Construction Services, Brazos M&E, and United Electrical & Instrumentation Selected as Finalists for the 26th Annual HBR Safety Excellence Awards The Houston Business Roundtable and the Houston Area Safety Council announced the finalists for the 26th Annual Safety Excellence Awards at the Network Tailgate Event on Friday, February 5th. Infinity Construction Services and Brazos M&E were announced as finalists in the General Contractor category, and United Electrical & Instrumentation as a finalist in Specialty Contractor Hard Crafts. Since 1988, the Houston Business Roundtable (HBR) has been recognizing Greater Houston Area Industrial Contractors for their outstanding safety performance. This award process has brought Owner Companies in the Gulf Coast together with their Contractors in a cooperative effort to improve onsite safety, eliminate accidents, and most importantly, reduce injuries to employees. The HBR Safety Excellence Award process is a six month long effort and involves hundreds of volunteers and participants. It begins with a request for nominations,. This award is unique in that contractors must be nominated by their clients, the users of their services and there are no self-nominations. Contractor work sites are nominated where the client had verifiable safety performance data for the past calendar year. Nominated contractors are put into award categories based on the type of work they do and in some cases by company size based on total U.S. work hours reported for the previous year. Mentors from the Contractors that achieved “Best in Category” the previous year provide assistance to Contractors in preparing for the submission of information and field audits. Mentors also assist in selecting Contractors to be audited and serve as Co-leaders of the field audit teams. Each nominee submits their OSHA required safety data regarding fatalities, lost workday cases, recordable injuries and illnesses, copies of their safety policies and procedures and documentation of their “best practices”. The initial evaluation is based on total US data and the information submitted is evaluated and scored by a group of Owner representatives and Mentors to determine the Contractors that will be further evaluated in the field. Typically, three or four Contractors in each of seven categories are identified for field auditing. Mentoring Companies do not compete for award recognition while assisting other Contractors in the award process. They will be eligible again the following year. This allows for more Contractors to be recognized and Mentors are fulfilling a significant purpose of the awards program by sharing their best practices and experience with others. Safety audits are conducted by teams of owner and contractor representatives at the Contractor’s work site. Site management and field employees are interviewed to verify that the written safety policies and procedures have been implemented at the work site. This provides an opportunity for team members to see what others are doing to improve safety performance and take the good ideas back to their respective companies and work sites. The audit team leaders with assistance from the Mentors and team members compile the audit scores and determine the level of achievement for each Contractor and identify the Best in each Category. A unique feature of this award process is the requirement that the “Best in Category” Contractors come to a “Best Practices” seminar and present their best practices that led to their excellent achievement. All other companies are encouraged to take these ideas back to their own companies to improve their own performance. Through the sharing of best practices the entire industry improves its safety performance. There is an attitude that is prevalent among the owners and contractors that outstanding safety is not proprietary and must be shared with others. Many of the volunteers that participate in the award program are safety professionals with many years of experience and many have been involved in the HBR Safety Excellence Award process for several years. This is a win-win process for everyone involved. This award process was developed and is maintained by the HBR Safety, Health & Environmental Resource Committee that meets monthly. HBR believes that the Greater Houston Area Contractors involved in this award process have developed world class safety programs and have produced significantly better safety records than any other group of contractors in the world. The 26th Annual Safety Excellence Awards Banquet will take place at Moody Gardens in Galveston on Friday, May 17, 2013. We’ve Expanded Our Capabilities The Infinity Group and WorleyParsons Offer EPC Capabilities In 2010 Infinity Construction Services successfully executed a low BTU project for WorleyParsons in Odessa, Texas. The project earned high praises from WorleyParsons and the idea of putting together WorleyParsons’ EPCM capabilities and Infinity’s construction capabilities was born. Since both Infinity and WorleyParsons had many clients in common the formation of a joint marketing effort in 2012 was relatively painless. To date the reception has been very positive. Several clients have shown serious interest and the first joint project for ChevronPhillips Chemical Company is deemed a success. Please direct any questions regarding this proven new service to Peter Gebhardt at (281) 476-3121 or pgebhardt@inf-grp. com. Brazos M&E Begins Concrete Batch Plant Operations Brazos M&E began selling concrete to the public in August 2012. Our priority is to serve Brazos M&E civil construction projects. Our business plan is to start with pours that are smaller in size – less than several hundred yards. This will allow us to refine our raw material supply chain, ordering process, dispatching and overall plant operation without risk to ourselves or our customers. Ultimately, our capacity will be determined by raw material supply, number of trucks/drivers and distance to the pour (turnaround time). We got into the concrete redi-mix business primarily as a way to solidify our position as a civil contractor. We view this as a natural extension of our existing businesses. This allows us to be more responsive on our civil construction projects that are schedule driven. We also believe that there is a need in the area for additional concrete batch capacity. Currently we have 7 concrete mixer trucks. With this number of trucks we should be able to batch 70-100 cubic yards per hour depending on total turnaround time. We can provide any of the industry “standard” mix designs at this time. With notice we can obtain the necessary aggregates or admixtures to provide any mix design the customer requires. Our business plan is to start by providing “standard” mix designs and add to those as we gain proficiency in our processes and operation. For more information about Brazos M&E’s Concrete Batch Plant Operations, please contact Daryl Dauer or Benny Dunn at (979) 233-2841. 3 4 Management Recognition Program Doing The Right Thing The Safety Management Team recognizes front line supervisors and employees for their performance and efforts in demonstrating, performing and promoting safe work practices. Whether it’s leading their crews, a member of the crew recognizing a hazard and making an intervention, or going above and beyond their normal job duties, we want to recognize those individual’s for their initiative and commitment. When the individual or group of employees is nominated they are invited to a recognition lunch that is attended by corporate management, safety management, employees’ supervisors and the nominated employees. The employees are presented with an award for their contribution and commitment towards safety on the jobsite. Pictured: (L to R) Bobby Gibson, Jose Tijerina, Luis Saucedo, Mike Rachunek, David Rodriguez, Mike Roach, Mark Coffee, Valentin Rodriguez, Ty Jeffers, Arnold Trevino, and Mark Smith Jose Tijerina has worked 36 years with the company and most of his crew has worked 5 or more years together. As I observe this crew on a daily basis I see good communication between each other and a drive to help each other out by intervening. Most of them know something about each other’s family, which makes it more personal if someone is involved in an event. Jose has earned the respect from his crew and they will listen and take care of any issues that might come up. This crew has gone many months without any significant non-compliance issues or incident and show patience with rules and changes that come up. Any comments or issues that I give to the group is taken with respect to our safety department. Jose and his crew represent The Infinity Group with respect. Thanks for your support and dedication to providing a safe work environment. Mark Smith – Project Safety Director, Infinity Construction Services, Pictured: (L to R) Bennie Mikel (Scaffolding Manager) and Adolph Estrada Adolf Estrada was recognized by the Infinity Group Management Team for his efforts while working at the Dow Chemical Freeport site. Adolf’s crew consisting of two supervisors had no non-compliance observations in the past year. They were also recognized by Dow Chemical at a supervisor’s training session. Thanks Adolf for your commitment to safety! Frank Mejia – Safety Director, Infinity Construction Services & John Crowley – Regional Safety Manager, The Infinity Group 5 Tim Burch’s crew was working on installing a blind on an existing line at a 3-way when David Simpson and John Wade noticed the line was not setting on its supports. After further investigation they found that the process line supports were cracked and the welds were breaking off. If the condition of the pipe was not noticed soon it could have resulted in a significant event in time and possibly injured someone. We appreciate the employees for being observant and paying close attention to their surroundings. Scott Martin – Assistant Superintendent, Infinity Construction Services Pictured: (L to R) David Simpson, John Wade and Grady Crawford Pictured: (L to R) Jerry Lancon (QA/QC Manager) and Jon Campbell I wanted to let you guys know that I really appreciate Jon Campbell’s level of dedication and professionalism that I have personally witnessed on my jobs (B-4400, B-4900, B-3300 to name a few). Since being here at Dow I have seen Jon often in the field doing what he is supposed to, checking over job details, ensuring that the job is installed properly and to a high degree of craftsmanship. I also appreciate his knowledge; he helps out troubleshooting items when such occasions have occurred. Jared Jenkins – Kelly Services, The Dow Chemical Company, Freeport Pictured: (L to R) Jack Kennon, Dean Mezger, and Jimmy Moon Jimmy Moon and his crew did a great job installing Jumbo EDC’s new intercom system. We were very impressed by their obsession with cleaning up, not just after, but during every task of the job. All of the UEI employees were respectful of Dow personnel, and other contractor employees. We needed the intercom system fully operational before turnaround started and UEI did what it took to make that happen for us. Jimmy Moon also self-initiated work to eliminate a “hum” in the system that he noticed after it was installed that could have just been written off as “normal”. This was especially impressive. His obsession with making sure the job was finished right has provided Jumbo EDC with an outstanding intercom system. Carlos Garza – The Dow Chemical Company, Freeport 6 Employee Recognition Doing The Right Thing Osiel “Fonzy” Elicerio identified a safety hazard on the grated decking above the E-150 exchangers. His crew was preparing to work in the area where the grating was not properly clipped down and presented a trip hazard. He also identified a suspected problem with an uncovered vertical fin-fan shaft (E-153’s) adjacent to a maintenance walkway. The safety concerns were reported to Operations and fixed prior to working in the area. Thanks Fonzy for your commitment to safety! JoAnn Zimmerman – BASF, Freeport Pictured: (L to R) Joe Gonzales (Supervisor), Osiel Elicerio, and Christie Ramirez (Safety) UEI Hosts Overhead Crane Inspection Training Program In February, the Crane and Elevator Group of UEI hosted an Overhead Crane Inspection Training Program conducted by Crane Tech in the Corporate Training Center. The program consisted of two 10hour days of comprehensive information about the process of overhead crane inspection and maintenance. The program provided knowledge of the regulations, specifications and inspection of various types of overhead cranes and a hands-on inspection of the overhead cranes in the company shop facilities. The program also contained the process of ensuring safety and compliance of the crane operations. The UEI participants were Richard Parker, Cody Case, Travis Timaeus, William Harris, David James and Rudy Silvas. Letters We Like Tony Lewis & Scott Vestal (UEI): What makes a project successful is having individuals like Silvestre Garcia that will take that step forward to stop some of his colleagues from getting injured. During a permit walk through with Silvestre he noticed some liquid on the ground in an area where they and many others would be working or walking through. After an investigation, we found that we had a tube leak on E-101D2 (fan B). Because of Silvestre’s willingness to question what the liquid was (PACE) we were able to tape off area and work on isolating/stopping leak without anyone getting exposed. We appreciate workers like Silvestre who look out for others and are willing to take ownership of his work environment. This could have been a bad situation had Silvestre not took action. From the Construction Group, Jumbo EDC thank you to Silvestre. That is the way Drive to Zero & PACE is successful. Dave Matthews & Philip Deville Dow Chemical, Freeport Tony Lewis (UEI): We did a “Because I Care” Safety audit at OC-479 and talked to Albert Helms and Mike Corsten. As always we were well received and your UEI team had all the proper safety equipment, knowledge and attitude. Please give my thanks to all the UEI team for working with us to make sure no one gets hurt. Charlie Condon Dow Chemical, Freeport Pete Valenzuela (UEI): James Moon and crew have done an outstanding job installing new intercom system in A-7001 EDC. They have shown a great sense of urgency in completing the project safely and on time. Interaction by the crew with operations by showing flexibility in their work has been exceptional. Safety observations and concerns were addressed promptly by escalating issues found in execution of their work. Great PACE fundamentals by the crew were shown on a daily basis. Gary Mills, Murphy Watson & Ross Koepke Dow Chemical, Freeport Mark Mahurin (UEI): I wanted to let you guys know that really I appreciate Jon Campbell’s level of dedication and professionalism that I have personally witnessed on my jobs (B4400, B-4900, B-3300 to name a few). Since being here at Kelly I have seen him often in the field doing what he is supposed to—checking over job details and ensuring that the job is installed properly and to a high degree of craftsmanship. I also appreciate his knowledge—he helps out troubleshooting items when such occasions have risen. Jared Jenkins Kelly Services at Dow Chemical, Freeport Speedy Wells (IMS): Today a bleach truck arrived to pick up a load of 12.5% bleach, but the Bill of Lading incorrectly had his load listed as 10.5%. Instead of simply loading what was on the paperwork, Richard Arriaga questioned the driver and determined the Bill of Lading was incorrect, preventing the costly mistake of Chlor/Alkali sending one of our customers an incorrect load of bleach. This is not the first time Richard’s efforts have come to our attention, and he is considered a valuable asset to our unit and has earned the respect from many of the FPC employees he works with. We think his efforts reflect positively not only on himself, but on Infinity was a whole, and we are glad to have him working in our unit. Scott Vick Formosa Plastics, Point Comfort James Anderson (UEI): We are wrapping up the second week of Pre-Commissioning a day early. We are 90 loops ahead of schedule and the he remainder will easily be completed in October. There have been very few items punch-listed, and all issues to date have been resolved. It has been a pleasure working with John Reans and his crew from UEI. Thank you guys for your hard work, professionalism, and for maintaining a serious commitment to safety. Randy Winnegar Dow Chemical, Freeport 7 Wayne Cromis (UEI): I just wanted to pass on what a great job Mike Gregory and his crew did for us at a very recent Air Liquide Turn-Around at our La Porte SMR. From their pre TAR preparation through the final loop check everything went as smooth I could have ask for. Craftsmanship to communication was excellent from all the hands and supervisors. Thanks for providing us with such great people to work with under what can sometimes be a stressful working environment. It was my pleasure & I look forward to working with great people within UEI again soon. Kevin Meismer Air Liquide, La Porte Tony Lewis (UEI): Great job by Scott Vestal and his team with the live migration of the 1702 instruments to date along with all of the prep work leading up to these migrations. Keep up the good work and lets complete this contract safely and move on to the next. It takes a great amount of planning and communication to accomplish what you have done with this migration work and I do appreciate the effort your team has put into this work. Tom Closs Kelly Services at Dow Chemical, Freeport Kenny McClellan (ICS): I just wanted to send an email in reference to the PSM and 1150 work that was completed last week. Infinity has been working with BASF production to make a schedule that can be used for both teams to help remove downtime and increase efficiency. I feel that the effort put in by Erik Chitwood, Cody Vollbaum, and Harold (Tiny) Barnes was very beneficial in getting the plant back up and running with minimal lost time. The schedule set by us was very aggressive and had been reviewed (SMED) several times to eliminate any waste that we could find. Cody and Tiny helped us review all tasks from clearing to start up. Above all, this work was accomplished without any safety incidents. Great job by everyone involved. Robert Massey BASF, Freeport 8 Letters We Like Tony Lewis (UEI): Just wanted to inform you that Jonathan Baggett has done a great job here during the Epoxy2 TA. He has helped out when we needed him, while maintaining a great attitude. Michael Gutierrez Dow Chemical, Freeport Jim Mobley (ICS): I want to say thank you to Milton Lopez for a job well done on the piping in Utilities last week. It was a very congested area with additional workers in the same area for a different project. Despite all these challenges, Infinity completed the job on time, and most importantly it was completed Safely. I appreciate the extra effort and team work. Joey Fountain Bayer MaterialScience, Baytown James Anderson (UEI): Albert Garcia and his crew did an outstanding job on the WK3140 Project in terms of quality and getting the project completed in a timely manner that pleased the block. The project team is very pleased with their performance and that all work was executed in a safe manner. Jared Jenkins Kelly Services at Dow Chemical, Freeport Chris Seymour (ICS): Thank you to Charlie Ovesny for his work on PM-3804 CBF Transfer Line Project. We are impressed with his good quality and hard work. Unfortunately we had at least one day rain per a week and lost almost 10 days for raining in January but we think this project had good progress. We appreciate your good work for this project. Hiroyuki Kobayashi Shin-Etsu Chemical, Louisiana Chris Seymour (ICS): Charlie Ovesny and his crew are doing a great job out here. We look forward to doing upcoming projects with Infinity. Dwain Boullard & James Bullard Goodyear, Houston Jim Mobley (ICS): I have been asked by the Project Manager to pass on to you our appreciation for a job well done on the 6” Instrument Air Header. Our Shift Technical Lead on Days reported that Milton Lopez had a well thought out work plan and applied the right crew mix. This good combination allowed your crew to finish the job Safely, and sooner than was anticipated. Steve Turner Bayer MaterialScience, Baytown Chris Seymour (ICS): I appreciate the job Yancey Marshall and his crew did and their patience with the project. John Taulbee AAF International, Kentucky
