Payman Simoni, MD
Dr. Simoni is a dual board certified Facial Plastic Surgeon, specializing in cosmetic and reconstructive surgery of the
face. Payman Simoni, M.D. earned his M.D. degree from the Albert Einstein College of Medicine. He then completed
general surgical internship followed by Facial Plastic and Head & Neck surgery training at University of Alabama
at Birmingham hospital.Following completion of his training, Dr. Simoni seeked an experience that most plastic
surgeons dream of but never achieve. He decided to travel through the country to study under and spend time with
some of the top leaders in the field of facial plastic surgery to benefit from their techniques and experiences.
His background in nasal surgery combined with his “natural looking” result has made him one of the most sought
after rhinoplastic surgeons on Long Island. Dr. Simoni is the director of Long Island Facial Plastic Surgery Center
located in Plainview and Great Neck.
Kevin Tehrani, MD
Dr. Tehrani earned his medical degree at SUNY Downstate Medical Center. Then, he completed his general
surgery residency at Beth Israel Medical Center in New York where he was Chief Resident in General
Surgery. He continued his postgraduate training in plastic and reconstructive surgery at Kansas University
Medical Center where he again served as Chief Resident.
Believing that plastic surgery can be the ultimate expression of art in surgery, Dr. Tehrani values reaching
the highest levels of achievement in reconstructive and aesthetic surgical training, continuing education and
clinical experience. Dr. Tehrani is the founder and director of Aristocrat Plastic Surgery with offices in Great
Neck and Manhattan.
Robert A. Jason, MD, FACOG, a Board Certified Obstetrician/Gynecologist, has always been on the cutting
edge of medical technology for women. He is the Medical Director of three leading medical practices in New
York – The Laser Vaginal Rejuvenation Institute of New York, ExAblate of Metropolitan New York, and
Great Neck Women’s Medical Care.
As Medical Director of Great Neck Women’s Medical Care, he was the first physician to open a comprehensive women’s diagnostic center and perform the first non-hospital outpatient Amniocentesis on Long Island.
After Dr. Jacqueline Fulop-Goodling completed her DMD degree and orthodontic residency at Boston University
School of Dental Medicine in 1996, she assumed a professorship at her alma mater becoming the first female Director of the Predoctoral Orthodontic Department. Dr. Fulop-Goodling has taught over 500 students world-wide and
more than 1200 dentists in multiple areas of orthodontics. She has several U.S. and International patents issued of
which two are dentally related.
In January of 2005, Dr. Fulop-Goodling was selected to Invisalign’s National Speaker’s Bureau. In September 2005,

with Smokin’ Eyes

o you desire to do a smoky look on your eyes when you go out but
cannot achieve the look? We have seen it on models, J-Lo and many
call the look “cat eyes”. Many women desire this look and it is stunning
when applied properly. I run into many women in my line of work wanting this look for themselves. I want to stress that different eyes contours, textures
and brow lines require different applications. You have to apply your eye makeup
so that it suits your eyes. The color of your skin, hair and eyes are to be considered
too. Dark circles? It’s important to remember a good eye cream and concealer
before the application process. Here are some easy tips to help you achieve that
look for a night out.

Wash your face with a face cleanser according to your skin type. Make sure
you pat gently dry.

Apply your foundation and concealer. When using concealer bear in mind it
is to cover imperfections in eyes.
Apply a dime size of eye cream around both eyes. A dime size is enough for
both eyes. Make sure that you tap it into the eye gently and do not press the
eye. Make sure you have applied moisturizer as well to face.
Apply to your eyelid generously apply a peach, beige or pale pink eye
shadow to the whole area from the eyebrow to the tip of the top lid. Then
take a plum or medium brown shadow and go along the center contour of your
eyelid with your shadow brush. To the lower part of your top lid if you used a plum
go with a pink, orchid or a gray or blue color that is lighter than the center color
but darker than between the center of the lid and brown bone. If using a brown a
gold or copper color in that same spot would be suitable. Blend, Blend, Blend!

Now if you prefer a dark top liner, I would suggest Benefit Cosmetics BAD
GAL Eyeliner. Just apply to top lid. If you have slanted eyes this is perfect for
you. Now if you have the slanted eyes do not apply anymore eyeliner and wait for
your next instructions.
For those of us who do not have slanted eyes – Using your angle/arch brush apply charcoal if you are a very light skinned blonde or redhead, for the rest of you
black powder eyeliner to top lid and blend stroking out. You want a thicker line
on top (please ladies do not take that a license to kill your top lid with too much).
Then proceed to apply a thin line under the eye and always sweep up and out with
the brush. Make sure you leave a tip at the edge and again not too much. Less is
always more, always apply a little at a time until you achieve your desired look.

Finish off with mascara a black preferably for nights out. No matter what
your hair color and skin tone! You could also use plum mascara for the bottom lashes if you used plum and/or pink in your eye shadows.
The main thing you should understand this is for fun so don’t be afraid to try it with
different color mixtures!

What You Should Know About Plastic Surgery of the Nose (Rhinoplasty)

Most Rhinoplasties are perfomred
becaue the patient desires an imporvement in appearance and/or nasal function. People may simply want a nose
which is in harmony with the rest of
the face rather than one which is out
of proportion with respect to the other
facial features.
Dr. Simoni believes that rhinoplasty
surgery is as much artisitc in nature
as it is scientific, rarely are any two
patients noses identical. He advocats
making each patient’s nose fit his or
her face. Dr. Simoni relies heavily on
computer generated before and after
pictures during his consultations; so
every patient can explain and observe
desired changes prior to surgery.
Dr. Simoni does not ordinarily “pack”
the nose after surgery. With the
elimination of nasal packing, pain,
swelling, bleeding, discoloration, etc,
are dramatically reduced making the
recovery period much more pleasant
for the patient. Dr. Simoni employs
a special technique of suturing the
internal nasal tissues back in place
eliminates the neces-sity of packing.
An often asked question by patients
conteplating rhinoplasty is “do you have
to break the nose?” In his technique, Dr.

Is Dr. Becker’s Lasik Surgery for
everyone?
If you are over the age of 21 and have a
refractive error you could be a good candidate for Lasik surgery as an alternative
to wearing corrective lenses. (Glasses or
contact lenses)
What is a refractive error?
When light is not properly focused on the
retina on the back surface of the inner eye
due to the axial length of the eye being
too long or too short it causes Myopia
(nearsightedness) and Hyperopia (farsightedness) respectively. Changes in the shape
of the corneas results in Irregular arrival of
the light rays to the retina and can cause
astigmatism.
What is LASIK?
LASIK is a surgical procedure that corrects
refractive errors by changing the shape of
the cornea, to refocus the incoming light
onto the retina effectively,
Is LASIK results guaranteed?
The goal of LASIK is to reduce or eliminate
a person’s dependence on corrective lenses.
Although LASIK cannot guarantee 20/20
vision for every patient, it can significantly
improve eyesight and usually eliminate the
need for corrective lenses
How does LASIk work?
Laser surgery utilizes a laser to reshape the
curvature of the cornea. For nearsighted
patients, the goal is to flatten the too steep
cornea; with farsighted patients, a steeper
cornea is required. For those with astigmatism, the irregular cornea is smoothed
into a more normal shape.
LASIK, (Laser in-situ keramtomileusis),
is the most commonly performed type of
laser surgery and is generally a safe and effective treatment for a wide range of common vision problems. In LASIK surgery,
the cornea is reshaped after making a flap
in the cornea, so that the laser treatment is
to the stoma (inside) of the cornea. There
is generally a faster recovery time after
LASIK than other procedures, such as PRK,
which treats only the surface of the cornea.
LASIK is a relatively quick procedure, usually 5-10 minutes per eye, with the entire
surgery process taking approximately one
half hour.
How is this procedure more advanced
then the previous Refractive Eye Surgery?
Today’s LASIK surgery is quite advanced.
An instrument called microkeratome is
used to create the corneal flap, usually set
to the thickness of about one human hair.
A computer driven laser, programmed by
your surgical team to produce extremely
accurate customized results, performs the
laser reshaping of the cornea.
The Food and Drug Administration first
approved the excimer laser in October
1995 for PRK. The FDA limited use of the
laser to practitioners trained both in laser
refractive surgery and in the calibration
and operation of the laser. As of November 1998, the excimer laser has been
approved for the procedure laser in situ
Keratomileusis (LASIK).
Is LASIK the only option?
LASIK is only one form of laser surgery.
Your physician should discuss the other
refractive surgery options, some which use
lasers and others, which use incisions or
inserts, such as PRK, RK and Epi LASEK.
Every patient is different and must speak
with their doctor to see which refractive
surgery will yield the best results.
What kind of recovery time can a patient
expect after Lasik?
Immediately after LASIK patients can see
well enough to comfortably ambulate, and
relax at home, or go out to a restaurant.
The majority of patients are able to drive
the first postoperative day after LASIK.
In terms of discomfort, most patients
experience some tearing and foreign body
sensation for a few hours to one day. Some
patients experience dry eye symptoms in
the post-operative period.
Why do you recommend LASIK to your
patients?
I recommend LASIK as an alternative to
glasses or contact lenses to those patients that
are unhappy with those first two options.
For an active lifestyle, it is just easier not
to have to think about putting in contact
lenses, or finding one’s glasses. For travel,
sports, and hobbies, glasses or contacts can
be quite inconvenient.
For those patients that are contact lens
intolerant, as I was. LASIK provides an
excellent option instead of wearing glasses.
What is the first step?
The first step in contemplating LASIK, or
any other refractive procedure, is to think
about what your visual needs and preferences. Are you unhappy in glasses? Are
you unhappy in contact lenses? Are your
contact lenses uncomfortable? Are your
glasses or contact lenses inconvenient in
your lifestyle?
The next step is to be evaluated by an experienced LASIK surgeon to make sure that
you are indeed a good surgical candidate.
Your doctor needs to make sure that you
have no underlying eye diseases, irregularities of your cornea, that your cornea is thick
enough for the procedure, and that your
refractive error fits the treatment protocols.

006 marks the 50th anniversary
of our first graduating guide dog
team. Guiding Eyes for the Blind was
founded upon a simple and beautiful premise; to enrich the lives of
blind and visually impaired men and
women. To date Guiding Eyes has
graduated more than 6,600 guide dog
teams. The men and women who
have received our dogs and worked
with our professional trainers have
returned to their communities with
greater independence and with new
horizons of opportunity.
We have always striven for excellence in all facets of our program: in
the maintenance of our facilities, in
the remarkable dogs we breed, in the
training programs for our dogs and
our blind students, and in the lifelong
follow-up services we provide our
facilities, programs and professional
training staff on a regular basis to
insure that Guiding Eyes remains in
the forefront of its field well into the
twenty-first century. Last summer,
Guiding Eyes for the Blind began work
on a 15,000 square foot addition and
enhancement project that will enable
us to better serve our students. Guiding Eyes has always been a leader in
the guide dog field, offering new and
innovative programs that better serve
the needs of the blind and visually
impaired.
Special Needs Program designed for
students with disabilities in addition
to vision impairment. This could
include certain developmental and
physical challenges. Specially selected
and trained dogs enable them to assist
these students.
Accelerated Client Training Option
(ACTION) a unique 15-day training
program for experienced guide dog
users. Students in this program begin
their training along with the residential class. After 10 days, ACTION students return home for an additional 5
days of training with a senior member
of the Guiding Eyes training staff.
Home Training Program enables us to
bring Guiding Eyes
dogs to the homes of
students who cannot
attend the residential
program and provide
them the necessary
training within their
own community.
Adaptive Technology Program at our
training facility offers students special
learning opportunities including talking
computers, reading
machines with print
magnification, as
well as orientation
and training material
for those interested
in the latest that
technology has to
offer.
Hispanic Community Initiative is a
program that Guiding Eyes is developing in order to better serve the needs
of visually impaired individuals within
the Hispanic community.
All of our services are provided to our
students free of charge. Guiding Eyes
for the Blind receives no government
support, instead relying upon the
outstanding generosity of private individuals, civic groups, foundations and
corporations. For 29 years, our most
prestigious and successful fundraising event has been the Ken Venturi
Guiding Eyes Golf Classic. There is no
denying the significance of what this
event has meant to the blind community, raising over $6 million. That is
an overwhelming achievement.

Til Snoring Do Us Part
Does snoring or irregular breathing disrupt your sleep?

noring is secondary to air forced
through a narrow airway with the
vibration of unstable tissues in the
nasal passages and the palate. Snoring
is not physically dangerous, (except

o exactly how does the ugly
duckling find her beauty? From
the inside out. Julie Michaels, now
an accomplished actress known best
for her role opposite Patrick Swayze
in “Road House”, stumbled her way
through high school and college without even a date to the prom. “Let’s tie
a bone around her neck…maybe the
dog will play with her” her dad said.
But Julie never gave up. She found
the Miss America Scholarship pageant
as a means to pay for her education.
There, she learned so much about
beauty, inside and out.

COSMETIC EYELID SURGERY

A
mericans are living longer and
much healthier lives than ever before. Nonetheless, age-related changes
are, at least for now, inevitable.
America is graying and sagging.
But, we will not go quietly. Almost
six million cosmetic procedures were
performed in 2000 according to the
American Society for Aesthetic Plastic
Surgery. It is estimated that in 2004,
Americans spent more than 12.5 billion
dollars on surgical and nonsurgical
cosmetic procedures. This represents
a significant increase over past decades
with eyelid surgery now being the
second most commonly performed
cosmetic operation.
Ageing affects all organs and tissue
systems. Cardiac output decreases, pulmonary capacity diminishes, and the
integrity of the skin surface becomes
impaired when one reaches their early
30s. Ophthalmic plastic and reconstructive surgeons are uniquely gifted
in restoring the youthful appearance of
the eyelids. Ophthalmic, plastic, and
reconstructive surgeons have a unique
knowledge of lid function and ocular
surface disorders including a dry eye.
INTRINSIC AND EXTRINSIC
SKIN AGEING:
There are two components of skin ageing: An intrinsic one and an extrinsic
one. Intrinsic skin changes reflect clinical and histologic changes that occur
in sun-protected areas of the body. The
intrinsic process includes decreased
vitamin D production, decreased sweat
gland secretion, and a delayed rate of
cellular replacement with time.
The extrinsic component of skin ageing
is largely due to the effects of sunlight
on exposed skin. If one compares facial
skin exposed to the sun over a lifetime
to skin typically hidden by garments,
there is a discernible difference in
the coloration and texture of skin of
identical age. Sun exposure over time
not only alters collagen production
and quality, but also increases the rate
of oxidative protein damage. Sunlight
shortens chromosomes and with time
impairs chromosomal stability and
resulting protein production.
The question arises as to how clinicians
can best manage the varied problems
resulting from intrinsic and extrinsic
skin ageing. Perhaps one day the answer will be in a molecular reprogramming of tissues. Until then, the arsenal
we command includes botulinum
toxin, laser resurfacing, chemical peels,
and upper and lower eyelid cosmetic
surgery.
The treatment modality of choice
ultimately depends on the needs of the
patient and his or her expectations. A
patient with prominent wrinkling and
no puffiness would best enjoy the benefits of laser resurfacing or a chemical
peel. Botox manufactured by Allergan best serves men and women with
strong muscle tone and wrinkling. A
patient with puffy eyelids and redundant and excess skin would be an ideal
candidate for cosmetic eyelid surgery,
otherwise known as a blepharoplasty.
PREOPERATIVE EVALUATION:
Preoperative evaluation for upper and
lower eyelid blepharoplasty includes
many relevant portions of an eye exam
including vision, eye movement, and
pupillary size and response. Particular
attention must be paid to the quality
of the tear film as dry eye is common
throughout the population and may be
exacerbated by cosmetic eyelid surgery.
In addition, the height and contour of
each upper eyelid, the location of the
upper eyelid fold, and the location of
the puffiness must be noted. A preoperative consultation should also include
a discussion of the risks of surgery and
potential complications. Surgical fees
including operating room cost, anesthesia, and the surgical fee are frequently
discussed at the time of an initial visit.
UPPER EYELID SURGERY:
Upper eyelid surgery is reasonably
straightforward unless the patient has
heavy brows falling into the eyelid
space. These patients need either to
have a brow lift or be warned that
eyelid surgery while improving their
eyelids will not completely remove all
the upper eyelid fullness. On the day
of surgery, the patient is instructed
not to eat or drink. The upper eyelid crease is outlined as is redundant
upper eyelid skin. Measurements are
made on both the right and left side to
ensure symmetry. Small adjustments
are frequently made with the patient
sitting since gravitational forces may
affect the draping of eyelid skin. The
patient’s face is then washed with soap
and water and local anesthetic infiltrated into the surgical site. Using a
blade or scissors or a laser, the outlined
skin is excised. Frequently, redundant
muscle is also removed. Thereafter, the
fat which has created the puffiness is
exposed and removed or repositioned.
The upper eyelid skin is then closed
with either one continuous stitch or
multiple interrupted stitches.
GROWING DEMAND:
As the American population ages and
demands to remain in the workplace
remain strong, Americans will be
undergoing more and more cosmetic
eyelid operations in the years ahead.
The face is the center of the cosmetic
universe and the eyes and eyelids tell
us much about our vigor and well being. With upper eyelid surgery, the
puffy and redundant upper eyelid skin
is removed restoring a more youthful
and vibrant appearance.

Today, there are many ways of improving one’s smile. Teeth that are
discolored or stained extrinsically (on
the outside from foods with tomatolike sauces, and drinks like coffee,
tea and red wine) can be bleached or
whitened. Did you know that aging
also causes a natural yellowing of the
teeth? There are so many techniques
and a variety of bleaching agents from
over-the-counter whitening agents to
in-office laser bleaching, which can
lighten teeth 8-10 shades.
The OTC agents consist
of a gel-like substance
comprised of carbamide
peroxide or hydrogen
peroxide. Their delivery
systems vary from being
able to paint the bleaching agent on to other
ones coming in bandage
like strips soaked in the
bleaching agent.

your dentist’s office can also vary.
You can sit in a
dental chair from
30-90 minutes
and walk out
with whiter teeth
because of a stron-
ger strength carbamide peroxide or
hydrogen peroxide used. Some of these
stronger agents require a blue curing
light to activate the bleaching agent.
Other agents are activated by mixing
the gel together on the teeth. Either
way, you walk out with whiter teeth,
immediately after the procedure. The
disadvantage is that you need to return
for touch ups or use trays with bleach
to maintain the whiteness of your
teeth and can add additional expenses.
Another professional bleaching system
offered in dental offices is a customized tray made to fit your teeth. These
are known as, “the trays.”An impression is taken of your teeth from which
a mold is made. Clear bleaching trays
are fabricated onto these molds or
casts of your teeth. Different varying
strengths of a whitening agent can be
applied into the tray. The strengths
vary depending upon the sensitivity a
patient has toward bleaching.
The newest bleaching system creating a lot of steam is GoSmile. It is a
5-6% hydrogen peroxide bleaching
agent in a lipstick lip container or
carpule. After squeezing the carpule,
you brush it onto your teeth. While
whitening your teeth, you get to
brush the current residue off of your
teeth with the added benefit of having fresh breath because of its mintlike breath freshening substance in
it. It is a one step system which is
perfect for a busy person, constantly
on the go.You do not need two hands
to use it since you’re not painting it
on. You don’t have to worry about
strips falling off of your teeth or
having to sit for trays to be made.

What prompted Long Island Heart Associates to start using Ultrafast CT for
the detection of heart disease?
No doubt, the best medical care is
preventative care. This exciting new
technology enables us to detect atherosclerosis or coronary artery disease
during an early stage, before the possible complications of a heart attack,
angina or sudden death. Screening for
coronary calcification (plaque build up)
is the most sensitive, non-invasive test
to determine early atherosclerosis. The
technology of Ultrafast CT allows for
visualization of the coronary arteries to
detect blockages. One of the tests, called
CT Angiography, provides information
that previously could only be obtained
by an invasive angiography. Invasive
angiography is performed in a hospital,
requires a needle to be placed in the
artery and a tube to be placed in the artery to inject dye to see blockages. CTA,
on the other hand, involves a simple IV
injection of dye in an outpatient setting.
Therefore risks of bleeding from the artery or other complications do not exist.
What do the tests involve?
The Calcium Score test is a CT scan of
a limited area of the chest and heart to
look for plaque buildup in the arteries.
The test takes less than 10 seconds and is
completely non-invasive-no needles or
contrast dye are used. CT Angiography
has several parts, but the test itself takes
less than 10 minutes. A needle is placed
in the vein for the contrast agent and
the patient is asked to hold their breath
for varying periods of time. The patient
can go home immediately following as
there is no recovery time. Both tests are
read by our cardiologists and a detailed
report is sent to the referring physician.
Who is the test appropriate for?
Patients with intermediate risk factors for heart disease such as smoking,
high blood pressure, high cholesterol,
diabetes, or family history of heart disease will benefit from a Calcium Score
test. Being a more sophisticated test, a
CT Angiography is ordered for those
patients that may have an existing heart
condition, when the physician needs to
know very specific information about
the condition of the coronary arteries.
The images obtained give our physicians important diagnostic information
needed to design prevention, treatment
or interventional programs depending
on the results.
What are the advantages of using
this method?
As mentioned, Calcium Scoring helps us
to identify the atherosclerotic process
in patients with risk factors at a much
earlier stage than with traditional testing
such as stress testing. Statistics show that
85% of heart disease progression can be
halted or prevented if diagnosis is made
sooner. CT Angiography is a useful
alternative to an invasive angiography
study in the hospital. Information can be
obtained without the inconvenience or
anxiety of a hospital admission.

A Desert Oasis Fit for A Queen

he Palm Desert area of Southern
California is certainly one of the most
beautiful Desert areas in the United States
if not the world. Because of its abundance
of groundwater and it close proximately
to Los Angeles it is also one of the busiest
tourist areas in CA in the winter time.
When it is cloudy and rainy any day
in LA it is sunny just two hours away.

n 1906, the year that San Francisco was
devasted with earthquakes and fires was
also the year that the famous Leland Stanford Mansion was demolished. It was then
restored in 1912 to the Luxurious Stanford

uaintly and situated in the newly
renovated Hilton in the Financial
District on Kearny St in San Francisco
is the tru. Because of busy schedules
and the spas financial district location,
it caters to a lot of locals. Being pampered on your lunch hour never gets
more convenient. At tru, they have
designed treatments focusing primarily on luxury, relaxation, rejuvenation
and effectiveness. The environment
is very welcoming and clean and offers an array of outstanding services
and products.
The décor is extremely relaxing and
fabulous. The colors and lighting are
based on moods and mood changes
and the light and your aura is a part
of each treatment room. One of their
specialty treatments endured by most
is the Jungle Love, and appropriately
is done in the Rainforest Treatment
Room. You are first exfoliated with a
coconut and kukui nut based exfoliant
, rinsed and covered with a creamy
mixture of tropical fruit (papaya, mango, pineapple and banana) and tropica
fruit enzymes (papaya, pineapple)
which are activated by the steam to
soften and clarify the skin. While this
is conditioning
and replenishing
your body benefits your body,
your hair will be
applied with a
deep conditioning
pack, to hydrate
and smooth your
cuticles. After
the steam, you
will enter a small
rainforest room to
rinse your body
of any left over
debris. This treatment is stimulating along with
very beneficial
for hydrating of
the skin.

that is a must to try is one of their
facials; the most popular being the
About Face. This facial is 100-110
minutes depending on the condition of your skin. The aesthetician
will first examine your face under
microscopic lenses to tell you what
your needs and what will be most

gentle massage and cleanse to activate the skin cells. After an oxygen
wrap and oxygen spray to open up the
pores (at Tru they do not believe in
steam, they believe in the purest of
