Caldwell Hospice among Best in US
Welcome to the spring issue of CareLines.
First things first: Caldwell Hospice and Palliative
Care homecare nurse Dottie Metzger’s retirement is
our “cover story.” She retired at the end of
February, as our longest-tenured employee with 27
years of service.
If you look for “hospice nurse” in the dictionary,
you might see a picture of Dottie Metzger. We have
shared some “Dottie stories” on page three, and
their common theme is her respect for the dignity
of her patients and families. She combined professionalism with extraordinary people skills to serve
as a role model for all of us.
Dottie’s contributions to hospice care were proven,
when Lenoir’s News-Topic readers voted her
Caldwell County’s best nurse in 2005 and 2008.
We cannot measure Dottie’s value to our patients
or our staff. We will miss her!
Next, our entire staff deserves thanks for helping
us maintain our status as one of the top hospices
in the country. Each quarter, the National Hospice
and Palliative Care Organization (NHPCO) conducts
surveys with patient family members and caregivers. Caldwell Hospice is consistently among the
national leaders, and the numbers show why.
Fourth-quarter 2011 survey results place Caldwell
Hospice among the top 10 percent of all hospices
nationwide in 19 of 22 quality indicator results. In
14 of those indicators, we recorded a perfect score.
The overall quality of care given to our patients
during the fourth quarter of 2011 was 93.9%,
compared to 81.1% for hospices in North Carolina
and 75.6% for
hospices nationwide.
The survey reports a
hospice’s performance
in four key areas:
coordination of care,
attending to family’s
needs, providing information about
symptoms, and informing and communicating with
patients. Performance is presented as a “problem
score,” with zero noting no problems. Below are
Caldwell Hospice’s performance numbers, followed
by North Carolina and national figures:

Dottie Metzger Retires after 27 Years
She’s More than “Just a Nurse”
Dottie Metzger retired from Caldwell Hospice and
Palliative Care at the end of February as its
longest-tenured employee, concluding a 27-year
hospice homecare nursing career.
Dottie’s home visits with patients go far beyond
“duty”; she cares about them. “I will miss my
patients and their families,” Dottie said.
Dottie’s patients and their families will tell you that
the feeling is mutual, and people throughout
Caldwell County agree. Lenoir’s News-Topic readers
voted her Caldwell County’s best nurse, in 2005
and again in 2008. Her philosophy of hospice care
is “to keep patients comfortable, give them the
dignity they deserve, and give them the opportunity to die at home.”
During a recent home visit, Dottie pulled out her
stethoscope and listened to Alley Brooks breathe;
she read medicine labels and reassured his wife
that the store-brand cough medicine was okay; she
asked Mr. Brooks if he was feeling better and then
got a second opinion from his wife to be sure.
The Brooks’ cousin who lives next door came in,
followed by a friend who was dropping by to visit.
The couple’s German Shepherd Charlie sat to the
side of the couch, close by the patient. Laughter
rose and fell, as jokes and teasing were matched
with questions and answers about Mr. Brooks’
health.

This is the atmosphere that Dottie cultivates: you
might think she’s a friend dropping by for a weekly
visit, except that Dottie gathers necessary medical
information and learns essential details during the
visit. This is her gift: she combines medical skill, a
professional focus on patients’ well-being, and the
special qualities that make her visits feel personal.
“Dottie carries skill, respect, and her stethoscope
on each patient visit,” said Executive Director Cathy
Swanson. “There is no way we can measure her
value to our patients or to our staff. We will miss
her.”
Dottie didn’t plan on a career as a Caldwell Hospice
nurse. She worked in Baltimore’s Lutheran Hospital
emergency room
for five years
before she moved
to Lenoir. It took
two requests to
entice Dottie to
join the Caldwell
Hospice staff. The
first time, Dottie
explained that she
wanted to help
people get better.
When the second
request came after
her father’s death,
Dottie welcomed
the chance to
“repay for services
rendered.”
It turns out that Dottie was a natural for providing
hospice care. “I believed in what I was doing,”
Dottie said. “I’ve always loved being here.”
“Here” is Caldwell Hospice, generally, and although
Dottie worked in the Stevens Patient Care Unit for
almost a year, after it first opened in 1989, she
was relieved when she returned to making home
visits. Dottie likes getting to know families, building
relationships with them, and teaching them that
patients can die at home, comfortably.
Co-workers can recite the important lessons they
learned from Dottie: be prepared for any situation,
ensure that patients will be pain-free, and maintain
a sense of humor. Dottie’s parting words to them,
at the retirement lunch, were: “I’ll genuinely miss
you all, but it’s time to go.”
Dottie plans to limit her travel to visiting her four
children and their families, including three grandchildren. She has been to Europe and on a cruise
to the Bahamas, Dottie explained, but “my wanderlust has left.” She will bake cakes—another of her
talents—and crochet and return to Caldwell Hospice
periodically to teach CPR classes.—Pam Hildebran

for both Camelot staff and Caldwell Hospice.”
The Fall 2011 Above and Beyond recipient, Brenda
Monahan (photo, right), is Camelot Manor’s nursing
unit secretary. Even in a “noisy environment, she
remains calm and steady,” wrote the staff member
who nominated her.
Caldwell Hospice employees and volunteers can
nominate staff members of area long-term-care facilities who encourage quality end-of-life care for residentpatients and their families and who go “above and
beyond” to support resident-patients, their families,
and Caldwell Hospice staff members. The award is
presented each quarter.

Caldwell Hospice’s Path of Remembrance
Each person’s life tells a story, and we at Caldwell
Hospice are happy to offer a way of ensuring that
your loved ones’ lives can be remembered.
You can purchase a personalized brick in memory
or in honor of your loved one to be placed on the
walkway that leads from the Kirkwood front porch
to our Stevens Patient Care Unit.
Each four-by-eight brick can be purchased for a
$200 donation; it comes with a certificate that
recognizes your gift. For an additional $25, you
can receive a duplicate brick with felt backing
and a display stand.
We will notify you when your brick has been
installed, approximately 12-16 weeks after the
purchase.

Ashewood Grief and Counseling Services:
Exploring Grief in All Its Varieties
Suicide Grant Put into Practice. We applied for a
Hogan Family Foundation grant in 2011, based on
our awareness that suicide rates in this community
and our resources for responding to them were not
in sync:
Ashewood…provides bereavement services for
anyone in the community who experiences the
loss of a loved one. In addition to providing
what might be identified as “normal” bereavement support, we have averaged 10 to 15
suicide-related cases per year over the past four
years. The gravity of the issue led two members
of our staff to participate in a workshop led by
UNC-Chapel Hill School of Social Work Clinical
Instructor Jodi Flick, ACSW, LCSW. The staff
members were determined to strengthen the
community’s ability to respond to the complicated grief that accompanies deaths by
suicide....
Our idea was to bring together, as a mutually
supportive group, the people who are charged with
resolving this issue:
Caldwell Hospice and Palliative Care proposes to
host (1) a day-long suicide-prevention workshop
to educate and train mental health professionals, nurses, physicians, teachers, counselors,
social workers, youth workers, police and correctional officers, school support staff, clergy, and
volunteers and (2) a half-day program for area
clergy. Ms. Flick will teach both workshops....
Using the Hogan Family Foundation’s generous
financial support, we held two workshops in March.
The first, Suicide: Prevention, Risk Assessment,
Intervention, Postvention, was a day-long presentation to professionals. The second, Community
Clergy: Caring for Families in the Aftermath of
Suicide, was a shorter presentation to a different
audience whose members are also called upon in
crisis situations. Both events were co-sponsored
with Catawba Valley Behavioral HealthCare.
We are grateful for the opportunity to explore,
explain, and offer practical answers that allow
professionals to provide support, with confidence,
for coping with the complicated grief that follows
deaths by suicide.
What Color Is Your Grief? Parents and guardians
might wonder how to help children who have lost a
parent, grandparent, aunt or uncle—the children
might be sleeping too much or too little; they
might be too rowdy or too quiet; schoolwork or
grades might suggest a problem.
At Grief Camp, children meet others who’ve known
similar losses. The staff, adult volunteers, and
volunTEENS use every activity to help the children
understand that it’s okay to feel sadness and loss.
As they talk, draw, write, listen, and laugh, they
can find the healing tools they need.
Children may not know how to express their
feelings. Who would know that COLORS would be
so helpful to this process? Well, Dr. Seuss, for one,
as these lines from My Many Colored Days illustrate:
"Some days are yellow. / Some are blue. / On
different days / I'm different too. / You'd be /
surprised / how many ways / I change on
Different / Colored / Days."
Last year’s Grief Camp
committee used Dr.
Seuss’s book and
explored “the Colors
of Grief.” Emotionconnected colors were
in every activity.
Colors gave children
freedom to choose
their expressions of
grief; colors gave
parents and guardians
a vocabulary for
talking with them.
One parent said,
afterward, “He was
able to express the anger, and now the sadness is
catching up.”
“We saw the anger he had, following his terrible
loss,” said another parent. “He filled the [sand art]
jar with his anger.”
At Grief Camp, staff members talk with the adults,
too, about how to continue the learning experience
once they return home, how to respond to anger,
tears, and other emotions; they offer support to
the grown-ups who are still grieving, too.
The camp is FREE, and lunch will be provided. To
pre-register a child, ages six to 12, for Grief Camp,

Unifour Volunteer Conference:
“Dementia Is More than Just Forgetfulness”
Caldwell Hospice and Palliative Care sponsored the
2012 Unifour Hospice Volunteer Conference,
welcoming approximately 170 volunteers from
Hospice and Home Care of Alexander (County),
Burke Hospice and Palliative Care, Palliative
CareCenter and Hospice of Catawba Valley, as well
as our own volunteers.
Heather McKay, MS, OTR/L (pictured above), an
occupational therapist and dementia care specialist,
was the day’s featured speaker. Attendees raved
about the quality and depth of information she
presented, including:
Dementia Facts
l It is not part of normal aging! It is a disease!
l It is more than just forgetfulness, which is part
of normal aging.
l It makes independent life impossible.
l It … includes many cognitive loss conditions
[Alzheimer’s, vascular dementia, Lewy Body

Caldwell Hospice needs licensed hairdressers
and barbers to serve as “special volunteers.”
They take their skills to the homes of our hospice
patients who aren’t well enough to go to a salon.

Board Spotlight on Rob Hinman
Wonderful Differences and Wonderful Similarities
Did you hear the one about the
banker who became a
Presbyterian pastor? If you’re
unfamiliar with the story, this is
how it goes:
Rob Hinman went into banking
after earning his bachelor’s
degree from Wake Forest
University. He was in the profession for 10 very good years; then
he went dove hunting, and everything changed. No earthly birds
were flying that day, but Rob’s
life changed forever. It was “not
the first time I heard the call,”
Rob said, “but this time I knew.” He went home,
talked with his wife, and they moved to Virginia,
where he attended Union Theological Seminary.
After graduating with a Master of Divinity degree in
1990, Rob returned to North Carolina and pastored
the Presbyterian Church in Lowell for eight years and
Bethel Presbyterian Church near Lake Norman for
seven years. Then he came to the First Presbyterian
Church in Lenoir and soon, thereafter, he was invited
to join Caldwell Hospice’s board of directors.
Rob’s father is his model for the ministry; the elder
Hinman, a medical doctor, was available 24 hours a
day, seven days a week. He was one of seven physicians called to Washington, DC, by the late President
John F. Kennedy to serve on a mental retardation
committee. His influence is clear in the younger
Hinman’s community service, as well.
Rob was asked to serve on the board of directors for
Holy Angels in Belmont. He calls his ongoing involvement “one of the greatest joys of my life.” He
believes in the Rotary Club International, too,
although he is an inactive member, currently. He
realized how “international”
its membership is when he
traveled to Ghana while in
the seminary—and attended
a local meeting. They were
collecting money to fund the
eradication of polio, just as
American Rotarians do.
Rob has been a Caldwell
Hospice board member for
six years, and he serves on
the finance committee. He
views hospice care, as a
board member and as a
minister: “The help our
Caldwell Hospice staff and volunteers give makes a
big difference, but the heart of Christ that they share
is life-giving to the families we serve.”
Community work and ministerial duties offer similar
opportunities: to respond to needs and bring groups
together, to appreciate and respect both differences
and similarities in people.
Even in the Hinman household, similarities and
differences are honored—the Wake Forest graduate’s
wife Ginger graduated from NC State, and their sons
attend UNC-Chapel Hill, where one is a freshman and
the other is a junior.
Rob enjoys hunting and backpacking, and he counts
having hiked in Israel—where the terrain climbs from
the Dead Sea at 1,349 feet below sea level to Mt.
Hermon at an elevation of 7,330 feet—among his
accomplishments.
The core of Rob Hinman can be stated in these
words: “I absolutely love Jesus Christ because he
loves me, and I look out for the new person who is
wondering if he or she will be welcome.”
