2 Metro South Health | GP Referral Directory 3 General antenatal referral information This section contains specific information for antenatal shared care at Beaudesert Hospital, Logan Hospital and Redland Hospital. Uncomplicated pregnancies • • • • Ensure all minimum referral criteria (see p. 9) are met including a copy of the required pathology tests: Refer privately for detailed scanning (dating, morphology) Expect the first hospital visit with the consultant to be offered after 20 weeks You will be responsible for the care until she is seen by a doctor at the hospital Note: The mother will be required to see the midwife and the consultant before a booking is confirmed. Complicated pregnancies Please refer urgently to the Antenatal Clinic and specify the reason in the referral letter and include relevant specialist correspondence. The letter will be triaged within the same week. Please ensure you copy pathology results to the hospital clinic directly from the laboratory. High risk of gestational diabetes (GDM): • • • Risks for GDM include: previous GDM or baby over 4000g, polycystic ovarian syndrome, strong family history, glycosuria, or BMI greater than 30, multiple pregnancy and at risk ethnicity (Aboriginal and Torres Strait Islander, Pacific or South Sea Islander, Indian subcontinent, South East Asia, Middle Eastern or African) If there is a high risk, please request a glucose tolerance test (GTT) in the first trimester. Hospital visits Hospital visits usually occur at 36 and 41 weeks (Logan) / 40 weeks (Redland). This will be increased if clinically indicated. At 41 weeks, an induction is booked for approximately 10 days post the estimated delivery date. Metro South Health | GP Referral Directory 4 Early Pregnancy Assessment Unit (Logan Hospital only) Logan Hospital offers an Early Pregnancy Assessment Unit for the assessment of early pregnancy complications. Eligibility criteria Women may be referred directly to the unit if they have: • • a positive pregnancy test or suspected pregnancy, and a pregnancy less than 20 weeks completed gestation, and are experiencing any of the following symptoms: • • • • • pain or vaginal bleeding but are clinically stable no bleeding but with a non-viable pregnancy a confirmed stable ectopic pregnancy to be treated conservatively pregnancy of unknown location, stable and requiring follow up hypermesis gravidarum requiring follow up Accessing the service • • Telephone the Early Pregnancy Assessment Unit nurse on (07) 3299 8456 to book an appointment Follow up with a referral to Dr Mano Haran via fax on (07) 3299 8963. General referral criteria Referrals to specialist outpatient services within Metro South Health must be in writing, and as a minimum contain the following: • • • • • • • • • Patient demographics including Medicare number and Interpreter requirements Patient’s mobile phone contact number and an alternate postal or contact address (if not the same as usual residence) Reason for referral (including the problem to be assessed, degree of loss of function, pain experienced) Results of relevant investigations (pathology, radiology, histology) Current medications and doses, prescribed and over the counter (note any recent changes in drug therapy) Allergies (drug/topical preparation) Relevant information about the patient’s condition such as previous medical/surgical treatment (include systemic and topical medications prescribed for the condition) and details of any associated medical conditions which may affect the condition or its treatment (e.g. diabetes) Relevant psychological and social issues impacted by condition Smoking and alcohol history. This means that your patient has the right to choose to be treated as a private patient within any facility listed in this guide and can be bulk billed for the service with no gap payment. These funds will allow for the delivery of improved clinical services to the local community and your patients. If seen as a bulk billed (private) patient, the benefits will be that: • • your patients will have access to one of our staff specialists with right of private practice, and the consultation and investigations performed will be bulk billed. Eligibility criteria All new referrals are to be directed to the local Acute Care Team. It is very important to indicate if you think an urgent response is required (e.g. immediate, within 24 hours, etc.) and why. If relevant, referrals should involve the person’s appointed Guardian or Attorney for Health Matters if it is clear that the person has lost the ability to understand and make healthcare decisions. If an individual is admitted to an acute inpatient unit it is often because their mental illness is not manageable in a less restrictive setting, such as community based support. It is preferable that consumers are admitted to inpatient services voluntarily, however on occasion admission under the Mental Health Act 2000 may occur. The Mental Health Act 2000 provides for the involuntary assessment and treatment, and the protection, of persons with mental illness. Primary, secondary and tertiary consultation is provided at consumer, carer, clinician, support worker and organisational levels. Includes specialist functions delivered in relation to service development, workforce education and early intervention. Eligibility criteria Referrals can be made by self, family, friend, multicultural sector agency, mental health service, other health services, or any other service provider in consultation with consumers from all age groups and where possible their family. Referral is for assessment of complex mental health problems, particularly where there are significant cultural barriers and/or mental health problems that arise from complex interactions of settlement and adjustment issues and pre or post migration trauma.
