Passport checklist (http://travel.state.gov/passport) 1. Complete form DS-11 Application for U.S. Passport a. If this is your first passport - you must apply in person at a passport agency with all of your forms, picture and fees b. If you are renewing you can do that by mail, you need form DS-82 Application for U.S. Passport by Mail 2. Submit evidence of U.S. Citizenship a. Certified birth certificate - must have the raised, embossed seal on the document b. Naturalization certificate c. Certificate of Citizenship d. Expired Passport 3. Submit Identification a. Previously issued U.S. passport b. Naturalization certificate c. Valid driver's license d. Government ID e. Military ID 4. Provide 2 passport photos a. Must be identical, in color, 2X2 in size and taken within the last 6 months. It must be full-face front view with normal clothing attire (no uniforms, hats, dark glasses). b. Walgreen's or any store that has an in-house photo area can take your picture. 5. Make appointment with the Passport Office (330-438-6432) at the Canton Post Office at 2650 Cleveland Ave. next to Johnson Center. 6. Pay fees ($100.00 - includes passport fee of $75.00 and execution fee $25.00) 7. Wait for the passport to come in the mail. Due to new safety precautions within the U.S. government your passport may take up to 8 weeks to arrive. 8. All forms are available online to print and complete.
