COMMUNITY RESOURCE GUIDE HELPFUL TIPS Things to know if you are making a referral: 1. 2. 3. 4. Know and understand the person's needs. Know what services are provided by the agency to the best of your ability. Find out the eligibility guidelines that the person must meet in order to obtain services. Make sure you have the person's approval, assure them of confidentiality and advise the person of what information you will be providing. If you are seeking help: Key points to remember include: 1. Write down name and phone number for EVERYONE you talk to and keep a file of these contacts regarding each issue. 2. Be honest when asking for help and ask questions. If there is anything you are unclear about (procedures, requirements, eligibility, etc.) make sure you ask someone. 3. Keep calling. If you leave a message and have not heard back in 24-48 hours Call Again. 4. If you are referred to an agency, ask what information you need to bring when you go to the agency. Referral Services Coalition for the Homeless http://louhomeless.org th 1300 South 4 Street, Suite 200, Louisville, KY 40208 (502) 636-9550 The Coalition for the Homeless strives to prevent and eliminate homeless in Louisville through advocacy, education and the coordination of 30 member agencies that provide housing, food, clothing, health care and legal services to the homeless – men, women, children, families and veterans. They provide centralized reservations of beds for the night and a centralized waiting list for housing for homeless families. Bed One Stop is the centralized way to secure a bed at any of Louisville’s emergency centers for the night. Homeless persons can go to 1300 South 4th St., Suite 200 Monday through Friday from 10:00 AM – 1:30 PM to reserve a bed or they can call (502) 637-BEDS (2337) from 10:00 AM -2:00 PM seven days a week. Women and families experiencing homelessness find "A Place to Call Home" through Norma's House, the Marijane Toney House, Martie's House, and Single Supported Housing on Muhammad Ali Blvd. In addition to permanent supported housing, Choices, Inc. also provides case management to each client. Each resident attends life skills training and house meetings while in the program. Open 24 hours, 365 days a year. They also have short-term housing for homeless men, women and children while they seek long term housing solutions. These services include short or long-term residential care, a home for pregnant and parenting teens, crisis shelter services, and therapeutic loving foster care services. The Home operates Kosair Charities Pediatric Convalescent Center for medically fragile children, who are dependent on technology to sustain life, as well as children who are terminally ill. In-home and in-school support services are provided for children with autism. The Home provides these families with a wide-range of services and effective treatment with lower overall costs. Home of the Innocents is the state’s largest emergency placement center. That means our staff is ready 24 hours a day to offer a safe place for children who have been removed from their homes because of abuse, abandonment or neglect. The Home cares for infants to age 18. Interlink Counseling Services http://www.interlinkservices.org/ 8311 Preston Highway, Louisville, KY 40219 (502) 964-7147 Interlink Counseling Services is the largest provider of transitional housing for Veterans in Kentucky. Interlink provides a comprehensive plan of integrated care for individuals and families suffering from an addiction or a mental health issue. It provides chemical dependency treatment and education through in-patient transitional residency programs and outpatient programs. Additionally, Interlink provides outpatient services such as marriage and family therapy, group counseling, DUI offender education and case management. Interlink transitional recovery housing for veterans include: Landing Zone – a safe-off-the-streets recovery program for homeless men to detox from alcohol or drug addiction Harmony House Residential Treatment – a 3 to 6 month recovery program to stabilize physical and mental health issues which includes intensive case management support Genesis House Transitional Living – up to 24 months transitional housing that includes education in life skills and relapse prevention sessions Serenity Court Apartments – for people who complete a defined treatment program with Interlink and who want to live in an alcohol/drug free residence Louisville Metro Housing Authority http://www.lmha1.org/ 420 South 8th St. Louisville, KY 40203 (502) 569-6060 The Louisville Metro Housing Authority (LMHA) goal is to provide quality, affordable housing for those in need, assist residents in their efforts to achieve financial independence, and work with the community to strengthen neighborhood. Guests can get breakfast, sign up for mail services, securely store their belongings or take showers. The shelter's hallmark is a commitment to serving the whole family in one location. The program also provides individualized case management, housing placement, and other services to help families transition back to the community and become self-sufficient. The Family Transitional Housing Program offers a place for the family to live for two years after leaving the Volunteers of America’s Family Emergency Shelter. The program offers intensive case management, addressing needs in the areas of housing, employment, education, mental and physical health, child-related issues, finances, and support services. Case management is tailored to each family’s needs and the family participates in all goal setting. Families in the Transitional Housing Program pay a service fee, which is equal to 30 percent of their income. Financial assistance is available to aid families with work, education, or training expenses, housing-related start-up expenses, transportation, child care, medicine, clothing, and other expenses that promote greater client self-sufficiency.
