Since we opened our doors on January 9, 1968, we have been honored to serve thousands of individuals and their families as they travel through the aging process. What an amazing journey these last 45 years have been. What began as a 200-bed single campus in the northwest suburbs has transformed into an organization that offers a true continuum of care on campuses across the metro area, as well as in individual homes. As the years past, our traditions continue to remind us who we are and where we came from. We hold steadfast to our traditions of insight, faith, volunteerism and generosity. In the 2012-13 annual report we spend time reflecting on the traditions of Saint Therese. You will read about how these traditions helped form the organization we are today. We are grateful to the friends and families who have celebrated these traditions and helped us create new ones. MISSION STATEMENT: Saint Therese, a Catholic organization serving seniors of all faiths, is dedicated to the well-being of each individual. Thank you for your support of the individuals we serve. Warmly, Barbara A. Rode President & CEO Kaye Foley Foundation Board Chair 1 Year at a Glance 2012-2013 Over the last year, Saint Youth Volunteer Scholarship Making a Splash Therese has seen great Recognizing youth volunteers serving older adults change. We are continually Last spring, Saint Therese launched the first Youth Volunteer Scholarship event honoring youth who serve older adults. The scholarship was created to recognize intergenerational connections and encourage other youth to engage in meaningful relationships with seniors. During the event four individuals were awarded $1,000 educational scholarships based on a written essay about their experience volunteering with an older adult. Residents and tenants dive into new pool at Oxbow Lake responding to the growing and shifting needs of the individuals we serve. The following articles are just a few highlights from the past year at Saint Therese. Saint Therese President and CEO Barb Rode, left, and Executive Director John LeBlanc, right, award Carly Koch with a $1000 scholarship. 2 Dorothy Zelenak, supported by Aquatic Therapist Stacey Neville, was the first tenant in the pool at the Oxbow Lake campus. Saint Therese at Oxbow Lake opened the warm-water aquatic pool in the fall of 2012 and Dorothy Zelenak was there. Dorothy, an avid swimmer at the local YMCA, was thrilled to learn the Brooklyn Park campus would be opening a pool for residents and tenants, and she wanted to be first in the pool. When the day arrived, Dorothy was smiling from ear to ear. The pool offers a variety of individual and group classes, as well as open swim for residents, tenants and local community members. Residents, families and staff are now able to enjoy the peaceful surroundings of Saint Therese at the St. Odilia community. On January 9, 2013 Saint Therese celebrated 45 years of care and services. A large group of Saint Therese staff gathered to take a picture to mark the special event. A Peaceful Retreat Landscaping project at Shoreview campus promotes the healing power of nature Throughout 2012 and 2013 Saint Therese at St. Odilia raised funds to enhance the landscaping surrounding the small community. With an outpouring of support from Saint Therese donors and the Catholic Community of the St. Odilia parish, the landscaping project was completed in the summer of 2013. The project includes a patio, walking path and a gazebo for individuals and families to enjoy. Celebrating 45 Years Honoring decades of compassionate care On January 9, 2013, we celebrated 45 years since the first resident of Saint Therese walked into the front Two members of the Ed Hudoba family team watch as their teammate makes his best putt. doors at the New Hope campus. And the rest they say—is history. For over four decades Saint Therese has served thousands of individuals and their families as they journey through aging. As our mission expands beyond our walls and into the greater community, we look forward to another 45 years of service. Swinging at Success Golf Classic raises $35,500 for Saint Therese Golfers enjoyed one of the first beautiful days of summer at Wild Marsh Golf Club in Buffalo on June 17, 2013. The 11th Annual Golf Classic raised $35,500 for Saint Therese care and communities. The golf event offered several games and contests throughout the day in which attendees could participate, with plenty of prizes for everyone. For over four decades Saint Therese has served thousands of individuals and their families as they journey through aging. 3 A Tradition of Insight The visitor parking lot at Saint Therese of New Hope in 1972. 4 On a brisk January day in 1968 a local woman by the name of Mary Zepp walked into the front doors of Saint Therese Home in New Hope, Minnesota, fulfilling the hopes and dreams of a special group of people. The mere presence of Mary turned the idea of Saint Therese into a reality. Prior to 1968, Saint Therese Home was simply a vision in the minds of founders Jerry Choromanski, Father Gordon Mycue, Sister Marcelline Jung and Elizabeth Hidding. The four visionaries had a mountain to climb before their dream of quality Catholic senior care in the northwest suburbs would be realized. At a time when nursing homes consisted of eight to 16 bed wards, Saint Therese planned to step out of the institutional box and create a nursing home that offered a bit of home. Instead of beds placed in lines in a large auditorium-style room, two individuals would share a room, with a door, a vanity and an adjoining bathroom. Instead of a large stately building amongst a concrete landscape, Saint Therese would sit nestled amongst the grandeur of nature with a peaceful outdoor setting for individuals and families to enjoy. As those four founders considered the future of the organization, the tradition of insight at Saint Therese began. Since Mary Zepp, Saint Therese has seen great changes in the senior care industry, often times leading the conversation for change. In the mid-1990s Saint Therese was one of the first nursing homes in the Midwest to create a Palliative Care wing. Palliative care can be provided to anyone, at any time, experiencing a terminal chronic illness. Launched by health care innovators, the Saint Therese of New Hope campus’ palliative care wing helped to change the perspective of caring for older adults as they begin their end-of-life journey. During a simple survey in 2006, Saint Therese learned that older adults living on campus had a great fear of falling. In response to the survey, Saint Therese Rehabilitation and Fitness, now STR, created a balance initiative that focuses on preventing and treating balance disorders through education, exercise and treatment. Each Saint Therese community now features an extensive balance program which utilizes one of the top balance treatment pieces of equipment in the nation. Developed by NASA, the NeuroCom SMART Equitest Balance machine provides objective data to assist therapists as they develop action plans for older adults who are concerned about their balance. Saint Therese has been blessed by insightful individuals who see a need and respond. Although we have many notable accomplishments, our tradition of insight can be seen each day in the care we provide to each individual. At a time when nursing homes consisted of eight to 16 bed wards, Saint Therese planned to step out of the institutional box and create a nursing home that offered a bit of home. We invest in our programs and services, our buildings and our staff. We take special note to invest in areas that will make the biggest impact for the individuals and families we serve. Our investments are often led by the thoughts and innovations of our board and staff, but it is the generosity of others that makes it possible for us to fulfill our mission. Seeing a need for Catholic senior care, our founders created an organization that serves seniors no matter what their financial status. As our founders oversaw the construction, ordered the equipment and ensured all the requirements were met, the sisters from St. Paul’s Priory began serving with a generous spirit. Before each resident moved into a room at Saint Therese they were visited in their own home by a sister. In 1968, Saint Therese was a 200-bed community and there were many people to visit. The sisters visited with each resident to not only give them an idea of what to expect, but they also wanted to get to know the resident. Each offering their own personal touch, the sisters wanted to make sure each individual would know someone on their first day at Saint Therese, making that day a little more comfortable. When family members face a loved one’s diagnosis of Alzheimer’s or dementia, Saint Therese offers a support group hosted on campus. The support group is provided during non-business hours so families can attend and is led by a staff member on her own time. As with their own families, Saint Therese employees are generous in the care and support of the individuals and families we serve. In the fall of 2012 the Saint Therese Board of Directors reaffirmed their commitment to support older adults with limited resources by establishing The Monsignor J. Jerome Boxleitner Endowment Fund. A founding board member of Saint Therese, Monsignor J. Jerome Boxleitner was a passionate advocate for the poor and underserved for over 50 years. The new fund provides support to older adults who have outlived their resources, allowing them to remain at Saint Therese. The Saint Therese Foundation raised funds for the endowment at the 23rd Annual Associate Dinner in 2012. Inspired by a compassionate speech made that evening by Msgr. Boxleitner about giving back, social justice and the future of those less fortunate, the generous attendees of the dinner raised over $146,000 for older adults with limited resources, one of the highest amounts ever garnered at an associate dinner. Our tradition of generosity comes from many sources, especially people like you. Seeing a need for Catholic senior care, our founders created an organization that serves seniors no matter what their financial status. TOP Monsignor J. BOTTOM RIGHT Through special funding, the Saint Therese of New Hope grounds recently received a face-lift that included a new paved pathway, landscaping and fence along the pond. 7 A Tradition of Faith Sisters from St. Paul’s Priory in 1966. 8 Faith has always been an honored tradition at Saint Therese. As the health care industry has changed over the last 45 years with new technology and innovation, faith has remained an influential pillar of care at Saint Therese. Saint Therese founder Jerry Choromanski relied on his faith and sought the guidance of St. Therese of Lisieux as he planned, researched and built the Saint Therese organization. Sisters from St. Paul’s Priory were the first caregivers to serve the residents at Saint Therese, offering medical and spiritual support. A small convent was built on-site for the sisters to find respite following 14- to 16-hour days serving individuals in the care center. Just as they offered faith to older adults, they leaned on their faith for the strength to continue to support others. Over the years, with each new addition and new phase of the Saint Therese organization, our leaders have relied on their faith to guide and uphold us. From the late 70s, when the Saint Therese of New Hope apartment building was breaking ground, to 2006 when the Oxbow Lake campus was being built, Bishops from the Archdiocese of St. Paul and Minneapolis were present; blessing the grounds, the building and the future inhabitants. Faith has always remained at the forefront of our thoughts. However, the face of faith has changed. Over the years, the religious demographic at Saint Therese has expanded well beyond our Catholic beginnings. While we honor our Catholic faith, we also recognize and respect the variety of faiths served at Saint Therese. With one of the largest senior care pastoral teams in the area, individuals seeking spiritual guidance at Saint Therese will find support through an eclectic group of spiritual servants. Our Catholic population is served with the assistance of deacons, priests and sisters from a variety of religious orders, as well as some of the original sisters from St. Paul’s Priory. Saint Therese has also been blessed with other spiritual leaders consisting of chaplains, a Baptist minister and a Lutheran pastor. From one on one visits to group bible studies and daily worship services, individuals from all faiths can find spiritual support as they journey through aging. As Saint Therese continues to grow and expand, so does our tradition of faith. The face of faith has changed. Over the years, the religious demographic at Saint Therese has expanded well beyond Catholic. TOP We are guided by our faith in everything we do. Guests to the private care suites at the Oxbow Lake campus are greeted by our commitment to faith with each visit. BOTTOM RIGHT The pastoral care team features an eclectic blend of spiritual servants from a variety of faiths. 9 A Tradition of Volunteerism Saint Therese employee Pauline Stassen, left, is pictured with a fellow candy striper and a resident in 1972. 10 Volunteers at Saint Therese have always had important roles to play. From candy stripers and Auxilians to school groups and local bands, volunteers bring a level of warmth and energy that enhances the quality of life of the individuals served by Saint Therese. Before any of the Saint Therese buildings or campuses existed, volunteers were a driving force in the organization. In the summer of 1967 a special group of women gathered to create the Saint Therese Auxiliary. Auxilians not only volunteer their time, but they are dedicated fundraisers who have helped Saint Therese purchase critical care center nursing equipment and buses, as well as operate an on-site gift shop. In the 1970s Saint Therese’s volunteer program included young adults in candy striper uniforms who supported residents with their daily lives. In her early teens Saint Therese employee Pauline Stassen was a candy striper. For a couple hours each week Pauline would ride her bike down to the Saint Therese of New Hope campus to help out with various daily activities. Dressed in a red and white striped dress, Pauline and a few of her friends would visit with residents, feed them lunch and play card games. In the 70s candy stripers were irreplaceable at Saint Therese. Today, there are many events and activities that simply would not be possible without the support of volunteers. Events and activities like card making, rug weaving, Happy Hour socials and bingo are all supported by volunteers. Volunteers also help with mass and other worship services. “Without volunteers we wouldn’t have readers, cantors, escorts, Eucharistic ministers or organists to assist with services,” said Matt Crawford, Director of Volunteer Services. “Over the years Saint Therese has been honored with an amazing group of volunteers who are committed to our mission.” Individuals like Dick Macko. Dick first played an instrument at Saint Therese back in the late 60s at an ice cream social for a friend. As his popularity grew at Saint Therese and in the larger community the Dick Macko Band was a hot commodity, but Dick always kept Saint Therese in his heart. Dick and his band have played for the residents and tenants at Saint Therese for over 45 years. Dick knows many of the tenants and will play their favorites including “Edelweiss,” “In the Mood” and “Under Paris Skies.” “It’s fun, I enjoy being here,” Dick said. “The people are great and I enjoy making them happy.” Volunteers at Saint Therese provide a level of support that cannot be matched by any other department in the organization. Volunteers provide the extras in life that make Saint Therese such a wonderful place to call home. Without volunteers we wouldn’t have readers, cantors, escorts, Eucharistic ministers or organists to assist with services. TOP Volunteer Harry Hurley, right, leads a golf clinic at Saint Therese. Some traditions are connected to events and activities, while others represent an exceptional care or service Saint Therese provides. The tradition could be the celebration of an “angel” for the “Angels Among Us” dinner or the tradition could be an expanded level of care that launches a department into a division such as Saint Therese Rehab. Most often, the tradition begins when an individual or a group proposes an idea that takes on a life of its own. In 2011, a member of the Associate Dinner committee said, “To keep me involved, get my kids involved.” This statement sparked an idea. The committee decided something was needed to engage and recognize youth volunteers at Saint Therese. A scholarship program was developed to encourage intergenerational relationships and recognize youth who volunteered with older adults. Individuals, between the ages of 12 and 18, submitted an essay about their experience volunteering with an older adult. Two recipients were chosen and awarded $500 scholarships. Blessing Ogunyemi, 14, and Jacob Shrode, 18, were the first two recipients of the Saint Therese Youth Volunteer Scholarship. Blessing spoke about her volunteer experience at the Associate Dinner in 2011. “Volunteering at Saint Therese was a wonderful opportunity for me to not only help out in the community, but to also get to know and learn things that I probably wouldn’t have learned,” said Blessing. “I started thinking of what I want to do when I grow up and where I want to have gone and the places I want to have visited and the people whose lives I want to have helped and impacted.” In 2012 the Youth Volunteer Scholarship was expanded and separated from the Associate Dinner into its own event. Participants were invited to submit an essay or illustration—many inspiring entries were received. In April 2013 approximately 50 friends, families, volunteers and staff gathered to recognize youth volunteers who made a difference in the lives of older adults. Four youth were each awarded a $1,000 educational scholarship. And just like that, a new tradition has begun. To learn more about the program or to apply, visit www.sttheresemn.org/youth-scholarships. New traditions begin when an individual or a group proposes an idea that takes on a life of its own. The Saint Therese Foundation supports the mission of Saint Therese by raising funds to enhance services, programs and communities. Financial gifts help provide pastoral care, nursing scholarships and many capital improvements. Each year, the Foundation organizes three major fundraising events: Associate Dinner, Golf Classic and Tree of Lights. These events are a great way to introduce individuals to the work of Saint Therese and raise much-needed funds. For those whose goals include something more personal, we offer the opportunity to establish a named endowment fund. For a minimum gift of $25,000 in either a current, pledged or deferred gift, a donor may establish a permanently endowed, named fund. The fund is maintained in perpetuity, and can be named to reflect the wishes of the donor. Those who wish can restrict the earnings of this fund to support a specific program or purpose. Donors who establish a named endowment become members of our Pillar Society at one of these levels: 22 Founder $1,000,000 and over in cash, pledge or deferred Builder $500,000 - $999,999 in cash or pledge $750,000 - $999,999 deferred Trustee $250,000 - $499,999 in cash or pledge $500,000 - $749,999 deferred Patron $100,000 - $249,999 in cash or pledge $200,000 - $499,999 deferred Benefactor $ 50,000 - $99,999 in cash or pledge $100,000 - $199,999 deferred Pillar $ 25,000 - $49,999 in cash or pledge $ 50,000 - $99,999 deferred Saint Therese Endowment Funds The Saint Therese Pastoral Care Endowment Fund The Monsignor J. Jerome Boxleitner Endowment Fund The Board Quasi Endowment Fund The Jerome and Ursula Choromanski Pastoral Care Fund The Edward and Julie Hudoba Family Fund The Mathilda Newinski Family Fund The Frances and Munn Scherer Family Pastoral Care Fund The Vandergon Family Fund for Oxbow Lake Saint Therese Organizational Endowments The Saint Therese Pastoral Care Endowment Fund Since Saint Therese’s inception, pastoral care has been a core value shared with the individuals and families we serve. The Saint Therese Pastoral Care Endowment Fund was established by the board of directors in 2008 to support the unique programming offered by on-campus clergy, religious leaders and lay ministers. Gifts to this fund will allow Saint Therese to provide quality spiritual support to individuals at each Saint Therese campus for years to come. The Monsignor J. Jerome Boxleitner Endowment Fund A founding Saint Therese board member, Monsignor J. Jerome Boxleitner was a passionate advocate for the poor and underserved for over 50 years. The Monsignor J. Jerome Boxleitner Endowment Fund was created in the fall of 2012 to support older adults who have limited resources. Gifts to this fund will enable Saint Therese to serve individuals who have outlived their resources, allowing them to continue to live with dignity at any stage in the aging process. The Board Quasi Endowment Fund At Saint Therese we rely on the generosity of our donors to support our mission to serve older adults as they journey through the aging process. Often times donors give to a specific project or event close to their hearts. In the event a donor has not restricted their estate or tribute gift for a specific purpose, those gifts are added to the Board Quasi Endowment Fund. The fund was created by the Saint Therese board of directors in 2008 to ensure special initiatives can be enacted through full financial support now and into the future. Saint Therese Named Endowment Funds The Mathilda Newinski Family Fund A long-time resident of the Saint Therese of New Hope care center, Mathilda Newinski was a beloved mother of seven children who adored her. Since her passing in 1993 the Newinski family has remained involved through a variety of activities and events. In 2010, in honor of Mathilda, the family established the Mathilda Newinski Family Fund. The Mathilda Newinski Family Fund supports a variety of care and services offered by Saint Therese. The Jerome and Ursula Choromanski Pastoral Care Fund The Frances and Munn Scherer Family Pastoral Care Fund Guided by his Catholic faith and devotion to St. Therese of Lisieux, Saint Therese Founder Jerry Choromanski was looking for a way to support older adults as they journeyed through the aging process when he founded Saint Therese Home, Inc. in 1964. Over 45 years later, Jerry, along with his wife Ursula, continue to provide leadership and guidance to Saint Therese. Guided by their faith and a calling to serve others, the Scherer family has supported the mission of Saint Therese through their time, talents and financial contributions for over 40 years. In 2009, the Frances and Munn Scherer Family Pastoral Care Fund was created to ensure financial stability for pastoral care at Saint Therese. Established in 2009, the Jerome and Ursula Choromanski Pastoral Care Fund provides Catholic spiritual care to the individuals served by Saint Therese now and for generations to come. The Edward and Julie Hudoba Family Fund A seasoned Saint Therese family, the Hudobas have seen the Saint Therese organization grow from a single campus to four campuses and multiple support services. In 2008 Edward created the Edward and Julie Hudoba Family Fund in gratitude for the care and support offered by Saint Therese. The Frances and Munn Scherer Family Pastoral Care Fund benefits the individuals and staff who provide an array of spiritual care support services at Saint Therese. The Vandergon Family Fund for Oxbow Lake Touched by the sense of community on the Saint Therese at Oxbow Lake campus, Donald Vandergon created the Vandergon Family Fund for Oxbow Lake in 2012. The Vandergon Family Fund supports the Oxbow Lake community to further programming and to ensure the long-term financial strength of the campus. The Edward and Julie Hudoba Family Fund’s earnings are designated to support the buildings and grounds for Saint Therese campuses. 23 Financials Consolidated Balance Sheet St. Therese Foundation, Inc. and Affiliates Financial Summary For the fiscal year ended June 30, 2013 Assets Current Assets Assets Limited as to Use Property and Equipment Other Assets Total Assets $ 10,276,398 7,562 45,613,246 20,551,799 $ 76,449,005 Liabilities and Net Assets Current Liabilities Long-Term Liabilities Net Assets Total Liabilities and Net Assets $ 5,008,266 38,260,251 33,180,488 $ 76,449,005 4% 2% 4% Sources of Revenue and Support 90% Resident Service Revenue Other Revenue, Net Contributions Investments Consolidated Statement of Operations 9% 59% 11% How Dollars Are Spent Program Expense Plant and Property Expense General and Administrative Payroll Taxes and Employee Benefit 12% Depreciation and Interest 9% Please contact Mike Warden, CFO, at 952.283.2204 with questions regarding this financial summary. As a non-profit organization, Saint Therese strives to help others within our neighborhoods and beyond. We believe it is also important to communicate our outreach and support of the greater community as part of our mission. Documenting this work is an essential element for earning public trust and validating our status as a non-profit organization. Activities for the 2012-13 fiscal year are listed below: • To foster education and help train future leaders in our industry, we contribute $22,500 a year to the University of Wisconsin— Eau Claire to sponsor a student administrator. • Saint Therese is a host for social worker students from the University of Minnesota Twin Cities campus. • Spiritual care is at the center of our mission. Our extensive pastoral care services include staff, liturgical costs, music and much more. This vital program is paid for by donations to Saint Therese. • Saint Therese employees were awarded over $11,780 in scholarships through the Saint Therese Foundation’s White Rose Scholarship. The scholarship provides the opportunity for staff to continue their education and improve their knowledge on caring for older adults. • Our staff recruited blood donors for the Red Cross. Staff members volunteered their time and donated blood for this worthy cause. • Our staff participated in trade and professional associations to expand their skills and to advocate for social fairness. This is also a way we can share our experiences and knowledge to benefit others. • Saint Therese hosts a variety of educational opportunities for local administrators, social workers and other staff to earn continuing education units. • Tenants and staff members in our communities hold a food drive at various times throughout the year to help families in need in our surrounding communities. Saint Therese donated to various community partners to strengthen their work. This year’s partners included: North Memorial Medical Center, North Memorial Community Foundation, Catholic Community Foundation, Catholic Eldercare, The Power of Possible UW-Eau Claire, Our Lady of Peace, Church of St. Vincent, St. Gerard, St. Raphael’s, North Memorial Hospice Golf Tournament, St. Joseph’s Parish of New Hope, St. Paul’s Monastery, Aging Services of Minnesota, Empty Bowl Fundraiser for the PRISM and NEAR Food Shelves, Alzheimer’s Association and Twin West Chamber of Commerce. Closing the Gap Saint Therese provided over $2.5 million of free care as a result of Medicaid reimbursement and State allowed rates not covering the entire cost of care. Approximately 44% of our residents participate in the Medicaid program. In addition, another 35% of our residents pay for their care on their own. For both of these groups, the amount we are reimbursed or are allowed to charge is $32 a day less than the actual cost of providing the care. If we were to make cuts to recover these expenses, we would compromise care of our residents. Caring Fund Supports Tenants Saint Therese serves a variety of individuals from an array of financial backgrounds. Due to changes in the economy, early retirement or simply living longer than expected, many seniors have run out of their resources and are in need of financial assistance. The Caring Fund supports tenants with additional services. 25 Saint Therese Care and Communities Saint Therese of New Hope 8000 Bass Lake Road New Hope, MN 55428 763.531.5000 Saint Therese at Oxbow Lake 5200 Oak Grove Parkway Brooklyn Park, MN 55443 763.493.7000 Saint Therese at St. Odilia 3500 Vivian Avenue Shoreview, MN 55126 651.842.6780 Saint Therese of Woodbury Welcome Center 2165 Woodlane Drive, Suite 101 Woodbury, MN 55125 651.714.7144 Saint Therese Rehab 8000 Bass Lake Road New Hope, MN 55428 763.531.5435 At Home Solutions by Saint Therese 1660 South Highway 100, Suite 103 St. Louis Park, MN 55416 952.283.2220 Saint Therese Foundation Contacts Barbara A.
