butt,” Rambert says. “It’s the most fun part.”
Rambert, Lassen and Tobey are part of a growing corps
of senior fitness enthusiasts. Some, like Tobey, are continuing a life-long love of exercise. Others, like Lassen and
Rambert, have embraced physical activity later in life and
under doctors’ orders.
“I have arthritis, and my rheumatologist suggested that
water therapy would help me,” says Rambert. “I found the
Main Line Y’s class. Back then, it was called ‘Hinges and

This 82-year-old grandmother may knit and bake cookies—but she also does yoga. Jacqueline Lassen attends a
weekly class at Wayne Presbyterian Church, plus aerobics
four times a week. Grandfathers around here are surprisingly active, too. Raymond Tobey, age 80, hikes the trail at

Twinges,’ which is just awful. But I started
coming. I did feel some improvement, so
I kept at it.”
That was 29 years ago. Rambert got
certified by the Arthritis Foundation to
lead the class and teaches it twice a week.
“Knees, hips, elbows, whatever,” she says.
“The warm water helps everything—I can
do exercises that I couldn’t do on land.
Our bodies have more buoyancy in water.
It provides natural resistance that works
our muscles without straining them.”
Other anti-arthritis pool programs are
held at the YMCA of the Upper Main
Line in Berwyn, which also has aqua tai
chi. Regular tai chi is also great exercise
for older bodies. “When I started here 20
years ago, we had maybe 30 people every
day,” says Susan Shapiro, executive director of the Wayne Senior Center, which
serves Montgomery, Delaware and Chester
counties. “Now, it can be anywhere from
50 to 100 people. That’s not all related to
exercise, but it’s a lot of it.
“Exercise helps with two things that
most older people are afraid of: falling and
cognitive decline,” Shapiro says. “And
although everyone knows that is part of
the purpose, we don’t position exercise as
a thing that people have to do. We make
it fun. We have a wide variety of classes
that are on the cutting-edge of fitness and
turn exercise into something people want
to do.”
The physical benefits of exercise are
many. “Joint movement, weight loss,
cardiovascular improvements,” says Mary
Francis Reilly, executive director of Main
Line YMCA. “And there are just as many
mental benefits.”
Research backs Reilly’s statement.
In 2011, Brown University’s Alpert Medical School and the University of Illinois
showed that aerobic exercise three times
per week increases the levels of BDNF, a
protein in the brain that regenerates the
hippocampus, which is responsible for
forming new memories.
Elsewhere, a study run by the Boston
University School of Medicine, New York
Medical College and Columbia College
of Physicians and Surgeons reported that
seniors who did yoga altered their levels
of gamma amino-butyric acid, which is
connected to stress-related illnesses like
depression, anxiety and high blood pressure. UCLA’s Department of Psychiatry
and Biobehavioral Sciences showed that
tai chi has many of the same effects.

“I absolutely believe that there is a
mind-body connection,” says Shapiro. “If
nothing else, doing things like yoga and
tai chi proves that you can—and that’s
empowering.”
Regular exercise is new for Lassen.
“If you told me 20 or 30 years ago that I
would do yoga and love it, I wouldn’t have
believed you,” she admits.
Lassen’s activities were limited to
recreational swimming and tennis for
most of her life. Then she was diagnosed
with diabetes at the age of 75. “The doctor said exercise would help control the
diabetes—and it certainly has,” she says.
“And if I’d known how much I enjoy
exercise, I’d have done more of it over the
years. I encourage other older folks to get
out there and find what works for them.
It’s use it or lose it, right?”
Lassen is using it so much that she won
the women’s 2012 Fitness Award from the
Wayne Senior Center. The male award
went to Tobey. His fitness routine would
put to shame someone half his age. “I do
the weightlifting machines and the treadmill at the center three times a week. And
four times per week, I walk the track at
Haverford College or hike with a buddy,”
says Tobey. “At least four times a week, I
do laps in the pool at Villanova.”
Tobey gets his swimming in via the
Friends of Villanova program. For a $25
annual fee, Radnor Township residents
can access the university’s pool, fitness
centers and outdoor tracks. “We don’t
have classes specifically for seniors,”
says Lisa Harris, Villanova’s director of
intramurals and recreation. “But many
township residents choose to take advantage of the program and use our state-ofthe-art facilities.”
Senior who are residents of townships throughout Montgomery, Delaware
and Chester counties can access facilities at recreations centers, high schools
and senior centers—either for free or for
minimal fees. Programs and costs vary
by township.
“There are things that are not terrible
about being older—and getting discounts
is one of them,” says Tobey.
A medical doctor before he retired,
Tobey is well aware of the benefits of exercise. But he does it because he enjoys it.
“I turned 80 in February, but I say that
I’m barely into my 70s,” he says. “This
winter, I’ll be skiing with my grandchilHG

Boning up on osteoporosis treatments.

Confused and concerned about calcium,
bone health and osteoporosis medications
like Fosamax, Prolia and Boniva?
“It’s the number one thing that my
post-menopausal patients are asking
me about,” says Dr. Christine Ellis, an
OB/GYN with Associates for Women’s
Medicine in West Chester and Exton.
“There’s a lot of information out there,
but not all of it is correct.”
Every patient’s needs are different,
but one thing’s for sure: “We’re looking at
a huge population of baby boomers and
their parents who are dealing with osteoporosis,” says Dr. Gary Gordon, chief of
rheumatology at Lankenau Hospital.
The National Osteoporosis Foundation
reports that 10 million Americans have
the disease and 34 million may develop
it. Both men and women are at risk.
The NOF estimates that, of people
older than 50, half of all women and one
in four men will break a bone because
of osteoporosis.
In layman’s terms, bones are made of
calcium phosphate, collagen and cells that
slough away old cells and replace them
with new ones. This process happens
with human bodies of every age and both
genders. But, as we age, the regenerationreplacement process slows, resulting in
less dense, more fragile bones.
That slowing is exacerbated in postmenopausal women because of their
estrogen loss, which depletes bones of
that hormone’s calcium absorption benefits. How much the bones are affected
depends on how strong they were to
begin with, which is why doctors firmly
recommend prevention as the first wave
of treatment. But one of the problems
with prevention is that people are not
getting sufficient amounts of calcium.
In 2011, the National Academies’ Food
and Nutrition Board issued its recom-

mended daily allowances for calcium:
1,000 mg for all adults 19-50, plus an
increase to 1,200 mg when women turn
51, and an increase to 1,200 mg when men
hit 71. According to the USDA, eight
ounces of plain, low-fat yogurt has 415 mg
of calcium, one cup of cottage cheese has
138 mg of calcium and a half-cup of broccoli has 21 mg of calcium. Together, that
equals only half of the RDA for calcium
for women ages 19-50.
That’s why supplements are called for.
But recent studies show that calcium
supplements might have harmful effects.
Two long-term studies—one by the University of Auckland and one from the
European Prospective Investigation Into
Cancer and Nutrition—report higher
rates of heart attack in people who took
calcium supplements.
“It’s not how much calcium a person
takes, it’s what they absorb,” says Mark
Stayton, sales manager at Martindale’s
Natural Market in Springfield.
And to absorb calcium, you need
vitamin D and magnesium. “That’s how
the calcium gets delivered to the bone
cells,” says Stayton. “Without D and magnesium, you have elemental calcium in

Ellis and Gordon confirm Stayton’s
analysis. “But when taken properly,
calcium can go a long way toward preventing osteoporosis,” says Ellis. “And
the problem with osteoporosis is that it’s

treatable, but not reversible. That’s when
we need osteoporosis medications.”
Fosamax, Boniva, Reclast and Actonel
are bisphosphonates; Forteo is a parathyroid hormone; and Prolia is a RANK
ligand inhibitor. The purpose of all the
drugs is the same: to manipulate the
regeneration-replacement cycle of bone
cells. The medications do that in basically
the same ways—although some are stronger than others, Gordon says.
While ads featuring Sally Field and
Blythe Danner promote the bimonthly or monthly medications as easier to
take than daily Fosamax, they also can
have side effects. Those side effects—
including femur fractures and breakages
in other bones—have caused concern
among post-menopausal women.
For many, the side effects may have
been exacerbated. “When the drugs were
introduced, we were misled by the drug
companies and told to treat everyone
with osteopenia—and right away, before
it advanced to osteoporosis,” Gordon
says. “As a result, people have been on
these medications for longer than perhaps they need to be, so some of the side
effects may be more pronounced.”

Evidence of that is slowly becoming
public. In May of this year, the Food and
Drug Administration issued an analysis
of bisphosphonates, suggesting—but not
mandating—that patients should be on
the medications for no more than five
years. That comes as an unwelcome shock
to patients who’ve been on Fosamax for
up to 10 years, Ellis says.
And her practice has made changes.
“We reevaluated who was really at risk,”
she says. “Certainly, that was done on a
case-by-case basis. But we no longer suggest that everyone with osteopenia goes
on osteoporosis medication.”
Which medication remains an issue.
Ellis still prescribes Fosamax for her
patients. She believes the drug’s benefits
outweigh the risks, which happen in an
extremely low percentage of the population. She only prescribes Prolia, Boniva,

Sound opinions on age-related hearing loss.
What? WHAT??
Sound familiar? The Journal of the American Medical
Association reports that agerelated hearing loss is the
third most prevalent chronic
condition in seniors. Between
25 and 40 percent of people
65 years or older are hearing
impaired.
Yet, many of them resist
getting hearing aids. “There’s
a stigma surrounding hearing
aids—that they’re bulky, stick
out and make people look
old,” says Dr. Andrew Chuma
of Chester County Otolaryngology and Allergy Associates in Kennett Square. “But
new technology has made a wide variety of hearing aids
possible—many of which sit in the ear or just behind it and
are almost invisible.”
Chuma knows this first-hand. Born with otosclerosis,
he wears a hearing aid. “Mine sits behind the ear; there’s
a thin wire that leads to my ear drum, which is almost
invisible,” says Chuma.
Often, people don’t realize the severity of their impairment. “It’s a gradual loss and the brain compensates for

Presbycusis is the medical term for
age-related hearing loss, and it’s a problem of degenerating
cells. “There are minuscule hair cells in the ear that catch
sound waves, evaluate them and send that information to
the brain,” says Dr. Richard Herman of Otolaryngology
Plastic Surgery Associates, with offices in Doylestown,
Lansdale and Sellersville. “The degeneration of those hair
cells causes the loss in auditory processing.”
And it’s not just an inconvenience. Presbycusis is

frequencies. People can’t hear some of what’s said within
words and sentences; the brain loses the ability to discriminate between those sounds and process them.”
When caught and treated early, presbycusis can be
halted and sometimes reversed. With time and the right
hearing aids, the brain might even relearn to distinguish
sounds. “It could take months or years to recover, and if
the loss is too severe, it might not happen,” says Chuma.
“But it is possible for the brain to retrain itself to hear
sounds. That’s only possible if the hearing aid is worn all
the time so the brain hears background noise, complex
sentences and the rest of the sound in the world. And, of
course, the right hearing aid is required.”
Hearing aids start at about $1,000 an ear, topping out at
more than $2,500. They usually need to be replaced every
five to seven years. Manufacturers continually update their
technology and introduce new models, then stop making
replacement parts for older ones.
Hearing aids to alleviate presbycusis aren’t covered by
most health-insurance plans, Medicaid or Medicare. But
reimbursements are offered by the U.S. Department of
Veterans Affairs, the Federal Employees Health Benefits
Program and some teachers’ unions. “I wish that more
people would investigate hearing aids,” says Herman.
“They vastly improve quality of life.”
