12. RELEASE As additional consideration for the student’s instruction, the undersigned hereby releases the N.C. Academy of Dance Arts, Inc. from liability for injuries to the person or property of the student which may occur while participating in activities of the School. The undersigned further agrees to indemnify the N.C. Academy of Dance Arts, Inc. in the event any claims are asserted against it arising from the student’s participation in the activities of the School or the affiliated company. 13. I release media rights for NCADA to use any photos, videos, or _DVD’s of my child for publicity for the N.C. Academy of Dance Arts, Inc. _________ (initial, if agree). We, the undersigned, have read and agree to adhere to the terms and policies set forth by NCADA for the 2016-2017 season. MONTHLY TUITION is due the 1st day of each month. 2. TUITION PAYMENTS must be made by cash, check, money order, Master Card or Visa. Please Note: Checks will not be accepted for payments after April 30th, 2017. All _ accounts must have a zero balance by May 8, 2017. 3. RETURN CHECKS are charged a $35.00 check fee and must be paid by cash, money order, or credit cards. 4. DECLINED CREDIT CARDS (2nd offense) are charged a $25.00 fee and must be paid by cash, money order, or a valid credit card. 5. LATE FEE CHARGE of $25.00 will be charged to accounts with payments received after the 15th of each month. 6. COSTUME POLICY dictates that one costume per child per class is ordered by January 20th. Each student is responsible for costume payment unless NCADA is notified in writing by January 15th of student non-participation. 7. COSTUME DEPOSIT (non-refundable) is charged to each account on March 1st. The following rates will apply: • $ 50.00 deposit (partial payment) for 1 class ... 1 costume • $100.00 deposit (partial payment) for 2 classes ... 2 costumes • $150.00 deposit (partial payment) for 3 classes ... 3 costumes • $200.00 deposit (partial payment) for 4 classes ... 4 costumes • $250.00 deposit (partial payment) for 5 classes ... 5 or more costumes 8. COSTUME BALANCE (remaining balance due) is charged to each account on April 15th. 9. PRODUCTION FEE of $50.00 is charged to each account on February 1st. 10. WITHDRAWAL POLICY states that a student that wishes to withdraw must submit a written notice to the office manager of NCADA. Verbal acknowledgement is not acceptable. 11. WITHDRAWAL FEE will apply: • $ 65.00 withdrawal fee for 1 NCADA class • $100.00 withdrawal fee for 2 NCADA classes • $140.00 withdrawal fee for 3 NCADA classes • $160.00 withdrawal fee for 4 or more NCADA classes • $200.00 withdrawal fee for Dance Arts Theatre Company members • If written notification is received after the 15th of the month... the account will also be charged tuition for the month that notification of withdrawal is given. Whether your child wishes to dance for fun, or is an aspiring young professional...we can help make those dreams come true. Carefully Designed Classes. Our curriculum offers classes in ballet, pointe, jazz, modern, tap, and hip-hop for children through adults. Classes are designed to be foundation blocks for our students...providing them with a FUN and structured syllabus that enables growth. NCADA’s curriculum also includes special sessions of TODDLERS and TOES, adult classes and exciting summer programs. Talented Instructors. Each instructor is a professional dancer and/or educator and a caring teacher. We believe strongly in combining a love of dance with innovative education methods that enable the students to reach their potential. Each season guest artists from across the United States come and share their talents with our students. The N. C. Academy of Dance Arts home is a 10,000 sq. ft. building. There are 5 state-of-the-art studios complete with “floating” floor systems and “Bose” surround sound systems. Spacious lobbies, custom designed dressing rooms, a student lounge, and media center are for the enjoyment of both parents and students. One of the most exciting aspects of dance is performance. Throughout the year the studio provides various opportunities for students to perform and participate in small studio productions and benefit performances for local fundraisers. A special highlight of each year is the annual fast-paced and entertaining student production held at Wright Auditorium, E.C.U. School of the Arts, N.C. Governor’s School, the Carolina Ballet, the American Dance Festival, and the Sarasota Ballet. Dance Arts Theatre. Dance Arts Theatre was founded in 1985 as a non-profit dance company.
