CHWN is a clinically integrated healthcare network that will provide payer contracting and care management services to participating providers, including all of Centegra hospitals, all Centegra Physician Care physicians, all independent physicians who contract with CHWN, and all of the Centegra hospitals. CHWN has also contracted with non-Centegra affiliates to provide the best continuum of care and access for patients across our broad and diverse service area. What does this mean to you? CHWN represents a new, high-quality alternative for your healthcare needs. Clinical integration enables collaboration among different healthcare providers and locations to improve the quality, efficiency and coordination of the care you receive. Physicians participating in CHWN will support and comply with evidence-based medical guidelines, preventive care and chronic care management initiatives designed to keep people healthy and help patients to better manage chronic conditions. On January 1, 2013, CHWN will become a participating medical group in the Blue Cross Blue Shield of Illinois HMO Network. O ver the years, John and Michele Carbonara struggled with excess weight. At times they managed to diet away pounds; however, the weight always returned. “We’ve tried so many things that didn’t work,” Michele says. Their family histories weighed on their minds—each lost a father to heart disease at an early age. John had also developed a heart condition and Type 2 diabetes. “I was sick and tired of feeling sick and tired,” John says. That was a turning point for the Carbonaras. They decided they wanted to be around a lot longer for their teenage daughter and enjoy life more. In the winter of 2011, both John and Michele had bariatric surgery as a part of the Centegra Weight-Loss Surgery program—the start of a journey toward a healthier weight and a new life. “From the bottom of my feet to the top of my head, I feel much more alive,” John says. CENTER OF SUPPORT Centegra’s weight-loss surgery program, led by Amir Heydari, MD, FACS, is a local, nationally recognized option for Amir Heydari, eligible people unable to MD, FACS achieve significant weight loss with diet and exercise alone. You might be a candidate if you’re more than 100 pounds overweight or if you have obesity-related health problems, such as diabetes, heart disease or sleep apnea. Both John and Michele had Roux-en-Y gastric bypass, which promotes weight loss by restricting the amount of food the stomach holds and the amount of nutrients and calories absorbed. BEFORE As part of the program, Michele and John received individualized support from the Centegra team before, during and after surgery. They learned how to make dietary changes. A fitness trainer tailored an exercise plan to meet their needs. They also attend support group meetings—a key component of the program. Throughout their journey, John and Michele also inspired each other. “We were each other’s cheerleaders,” Michele says. AMAZING TURNAROUND Before surgery, John weighed 374 pounds and had several weight-related health conditions. Michele, at 377 pounds, had high blood pressure and back pain. Today, John and Michele weigh 180 and 164 pounds, respectively. John’s heart condition has improved, and he no longer has to take diabetes medication or wear a breathing mask at night for sleep apnea. Michele says they’re doing things “we never thought possible.” Both are avid exercisers—John runs nearly three miles a day. Michele enjoys running, too, and has Visit centegra.org. AFTER even tried indoor rock climbing. Michele also appreciates everyday changes— like being able to sit in an airplane seat comfortably or walk without getting winded. “My whole life has changed because of this,” she says. An especially memorable moment came at Disney World, where, to celebrate John’s 45th birthday, they ran a 5K race with their daughter. The fact that the Carbonaras turned their lives around so dramatically is an inspiration. They didn’t do it alone. They had each other, their family and the Centegra Weight-Loss Surgery program team. “They gave me the opportunity to save my own life,” John says about the center’s support team. Check out the Centegra Weight-Loss Surgery Support Group on Facebook! 5 THE CENTEGRA BACK AND SPINE CENTER Reducing pain, getting back to life F red Goebel knew he needed help for his back pain when it started to interfere with his golf game. At 76, he golfed two to three times a week, and the day after, he often couldn’t bend to put on his shoes. MAKING THE CALL When Goebel began to wake in the middle of the night from pain and it started radiating to his hip, he called the Centegra Back and Spine Center. “I’d seen the signs about the new center, and they got me in within a few days,” Goebel says. “The doctor gave me a plan of action and explained what we were going to do and when we were going to do it.” The center’s medical director, Sandhya Meesala, MD, a physiatrist, assessed Goebel and ordered x-rays and an MRI to see why he was experiencing radiating pain. She diagnosed him with Sandhya arthritis in his back. Meesala, MD “I was impressed with the doctor because she took a lot of time and she was very easy to understand,” Goebel says. “She brought her recommendations and her explanations down to my level. She repeated everything at least three or four times so I understood what she wanted me to do. She took good care of me.” A PLAN OF ACTION Dr. Meesala prescribed physical therapy and medication for his pain. Goebel began physical therapy by asking his therapist to teach him exercises he would use long after his prescribed treatments were complete. “My main goal was to avoid stiffening up,” Goebel says. “I said, ‘I’m willing to do exercises every day or every other day if I can stay flexible.’” Goebel’s pain improved through physical therapy and, as he’d requested, his physical therapist gave him daily exercises to keep him in shape. “I got out of it just what I’d hoped to learn,” he says. “I’m feeling a lot better.” Now that his back has improved, Goebel is back to golfing and drinking coffee with friends at a nearby restaurant every morning. Start by keeping the cold air out. Insulate your home’s walls and attic, and caulk and weather-strip doors and windows. Get your car ready by having the antifreeze, tires, defroster, battery and other essential components checked. ESSENTIAL SUPPLIES In case the power goes out, the Federal Emergency Management Agency recommends having: w A flashlight and extra batteries w A battery-operated or hand-crank radio w At least a three-day supply of food, primarily items that don’t need cooking or refrigeration w A first aid kit and a week’s supply of prescription medications w Extra blankets and sleeping bags It’s also a good idea to have drinking water available, since extreme cold can freeze and sometimes break water pipes. Protect pipes by wrapping them in insulation or newspaper and plastic. Let faucets drip so that water keeps flowing. Open cabinet doors under sinks to let more heat reach the pipes. DRESS WARMLY It’s best to stay inside during severe winter weather, but if you must go out, dress in layers with a water-repellent outer layer. If you must drive, take a cellphone, shovel, window scraper, flashlight and blanket. Have booster cables in case your battery dies and some sand or cat litter for traction. If you’re trapped in your car during a winter storm, staying put is often the safest choice. Tie a bright cloth to the radio antenna as a signal for help. Keep warm by turning on the engine and heater about 10 minutes per hour. Keep the exhaust clear of snow, and crack a window for fresh air. NUTRITION What about nutrition? “During periods of extreme cold, keep warm with well-balanced meals,” says William A. Stinson, William A. MD, an internal medicine Stinson, MD specialist with Centegra Physician Care. “Avoid alcoholic drinks—they only cause your body to lose heat faster. Try sweet, warm beverages instead to help maintain your body temperature. While it may be pretty to look at, snow can literally be a pain to shovel. Every year, thousands of Americans are treated at hospital emergency departments, doctors’ offices and clinics for injuries sustained while shoveling snow. Removing snow can be especially hard on back, shoulder and arm muscles because it involves a lot of bending and heavy lifting. It can also be hard on the heart: Older adults face an increased risk for having heart problems while shoveling. It’s not unusual for people to slip or fall—or get hit by the shovel—when shoveling snow. Remove it safely When it’s time to tackle that snowcovered walkway or driveway, follow these suggestions for safe shoveling: w Dress appropriately. Wear light, waterrepellent clothing; a hat; gloves; and warm socks. Put on shoes or boots with good traction to avoid falling. w Never use a shovel that is too heavy or too long. w Clear snow early and often. It’s easier to remove a light covering of snow from the ground than it is to clear packed, heavy snow. w Take plenty of breaks, and drink lots of water. w If you feel any pain, stop shoveling right away. If you have chest pain, seek medical attention immediately. w Push snow instead of lifting it. w Avoid throwing snow over your shoulder or to the side because it can stress your back. Seek advice If you have any questions about your snowshoveling fitness, you should talk to your doctor—especially if you’re older than 40, don’t exercise regularly or have a history of heart problems. CentegraHealthSystem 23 Teeing up for healthcare Golfer organizes for Centegra Gavers Breast Center L ocal businesses supported the McHenry Country Club Ladies Assocation’s annual Rally for a Cure day in droves this year. It totaled a generous donation to the Centegra Gavers Breast Center, thanks to Diane Spengel. Diane is an association member and breast cancer survivor. A PERSONAL MISSION This annual charity event supports national and regional breast cancer organizations. However this year, Spengel had the idea to include a local organization: the Centegra Gavers Breast Center. “It’s really important to support healthcare in our community,” she says. “I had excellent care at Centegra during my cancer treatment, and I wanted to give something back.” Hole sponsorships seemed to be a perfect way to fit local giving into the event, but only three weeks remained before the tournament. “I just jumped into my car and drove to every business I could think of,” Spengel says. THE COMMUNITY STEPS UP Spengel’s commitment paid off—as did Centegra’s reputation for excellent healthcare. “Nearly everyone was familiar with the Centegra Gavers Breast Center,” she says. Spengel ended up with many more than 18 hole sponsors. Just printing and staking all the signs could have taken a large part of the donations, but a local printing business offered to make and donate the signs. That meant 100 percent of the funds could go to the Centegra Gavers Breast Center. Spengel had set her goal at raising 24 CentegraHealthSystem $5,000 for the center. Hitting that mark was a milestone. Yet the checks kept on coming. “Members at the Country Club were amazed at how much money I raised,” she says. Eventually she was able to present the Centegra Health System Foundation with a check for $8,400, designated for the Centegra Gavers Breast Center. Their donation will help buy a tomosynthesis machine, the latest technology for mammography. It gives a more accurate view of the breast to better identify abnormalities, leading to better detection and fewer follow-up procedures. It is especially helpful to younger women with dense breast tissue. HOW YOU CAN HELP Spengel’s efforts were inspired by the excellent care she received during her breast cancer treatment. “All the doctors at Centegra were great,” she says. “They are very knowledgeable, and they take time to answer all your questions. They truly care about their patients.” That’s the kind of healthcare McHenry County deserves, and supporting charity events is a great way to ensure that it’s there when you need it.
