The County is issuing Addendum #2 to clarify the proposal, and the terms and conditions. The following revisions of the referenced portions of the solicitation documents for the above named project replace or supplement the previously issued document. Where any original item, term or requirement is not specifically amended, voided or superseded by this addendum, it will remain in effect. Proposals offered directly from manufacturers must indicate if a local dealer/representative will be involved. This clarification is being provided to all known respondents. All addenda and bid documents are also available online at www.claytoncountyga.gov/departments/central-services/view-bids-and-proposals.aspx. This Addendum #2 is an integral part of RFP #14-10. This Addendum must be completed, signed, and attached to the RFP to acknowledge receipt of Addendum. Boulevard 9157 Tara Boulevard Zip 30250 30236 Qty 13 283 3-QUESTIONS AND ANSWERS Question #1 Will the County provide the minutes, messages and revenue of the jail and prison for the past 12 months? To obtain this information, the request must be made in the form of an Open Records Act Request. You must submit a request in writing to Clayton County Sheriff’s Office, 9157 Tara Boulevard, Jonesboro, GA 30236 along with all your contact information. Question #2 Please confirm, should the response be directed to Director Theodis R. Locke? If not, to whom? Will the County provide a telephone number and email address for the UPS label. Yes, you may direct the RFP to the attention of Director Locke. As stated on page one (1) of the RFP, all submittals are to be delivered to Clayton County Central Services, 7994 North McDonough Street, Jonesboro, GA 30236, (770) 477-3587. RFP #14-10 Inmate Pay Phone Services for the Clayton County Jail Page 2 of 7 Question #3 Does the County want the same quantity of cost proposals as the technical proposal - 1 original and 7 copies? Does the County want a CD of just the cost proposal also? Proposer must submit one (1) cost proposal form per section, in a sealed envelope with the original proposal. The CD must contain all of the submitted documents, with the exception of the cost proposal forms. Question #4 On Pg 46, of the RFP, Successful Subcontractor form has a field for “Vendor Number”. Where does this number come from? The Vendor Number is for vendors that have submitted a Clayton County Vendors Application. If you would like to be in our system, you can find the Clayton County Vendors Application online at www.claytoncountyga.gov. You are not required to have a vendor number to submit a proposal. Question #5 Can the 1” binder size be increased to 2” binder? The RFP is a ½” in itself. You are not required to submit the entire RFP with your proposal, only the required documents listed on the Submittal checklist and your technical responses. Please submit a one inch (1”) binder per submittal (one for Prison and one for Jail). Question #6 Is the sentence listed under 4.3.4a. and under 5.3.3b. correct? Automatically, each data, at a time selected by the Institution System Administrator, and immediately following entry of the data, if elected by the Institution System Manager. Yes, it’s referring to the time frame for implementing changes of the database. Question #7 Please clarify when the performance and payments bonds should be included with response. The Successful bidder will be required to provide a Performance Bond and a Payment Bond within fourteen (14) days from receipt of the Executed Agreement (see section 32.1 of this RFP). Bidders are only required to submit a Surety Letter with the Proposal. Question #8 Please provide as much data as possible [# of Calls per month, # of minutes per month, total revenue per month] for each of the two facilities. Please see answer to question number one (#1) Question #9 Please provide a copy of the current inmate phone service agreement(s). Please see answer to question number one (#1) Question#10 Please provide an average of monthly commissions received over the past year from the current vendors and copies of commission statements (if available.) Please see answer to question number one (#1) Question#11 What fees, in addition to per call and per minute charges, are currently being added to the cost of calling for both facilities? Examples of such fees include bill statement fees, account funding fees, account set up fees, account refund fees, account closure fees, etc. Please see answer to question number one (#1) Question#12 Is the inmate trust account managed through the commissary system or the Jail Management System or other system? If other, please specify. Please provide this information for both facilities The Sheriff’s Office manages the inmate trust funds for the Clayton County Jail. Bealls Communications Group manages the inmate trust account for the Clayton County Prison. Question#13 Who provides the Jail Management System at each of the facilities? The Sheriff’s Office uses the AS400 County system. The Clayton County Prison inputs the inmate phone numbers using the data based provided by the current vendor. Question#14 Please provide the number and location of the pay phones required. Please see section two (2) of this Addendum. The list of phone numbers will be provided to the successful bidder.
