1. The purchase of The Ottawa Hospital Staff Lottery
ticket ($250) binds you to the rules of the Lottery.
2. The Ottawa Hospital Staff Lottery is open to staff,
physicians, retired employees and volunteers of
each campus of The Ottawa Hospital, the Ottawa
Hospital Research Institute and the University of
Ottawa Heart Institute.
3. Tax-free prizes are awarded as follows:
• “Jump-Start” Draw
– October 1, 2012 – 1 draw of $5,000
• Weekly “Stat” Draws
– $500 each on Monday, Wednesday and Friday
every week, October 1, 2012 to August 30, 2013
• Monthly “Infusion” Draws
– $1,000 on the first Monday of the month
October 2012 to August 2013
• Seasonal “Survivor” Draws
– $5,000 each
– December 10, 2012, April 19, 2013 and
August 30, 2013
4. No tax receipt can be issued for the purchase
of The Ottawa Hospital Staff Lottery ticket.
5. The Ottawa Hospital Staff Lottery is authorized
and licensed by the Alcohol and Gaming
Commission of Ontario Licence #4990.

6. The Ottawa Hospital Staff Lottery has been
approved by the Board of Directors of The Ottawa
Hospital Foundation for the purpose of raising funds
for patient care and research at The Ottawa Hospital.

7. Draws will take place at 9:00 a.m. as per the draw
schedule in the Foundation office at the Parkdale
Clinic located at the Civic Campus. Should the office
be closed on that particular day, the draw will be
conducted on the following business day at 9:00
a.m. The Campaign “Jump-Start” Draw will take
place at noon on October 1, 2012.
8. The winner is the person whose number is drawn.
The winner will be notified by phone that they have
won; their name and ticket number will be printed in
The Ottawa Hospital Journal and a local newspaper,
posted on The Ottawa Hospital InfoNet and displayed
on The Ottawa Hospital Staff Lottery Winners’
Boards. These Boards are located across from the
information desk in the main entrance of the Parkdale
Clinic and inside the Greenery Cafeteria at the Civic
Campus; outside the Human Resources Department
and the main cafeteria at the General Campus; and
inside the entrance of the main cafeteria at the
Riverside Campus. Winners’ names will be updated
on The Ottawa Hospital Staff Lottery “Winners’
Line,” 613-798-5555, ext. 13800 (press 1 for English,
2 for French). The winner must present a photo
identification card (e.g., employee ID, drivers’ licence)
to claim their prize. Prizes are to be picked
up at the Foundation offices at the Parkdale Clinic
at the Civic Campus and on the main floor of The
Ottawa Hospital Cancer Centre at the General
Campus, and in the Administrative Suites on the
main level of the Riverside Campus, depending
on the winner’s preference.
9. Only tickets that have been sold will be entered
in the draw. That is, if 2,000 tickets are sold, each
ticket holder will have a 1 in 2,000 chance to win
each draw. Once the number has won, the ticket
will be returned to the drum, so that the winner
will be eligible for all remaining draws, including
those held on the same day.
10. In the event that two or more draws are held
on the same day, the draws will be in the order of
highest cash value first, followed by next highest.
11. Group purchases are permitted; however, only one
name will appear on the ticket. Members must work out
the details of payment and payout amongst themselves.
The Ottawa Hospital Foundation will play no role in
settling any disputes relating to group purchases. The
winning prize, in the case of a group purchase, will be
issued in the name of the ticket holder.
12. The Ottawa Hospital Staff Lottery is limited to
a maximum of 2,000 active tickets at any given time.
13. The Ottawa Hospital Foundation’s Board of
Directors will be the final arbiter in any disputes
related to the Lottery.
14. Unclaimed prizes after 12 months of the final
draw date will be returned to The Ottawa Hospital
Foundation.
15. The deadline for purchasing tickets for the
2012–2013 season is midnight, September 30, 2012.
16. Ticket purchaser must be at least 18 years of age.
