Combo Doubles Format with 6.0, 7.0, 8.0
and 9.0 levels represented.
• Female and Male Divisions
$40 per player or $80 per doubles team - Each Entry Includes:
• Lunch, Player Bag, Prizes and Raffles.
• One Complimentary voucher for qualifying session tickets to the BB&T Atlanta Open coming to
Atlanta July 20-28 at Atlantic Station
• One ticket to the Willett Honda Championship to be held Sunday July 21st at The Jason T. Harper Event
Center, Heritage Park in McDonough, Georgia. (Additional general admission tickets are available for $20)
All tournament proceeds will benefit The Willett Foundation and Henry County Wheelchair Sports Program

For, and in consideration of the USTA, The Willett Foundation, Willett Honda, Get Set Grow Event Management and Henry County Parks & Recreation, I the undersigned; my heirs; successors; and assigns, agree to release
and forever discharge the USTA, The Willett Foundation, Willett Honda, Get Set Grow Event Management and Henry County Parks & Recreation; their officers; employees, volunteers and agents from any and all liabilities;
demands or claims for loss or damage resulting from any injury or damage which may be sustained on account of my participation in the Willett Honda Regional Slam Tennis Tournament. I also consent to medical treatment
in case of emergency. I agree to full responsibility for payment of any fees incurred as a result of necessary medical treatment. Entry, participation or attendance during ALL competitions involved in the Willett Honda
Regional Slam Tennis constitutes permission to be photographed for possible publicity, promotional or media purposes and constitutes a waiver of any and all claims for compensation from all sponsoring agencies. I hereby
agree to abide by the governed by the RULES OF TENNIS, “The Code” as contained in the 2013 Friend at Court, The USTA Handbook of Tennis Rules and Regulations, and the USTA Combo Doubles competition guidelines.
