Within the next three months, agency nurses will be recruited to work in Critical Care, ED, and the Medical/Surgical/Cardiac areas from January 5, 2015 through April 6, 2015. These nurses will be assigned to the Central Based Pool. They will start and end their coverage as a group. At the beginning of a shift, these travelers will be assigned to the units with the greatest need. The more challenging your day, the more of My Power I place at your disposal. You seem to think that I empower you equally each day, but this is not so. Your tendency upon awakening is to assess the difficulties ahead of you, measuring them against your average strength. This is an exercise in unreality. I know what each of your days will contain, and I empower you accordingly. The degree to which I strengthen you on a given day is based mainly on two variables: the difficulty of your circumstances, and your willingness to depend on Me for help. Try to view challenging days as opportunities to receive more of My Power than usual. Look to Me for all that you need, and watch to see what I will do. As your day, so shall your strength be. Magnet Source of Evidence SE 4EO Ebola Algorithm Magnet Source of Evidence EP 6 Magnet Source of Evidence EP 6, EP 20EO Is it Flu or Ebola The Centers for Disease Control and Prevention (CDC) published a guide for healthcare providers to help distinguish between the Flu and Ebola. Flu Ebola The flu is a common, contagious respi ratory illness caused the influenza virus. It can be mild to severe and complications can lead to death. The flu is spread mainly through droplets when a person sneezes, coughs, or talks. It can be spread before, during , and after a person is sick. Signs and symptoms can develop within two days after exposure and come on quickly. Symptoms: fever, headache, muscle or body aches, fatigue, cough, sore throat, runny or stuffy nose Ebola is rare and deadly disease caused by the Ebola virus. February 6, 2015 Ebola can only be spread through direct contact with blood or body fluids. It is not spread in the air or by food or water. Signs and symptoms can appear 2-21 days after exposure. The average time is 10 days and symptoms develop over several days. Ebola can’t be spread until the symptoms are present. Symptoms: fever, severe headache, muscle pain, fatigue, vomiting and diarrhea after 3-6 days, sever weakness, stomach pain, unexplained bleeding or bruising Jack Stephens Heart Institute will sponsor the Let’s Talk Heart Symposium The Symposium will held at CHI SVI Center for Health Education Retrieved from http://www.cdc.gov/vhf/ebola/index.html Ebola Preparedness Magnet Source of Evidence EP 6, EP 20EO CHI St. Vincent has been taking steps to ensure that every precaution is in place to effectively care for an Ebola patient should the occasion arise. There have been Ebola town halls in which Infection Prevention and Administrators provided education on the Ebola virus. Emergency Department coworkers have already been trained on safely donning and doffing PPE. Phase II of an education plan is now being rolled out to critical care units, Radiology, Respiratory Therapy, and EVS. More information will be coming soon. Nursing Contact Hours Anyone interested in volunteering to care for an Ebola patient should contact Tyler McDonald, Director of Nursing Operations, at [email protected]. Tyler will need your name, contact information, position (RN, PCT, etc.), facility, department, and manager’s name. Focused education and proper PPE techniques will be provided to the volunteers. Members of CHI SVI staff and Clinical Educators went through several scenarios with the manikins, such as PCA overdose, vaginal delivery, and cardiac arrest. The Education Department is seeking further funding to expand the simulation labs and add more equipment. Simulation provides hands-on training that helps nurses successfully navigate emergent situations and improve patient outcomes. CHI St. Vincent is one of the few non-teaching hospitals in the state that has high fidelity simulation manikins. We are the first in the nation to own the automatic birthing simulator! LRBNAA Holistic Nursing Magnet Source of Evidence OO13 The Little Rock Black Nurses Association of Arkansas sponsored a conference, Holistic Nursing: Mind, Body Spirit & Finances, October 25, 2014. There were over forty nurses that attended. February 6, 2015 Speakers were Marie Mesidor, Ph. D., Clinical Psychologist; Benjamin Moore, Financial Advisor; Keiona Middleton, Certified Personal Trainer; and Roderick McLean, Ph. D., United Methodist Minister. Jack Stephens Heart Institute will sponsor the Let’s Talk Heart Symposium Patient Safety Huddle Facts Magnet Source of Evidence EP 22EO Patient safety during hospitalization is of growing concern for both healthcare providers and patients. Verbal communication is essential when transferring information about the patient’s care. However, information conveyed can easily be lost or misinterpreted at each shift transition. Safety is a key component of CHI St. Vincent’s FY15 goals. In October 2014, four units in the health system piloted unit huddles to improve communication across shifts and patient safety outcomes on nursing units. The unit huddle focuses on detection of risk factors and prevention of harm. Huddles create a forum for examining nursing processes, troubleshooting, and resolution of conflict in patient care. The Symposium will held at CHI SVI Center for Health Education More information will be coming soon. Unit huddles will be implemented on inpatient nursing units across all 4 hospitals beginning December 2014. Training is currently in progress in those areas. These huddles are one way that CHI St. Vincent is working to identify and resolve issues related to patient safety. The huddle process is part of the CHI Clinical Process Excellence (CPE) initiative. It is a way to engage all employees in two-way communication to improve quality and safety. The huddles will focus on fall risks, CVL/PICC, Foley management, restraints, pressure ulcers, service recovery, and other staff identified concerns. Nursing Contact Hours Our FY2015 goal is zero events of harm caused by preventable mistakes. Patient Safety Huddles will help CHI St. Vincent accomplish is important goal! Closed Safety Board Open Safety Board Magnet Source of Evidence SE 4EO Magnet Source of Evidence EP 18EO, OO3 A safety story is published and distributed each month to share knowledge an lessons learned from events that have occurred. To obtain the safety story of the month: Ask you Manager/ Director ************ Ask your Unit Council Chair who represents your unit at Congress********** Go to the CHI St. Vincent Insider. Before a safety story is shard and distributed, the Quality/Safety Council reviews the story to ensure that the employee, patients and physicians can not be identified. The name of the author who submits the story is only published with permission of that author. Anyone can and should report near miss safety events or true safety events that cause harm. To complete an IRIS report go to the CHI St. Vincent Insider >>Web Apps (at top of page) >> IRIS Report. Near Miss Safety Event A nurse was in a patient’s room charting (patient #1). Another patient (patient #2) called for her. She went to that room to check on the patient. She had cared for this patient for several days and knew the medications the patient was prescribed. It was time for a new IV to be hung on patient #2. When she returned to the room with the IV, she scanned medication and checked two patient identifiers and scanned the patient armband. The computer gave her an error message. She looked at the MAR again and discovered that it was the first patient’s chart that was pulled up. When she logged in to the computer in patient #2’s room, patient #1’s chart came up since she had not logged out in patient #1’s room. By scanning the drug, checking two identifiers, and scanning the patient armband, the computer stopped her from charting on or administering a medication to the wrong patient. Questions for thought: 1. What if she had not scanned the medication and the patient’s armband? 2. What if she had administer a medication to patient #2 that was intended for patient #1? 3. What if another nurse entered patient #1’s room and started charting?
